In the last decade the health of young people in Ukraine as well as in many other countries of the world significantly deteriorated according to the results of screening tests. Because of this the main task of prophylactic medicine is development and introduction of new methods of early diagnostics of diseases. The aim of the work to assess the peculiarities of adaptation of students with different types of vegetative regulation. Material and methods: 415 second year students of Lugansk State Medical University were tested. The mechanisms of physiological functions of the body regulation were assessed by registration of cardiac rhythm variability (CRV). Analysis of CRV was performed by three methods: statistical, geometrical, and spectral analysis. The express method proposed by Shlyk, N.I. (2009) was later used to determine the prevailing type of vegetative regulation. To assess adaptability the method of R.M. Baevskij (1979) was used. Results: In the result of investigation 4 types of functional states of regulatory systems were determined: I type 295 (71 %) of tested, II in 14 (3 %), III in 96 (23 %) and IV only in 10 (2%). I type of functional state of regulatory systems in the wakeful state at rest was characterized by moderate prevalence of central regulation of heart rhythm (MPCR), decreased activity of autonomic regulation. It was noticed that some of the parameters of CRV (like RR, SDNN, RMSSD and pNN50) which characterize autonomic regulation, were statistically lowere (р≤0.01) compared to type III, and some (like AMo and SI) higher, in both female and male students. Summarized spectrum capacity and components of its wave structure (HF, LF, VLF) which characterize central regulation were statistically significantly lower (р≤0.01) for the I type of regulation compared to the III one which is characterized by moderate prevalence of autonomic regulation of the heart rhythm. In female students with type I autonomic regulation compared to male students such CRV parameters as RR and AМo were statistically significantly lower (р≤0.05), while the parameters of total potency (TP) and high frequency waves (HF) were higher (р≤0.05), which could point to more centralized heart rhythm regulation in male compared to female. The evaluation of regulatory systems overstress allowed to determine the state of adaptation and risk of overstress or breakdown in students with different types of autonomic regulation. The aim of our study was to determine the significance of lymphocyte-platelet adhesion (LPA), interleukins, transforming included 139 pregnant women aged between 17 and 27 years (21.3±4.22 years). GH was diagnosed in 119 women after 20 weeks of pregnancy. 20 patients (control group) were with physiological course of pregnancy. The distribution of patients by groups was carried out according to the level of blood pressure (BP) in accordance with ICD-10 (Geneva, WHO, 2002). The survey was conducted at the moment of detection pregnancy from 7 to 10 weeks and in dynamics of I, II and III trimesters of gestation. In the dynamics of gestation, were studied the number of desquamated endothelial cells circulating in the systemic circulation (CECs), nitrates levels, the adhesion of platelets by estimation their ability to form co-aggregates with lymphocytes by determining the percentage of lymphocytes aggregates with thrombocytes (lymphocyte-platelet plugs), at risk of hypertensive disorders, especially after 20-22 weeks of gestation and later, lymphocytes ability to platelets adhesion is rose, the concentrations of pro-inflammatory cytokines and NO level are increased. Cíle: Pseudoaneurysma intrarenálních arterií (RAP) může představovat život ohrožující komplikaci, k níž může dojít po parciální resekci ledviny. Autoři hodnotí incidenci RAP, u něhož zatím nedošlo k ruptuře (detekováno pomocí trojrozměrné CT angiografie – CTA), časně po minimálně invazivní parciální resekci ledviny (včetně laparoskopické a robotické operace). Dále hodnotí možnost využití nefrometrického skóre při predikci vzniku RAP. Metody: V období od února 2012 do listopadu 2013 podstoupilo 101 pacientů s renálními tumory minimálně invazivní parciální resekci ledviny. Tři až čtyři dny po operaci byla provedena CTA a radiologové (zaslepeně) diagnostikovali RAP. Pomocí logistického regresního modelu analyzovali autoři faktory, které mohou potenciálně ovlivňovat vznik RAP. Výsledky: Míra incidence RAP časně po minimálně invazivní parciální resekci (MIPR) byla nečekaně vysoká – 21,7 % (detekováno pomocí CTA). U pacientů s RAP byl přítomen signifikantně větší tumor (p = 0,02) a významně vyšší skóre parametru N indikující blízkost tumoru k renálnímu sinu (p = 0,01) než u pacientů, u nichž nedošlo ke vzniku RAP. Multivariátní analýza však prokázala, že ani celkové nefrometrické skóre ani hodnota jednotlivých komponent neměly významný vliv na vznik RAP. Velikost tumoru představovala jediný významný nezávislý prediktor vzniku RAP po minimálně invazivní resekci ledviny. Závěr: Parametr N nefrometrického skóre představuje významný prediktor vzniku RAP podle univariátní analýzy, nikoli však podle multivariátní analýzy. Komponentu N nefrometrického skóre tedy lze