Anémie z nedostatku železa je v celosvětovém měřítku nejčastějším hematologickým onemocněním vůbec. V posledních 20 letech byly popsány základní mechanizmy regulace metabolizmu železa, ať již se jedná o systém regulačních proteinů IRP/IRE či o objev hepcidinu, klíčového peptidu metabolizmu železa a mechanizmů regulace jeho syntézy. Příčinou sideropenie je nejčastěji zvýšená ztráta železa z organizmu, v diagnostice se uplatňuje zejména kombinace hladiny feritinu v séru a saturace transferinu. U stavů s kombinovanou poruchou metabolizmu železa (např. anémie u chronických onemocnění) je většinou k odhalení sideropenie třeba vyšetření několika parametrů. V léčbě sideropenie se uplatňuje substituce přípravky obsahujícími železo. Parenterální podávání železa je indikováno u stavů s porušenou resorpcí železa ze střeva, jako velmi efektivní se u těchto stavů ukazuje být podání nových přípravků s rovnoměrným postupným uvolňováním i vysokého obsahu železa v molekule (např. Fe3+ v komplexu s karboxymaltózou). Velmi dobrá biologická využitelnost těchto přípravků by mohla být výhodná i při iniciální korekci těžké sideropenie u nemocných špatně tolerujících perorální přípravky železa. Basic mechanisms involved in iron metabolism regulation have been described over the past 20 years, e. g. the system of IRP/IRP regulatory proteins or the discovery of hepcidin, a key regulatory peptide involved in the regulation of iron metabolism and synthesis. Increased iron loss from the body is the most common reason for iron deficiency, the combination of serum ferritin level and transferrin saturation represent the most effective laboratory tools for the diagnosis of iron deficiency. A combination of several parameters is usually necessary for an exact diagnosis of iron deficiency in conditions with a combined iron metabolism disorder (e. g. anemia of chronic diseases). Iron deficiency may be corrected with the administration of medications containing iron salts. Parenteral administration of iron is indicated in patients with altered iron absorption from the gut; new drugs with regular and gradual release of iron molecules, even in large amounts (e. g. ferric carboxymaltose), may effectively correct iron depletion in these patients. High bioavailability of these preparations could also be effective in the initial correction of severe sideropenic anemia in patients with poor toleration of oral iron medications. Myelodysplastický syndrom (MDS) je jedno z nejčastějších hematologických onemocnění ve věku nad 60 let. Přes intenzivní výzkum je však terapie této heterogenní krevní choroby velmi komplikovaná. V posledních letech se objevily 2 nové terapeutické přístupy, a to imunomodulační léčba a demetylační terapie. Zvláště imunomodulační léčba, reprezentovaná preparátem lenalidomid, znamená velký pokrok v léčně anemických nemocných specificky s 5q- aberací. Odpověď, především dosažení transfuzní nezávislosti, se pohybuje v původních studiích kolem 60–80 %. U nás byla tato terapie zahájena v roce 2008. Prezentujeme analýzu 34 nemocných, 31 žen, 3 mužů s mediánem věku 69 (46–80 let), z toho 27 nemocných (79,4 %) mělo diagnózu 5q- syndromu, 5 nemocných diagnózu refrakterní anémie s multilineární dysplazií, 1 nemocný refrakterní anémii s excesem blastů (RAEB 1) a 1 pacient neklasifikovatelný myelodysplastický syndrom/myeloproliferativní syndrom. Data nemocných jsou ukládána do lenalidomidové lišty Českého MDS registru. Jde o nemocné chronicky transfundované s diagnózou MDS s nižším rizikem a 5q- aberací. V našem souboru jsme zaznamenali odpověď na terapii reprezentovanou dosažením kompletní transfuzní nezávislosti u 91 % nemocných. Kompletní cytogenetická odpověď byla dosažena u 15 % nemocných a parciální odpověď u 67 %. U 15 % nemocných byla zjištěna mutace tumor supresorického genu TP53 (3 z 18 vyšetřených nemocných, 2 z nich progredovali do MDS s vyšším rizikem a následně do AML). Lenalidomid byl u většiny nemocných velmi dobře tolerován. Na tomto, byť zatím malém souboru prezentujeme vysokou efektivitu tohoto preparátu a základní principy a problematiku terapie lenalidomidem., Myelodysplastic syndrome (MDS) is a common hematological disease in patients over sixty. Despite intensive research, the therapy of this heterogeneous blood disease is complicated. In recent years, two new therapeutic approaches have been proposed: immunomodulation and demethylation therapy. Immunomodulation therapy with lenalidomide represents a meaningful advance in the treatment of anemic patients, specifically those with 5q- aberrations. As much as 60–70 % of patients respond and achieve transfusion independence. We present the initial lenalidomide experience of the Czech MDS group. We analyze Czech MDS register data of 34 (31 female; 3 male; median age 69 years) chronically transfused low risk MDS patients with 5q- aberration treated by lenalidomide. Twenty-seven (79.4 %) patients were diagnosed with 5q- syndrome, 5 patients with refractory anemia with multilineage dysplasia, 1 patient with refractory anemia with excess of blasts 1, and 1 patient with myelodysplastic/myeloproliferative unclassified. Response, as represented by achieving complete transfusion independence, was achieved in 91 % of patients. A true 5q- syndrome diagnosis in most our patients may be responsible for such a high response rate. Complete cytogenetic response was reached in 15 % of patients and partial cytogenetic response in 67 %, within a median time of 12 months. TP53 mutation was detected in 15 % (3 from 18 tested) and 2 of these patients progressed to higher grade MDS. The majority of patients tolerated lenalidomide very well. The present nuclear and cell body diameter measurements demonstrated size differences of the approximate cell space estimate occupied by the cell nucleus during the cell differentiation in lymphocytic, granulocytic and erythroid cell lineages. These lineages were used as convenient models because all differentiation steps were easily identified and accessible in diagnostic peripheral blood or bone marrow smears of blood donors (BDs), patients suffering from chronic lymphocytic leukemia (CLL), patients with chronic myeloid leukemia (CML) and refractory anemia (RA) of the myelodysplastic syndrome (MDS). The cell space occupied by the nucleus was constant and did not change during the cell differentiation in the lymphocytic cell lineages of BDs and CLL patients despite the decreased cell size. In contrary, the cell space occupied by the nucleus markedly decreased in differentiating cells of granulocytic and erythroid lineages of patients suffering from CML. In the erythroid cell lineage in patients with RA of MDS the small reduction of the cell space occupied by the nucleus during the differentiation was not significant. The measurements also indicated that in progenitor cells of all studied cell lineages nuclei occupied more than 70 % of the cell space.
