Cíl studie: Vytvoření stratifikačního modelu s cílem identifikace vysoce rizikových osob s MGUS, u kterých bude možno zvažovat včasnou léčebnou intervenci. Typ studie: Retrospektivní studie Název a sídlo pracoviště: Interní hematoonkologická klinika, FN Brno - PMDV, Brno Materiál a metody: Na souboru 314 osob s MGUS zařazených do Registru monoklonálních gamapatií (RMG) byla provedena pilotní statistická analýza. Byla využita základní klinická data se zaměřením na prognostické parametry stávající (typ monoklonálního proteinu - MIG, hladina MIG, poměr sérových volných lehkých řetězců – FLC ratio) i nové (přítomnost cytogenetických aberací, flowcytometrické stanovení množství klonálních plazmocytů a přítomnost imunoparézy). Výsledky: Vyšší zastoupení morfologicky identifikovaných plazmocytů a nález >95 % PC aberantního fenotypu CD19- CD56+/- se statisticky významně častěji pojí s transformací MGUS do MM (p<0,05). Také přítomnost jakékoliv imunoparézy či imunoparézy postihující oba nepostižené imunoglobuliny byla statisticky významně častěji detekována u transformací MGUS do MM (p=0,001). První analýza stávajících a nových prognostických faktorů u MGUS dosáhla senzitivity stratifikace cca 50 % během 2 let. Závěr: Byl potvrzen význam hodnocení PC pomocí morfologie a průtokové cytometrie a důležitou úlohu hraje také přítomnost imunoparézy. Stratifikační modely budou dále vylepšovány s cílem dosažení senzitivity 90 % v rámci 2 let a obdobná analýza bude provedena také u asymptomatických myelomů (aMM)., Objective: The creation of the stratification model and identification of high-risk individuals with MGUS, which should be considered to an early therapeutic intervention. Design: The retrospective study Settings: Department of Internal medicine - Hematooncology, University Hospital Brno Material and Methods: The pilot statistical analysis of 314 MGUS subjects from Registry of Monoclonal Gammopathies (RMG) was done. Basic clinical data were used together with existing (type of monoclonal protein - MIG, level of MIG, serum free light chain ratio – FLC ratio) and new prognostic parameters (presence of cytogenetic abnormalities, flow cytometry enumeration of clonal plasma cells and presence of immunoparesis). Results: Higher number of plasma cells according to the morphology and presence of >95 % PC with aberrant phenotype CD19-CD56+/- as well were more frequently associated with transformation of MGUS into MM (p<0.05). Also, the presence of any immunoparesis and/or immunoparesis affecting both uninvolved immunoglobulins was more frequently detected in transformed MGUS (p=0.001). The first analysis of existing and new prognostic factors in MGUS achieved sensitivity of the stratification about 50 % within 2 years. Conclusion: The significance of morphological and flow cytometry assessment of PCs was confirmed and presence of the immunoparesis plays an important role as well. Plicní aspergilóza patří mezi zánětlivá onemocnění způsobená houbami rodu Aspergillus. V závislosti na stavu imunitního systému a plicního parenchymu nemocného existují invazivní, semi-invazivní a neinvazivní formy onemocnění. V diagnostice tohoto onemocnění hrají hlavní úlohu prostý rentgenový snímek hrudníku a HRCT plic. Prezentujeme kazuistiku pacienta s nově diagnostikovaným high-grade gliomem mozku, u kterého se v důsledku antiedematózní terapie kortikoidy rozvinula invazivní plicní aspergilóza včetně mimoplicní disseminace., Pulmonary aspergillosis belongs to inflammatory diseases caused by Aspergillus spp. Invasive, semi-invasive and non-invasive forms of the disease exist depending on the level of immunity and possible impairment of pulmonary parenchyma. Autoři popisují případ 78leté ženy s masivní hemoptýzou, zdrojem které byla hypertrofická bronchiální tepna anomálně odstupující z a. thoracica interna. CT angiografie provedená na multidetektorovém CT přístroji ektopickou bronchiální tepnu zobrazila. Následná selektivní embolizace dané tepny akrylátovým lepidlem vedla k zástavě hemoptýzy. Nemocná neměla recidivu krvácení během 24měsíčního sledování. Anomální odstup bronchiální tepny není vzácný a proto je nutné na něj myslet zejména v případech, kdy není patrná žádná tepna