považovat za jeden z faktorů užitečných pro predikci vzniku RAP., Objectives: Renal artery pseudoaneurysm (RAP) can be a life threatening complication after partial nephrectomy. We investigated the incidence of unruptured RAP detected by 3-dimensional computed tomography arteriography (CTA) in the early period after minimally invasive partial nephrectomy (MIPN), including laparoscopic and robotic partial nephrectomy. We also examined the usefulness of nephrometry scoring system to predict the occurrence of RAP. Methods: From February 2012 to November 2013, 101 patients underwent MIPN for renal masses. CTA was performed 3–4 days after surgery, and the radiologists made a diagnosis of RAP in the blinded manner. The factors influencing the occurrence of RAP were analyzed with the logistic regression model. Results: The incidence of RAP was unexpectedly high at 21.7% when detected by CTA in the early period after MIPN. The RAP group showed a significantly larger tumor size (p = 0.02) and a significantly higher N component score (p = 0.01) compared to the No-RAP group. However, Neither the sum of nephrometry score nor each component showed significant influence on the occurrence of RAP according to the multivariate analysis. Tumor size was the only significant independent factor to predict the occurrence of RAP after MIPN. Conclusions: The N component of the nephrometry scoring system is a significant predictor for RAP by the univariate analysis, but not by the multivariate analysis. Úvod: Cerebrovaskulární rezerva (CVR) je jedním z důležitých parametrů k realizaci a monitoraci úspěšnosti angiochirurgické intervence u pacientů po ischemické cévní mozkové příhodě, tranzitorní ischemické atace nebo k upřesnění typu demence. Cíl: Cílem této práce je seznámit s provedením vazodilatačního testu s dipyridamolem jako možné alternativy farmakologické zátěže, s výskytem vedlejších účinků, získanými výsledky, jeho přednostmi a nedostatky. Soubor a metody: Byla provedena retrospektivní analýza 20 pacientů (8 žen, 12 mužů ve věkovém rozmezí 37–78 let) – 6 po TIA, 14 po ischemické cévní mozkové příhodě vyšetřených v průběhu dvou let. Snímání proběhlo na gamakameře Infinia Hawkeye s kolimátory fan beam technikou SPECT standardním způsobem za bazálních podmínek a v odstupu 1–13 dnů po farmakologické zátěži dipyridamolem (v 4minutové i.v. infuzi v dávce 0,84 mg/kg do 50 ml F1/1) s následnou i.v. aplikací 99mTc-HMPAO v 10. minutě. K potlačení vedlejší účinků zaznamenaných po dipyridamolu byl jako antidotum použit Syntophyllin i.v. Rozdíly bazální a pozátěžové studie byly hodnoceny vizuálně i semikvantitativně pomocí programu NeuroGamTMSegami a statistického parametrického mapování. Výsledky: 9 (45 %) pacientů mělo normální CVR, redukovaná se vyskytla u 11 pacientů (55 %). 8 pacientů (6 mužů, 2 ženy) s jednostrannou těsnou stenózou nebo okluzí vnitřní krkavice (ACI) bylo následně léčeno neurochirurgicky (karotická endarterektomie nebo arterio-arteriální extra-intrakraniální bypass). 6 (75 %) z těchto pacientů bylo s redukovanou CVR, 2 (25 %) měli normální CVR. Zbývajících 12 pacientů bylo léčeno konzervativně, 5 (42 %) z nich mělo redukovanou CVR (mírná redukce CVR nebo pacient rizikový k chirurgickému výkonu) a 7 (58 %) pacientů mělo normální CVR. Vedlejší účinky dipyridamolu (hypotenze, kašel, tlak na hrudi, bolest hlavy) byly zaznamenány u 5 pacientů, k ústupu potíží došlo během 2–5 minut. Závěr: Zátěžová perfuzní scintigrafie mozku s podáním dipyridamolu je vhodným testem k posouzení vaskulární rezervy mozku. Kontinuální tvorba výpotků jakékoliv etiologie zásadním způsobem ovlivňuje kvalitu života nemocných. Akumulace tekutiny v pleurální dutině postupně komprimuje plicní parenchym. Prvním příznakem bývá progrese námahové dušnosti, později se přidává i klidová dušnost. S nárůstem výpotku dochází ke kompresi plicního parenchymu s rozvojem plicní atelektázy. Nemocný je ohrožen vznikem zánětového onemocnění plic v nevzdušném plicním parenchymu. U diagnózy maligního onemocnění je riziko zánětových komplikací výrazně vyšší vzhledem k předchozí chemoterapii či radioterapii, která zásadním způsobem snižuje imunitu. Opakovaná tvorba výpotku se výrazně podílí na rozvoji terminálního stavu nádorové kachexie. Efektivní metodou léčby je chemická pleurodéza, která se užívá u vybraného spektra nemocných. Principem je navození aseptické pleuritidy. Nejefektivnější cestou aplikace léčiva je videotorakoskopie. V souboru bylo 47 nemocných, 29 (61,7 %) mužů a 18 (38,3 %) žen, s cytologicky prokázaným recidivujícím maligním pleurálním výpotkem. Věkové rozmezí 42–80 let, Ø věk 65 let. Cílem práce bylo objektivizovat průběh celkových a lokálních zánětových změn po talkáži. Efekt léčby nebyl ovlivněn základním typem nádorového onemocnění. U obou skupin nebyly významně odlišné pooperační komplikace, což dokládá bezpečnost zvolené metody a podporuje správně zvolený algoritmus léčby., Continuous pleural effusion production of any aetiology can significantly affect the quality of patients life. Chronic effusion accumulation in the pleural cavity can lead to lung parenchyma compression. The first symptom of this entity is a progressive exertional dyspnea, which can later turn in dyspnea at rest. With the increase of the pleural effusion volume, pulmonary atelectasis can develop due to chronic parenchycha compression. The patient is at risk of inflamatory complications from the territory of non-ventilated parenchyma. Patients with a diagnosis of malignant disease have this risk considerably higher due to their previous chemotherapy or radiation, which substantially affect the immunity system. Repeated pleural effusion formation can significantly take part in the development of the terminal status of cancer cachexia. An effective method of palliative treatment is a chemical pleurodesis, which is used in selected patients. The principle is inducing aseptic inflammation. The most effective way of application is via videothoracoscopy. In a group of 47 patients, there were 29 (61.7 %) men and 18 (38.3 %) women with cytologically diagnosed recurrent malignant pleural. The age ranged between 42 and 80 with average age of 65 years. The aim was to assess the course of local and systemic inflammatory changes after talc application. The effect of treatment was not influenced by the type of malignancy . In both groups there was no significant difference in postoperative complications. Background: Obstetric complications developing at antiphospholipid syndrome are serious medical-social problem. Seeking of the treatment methods sets conditions for urgency of the article reviewed. Enzyme preparations are widely applied for treatment of women with fecundity disorder and hemorheology changes. Research objective was in clinico-laboratorial evaluation of Serrata drug impact in antiphospholipid syndrome in women with reproductive losses of Andijan state. Methods: Data of 43 women in the non-pregnancy state with reproductive losses (RL) in past history with revealed APS had been studied by us. Cíl: Cílem příspěvku je předložit dosavadní poznatky o používání SQUALA standardizovaného dotazníku (dále jen „SQUALA“) pro oblast subjektivní kvality života. Předkládaný příspěvek obsahuje deskripci a analytickou práci s výsledky výzkumů (především v podobě plnotextů) subjektivní kvality života, v jejichž metodických přístupech byl cíleně užit SQUALA. Metody: Metodika systematické práce se odvíjela od vymezeného zkoumaného problému a výzkumných otázek. Hlavním přístupem byl koncept EBP a užitá metoda obsahové analýzy. Hledáno bylo v širokém spektru informačních zdrojů – v databázích předplacených Univerzitou Palackého v Olomouci, v dostupných archivech vybraných odborných časopisů, databázi Medvik Národní lékařské knihovny, nebo kupř. v archivech závěrečných kvalifi kačních prací. Výsledky: Z oblasti somatopedicky (která se zaměřuje na oblast chronických onemocnění) akceptovatelné klientely byla subjektivní kvalita života posuzována např. u jedinců s idiopatickou skoliózou, paraplegií, roztroušenou sklerózou, poraněními mozku, kardiovaskulárními onemocněními, posttraumatickou transverzální míšní lézí, diabetem, onkologickými diagnózami či chronickou obstrukční bronchopulmonální nemocí. U jednotlivých cílových skupin byla vymezena také konkrétní chronická onemocnění, která byla posuzována SQUALA. Jmenujeme, pro svou četnost, především diabetes mellitus, chronickou obstrukční bronchopulmonální nemoc, onkologická onemocnění, schizofrenie, psychózy, chronickou bronchitidu či HIV/AIDS pozitivitu. Závěr: SQUALA je široce uplatnitelný jednoduchý nástroj/dotazník k posouzení subjektivní kvality života. Užívá se u širokého spektra klientely/příjemců péče, včetně osob s chronickými onemocnění. Využít se však také dá u „zdravé“, tj. intaktní společnosti (bez zdravotního či jiného problému/omezení., Aim: Main aim is to present actual information in using SQUALA standardized questionnaire (next „SQUALA“ only) in area of assessing subjective quality of life. Article presents description and analytical work with results of research (mainly focusing on full texts) connected with subjective quality of life in which SQUALA was used. Methods: Methodics of systematic work was based on research problem and research questions. Content analysis and concept of EBP were used. Searching was made in wide area of information sources – databases of Palacký University Olomouc, archives of on-line research journals, database Medvik (National Medical Library) and in archives of qualifi cation thesis. Results: In area of somatopedics, which is focusing on chronic illnesses area, subjective quality of life was assessed in individuals with idiopathic scoliosis, paraplegia, multiple sclerosis, brain injuries, cardiovascular diseases, post-traumatic spinal cord lesions, diabetes, oncology diagnoses or chronic obstructive pulmonary disease. For each target group, specifi c chronic illnesses assessed by SQUALA were also defi ned. For its abundance, chronic illnesses were especially diabetes mellitus, chronic obstructive pulmonary disease, oncological disease, schizophrenia, psychosis, chronic bronchitis or HIV/AIDS positive in clients. Conclusion: SQUALA is widely applicable simple tool/questionnaire to assess subjective quality of life. It is used in a wide range of clients/care recipients, including people suff ering chronic illnesses.