zásobující bronchy v normální anatomické lokalizaci. CT angiografie provedená na multidetektorovém CT přístroji má vysokou úspěšnost v detekci odstupu bronchiálních tepen a slouží k navádění při jejich katetrizaci a embolizaci., The authors describe a case of a 78-year-old woman with massive hemoptysis, which was from to hypertrophic bronchial artery anomalously originating from the internal thoracic artery. CT angiography performed on multidetector CT shows anomalous origin of the right bronchial artery. The bronchial bleeding was treated with embolization. The patient had no recurrent bleeding during 24 month follow-up. Ectopic origin of bronchial arteries is not rare, therefore it is necessary to consider it especially in cases when no bronchial artery arises between the upper margin of the fifth thoracic vertebra and the lower margin of the sixth thoracic vertebra. Na sklonku loňského roku se možnosti léčby ovariálního karcinomu rozrostly o antiangiogenní léčbu. Evropská komise schválila rozšíření indikace bevacizumabu (Avastin®): „Avastin v kombinaci s karboplatinou a paklitaxelem je indikován k úvodní léčbě pokročilého (stadia III B, III C a IV dle FIGO) epitelového nádoru vaječníků, vejcovodů nebo primárního nádoru pobřišnice“. V průběhu posledních let dochází k nárůstu počtu imunosuprimovaných nemocných a tím i k vzestupu invazivních mykotických onemocnění. Terapie hlubokých mykóz je mnohdy obtížná, mortalita a morbidita těchto infekčních komplikací jsou vysoké. Stále lepší dostupnost nových antimykotik rozšiřuje naše léčebné možnosti a zlepšuje výsledky terapie těchto invazivních mykotických infekcí. Předkládaná práce podává informace o využití jednotlivých antimykotik v léčbě invazivních mykóz, shrnuje léčbu invazivní aspergilózy (IA), invazivní kandidózy (IC) a empirickou antimykotickou terapii. Doporučení vychází ze závěrů prezentovaných Europen Conference on Infection in Leukemia (ECIL)., The frequency of invasive fungal infections has significantly increased with the rise in at-risk populations of patients in the last years. Management of deep fungal infection is difficult and the morbidity and mortality of these infections are very high. New antifungal agents provide the managment options and improve therapeutic outcomes of these infections. This article informs about the role of available antifungal agents in the management of invasive fungal infections and reviews empirical antifungal treatmnet and the treatment of invasive aspergillus and candida infections. The aim of our study was to determine the effect of selected cytostatics on a human ovarian cancer cell line A2780 as a model system for ovarian cancer treatment. This cell line is considered cisplatin-sensitive. Panel of tested cytostatics included cisplatin, paclitaxel, carboplatin, gemcitabine, topotecan and etoposide. These cytostatics have a different mechanism of action. To evaluate cytotoxic potential of the tested compounds, the methods measuring various toxicological endpoints were employed including morphological studies, MTT assay, dynamic monitoring of cell proliferation with xCELLigence, cell cycle analysis, caspase 3 activity and expression of proteins involved in cell cycle regulation and cell death. Based on survey of 257 women with nondeveloping pregnancies of various geneses, it was shown that the level of apoptosis marker P53 in the decidual tissue is increased, and antiapoptotic protein Bcl2 simultaneously is reduced, compared with similar data of women with physiological pregnancies. Brachial Plexus is formed by the union of the anterior rami of cervical 5, 6, 7, 8 and thoracic 1 nerves. These nerves unite and divide to form the key nerves innervating the upper limb. Variations in the course of these nerves are clinically important to anesthetists, neurologists and orthopedicians. We report bilateral variations in the arterial and neural structures in the upper limb of a 65 year old cadaver. The muscles of the arm on one side were innervated by the median nerve with absence ofmusculocutaneous. While on the other side the musculocutaneous nerve contributed to the formation of the median nerve. There was a presence of high bifurcation of brachial artery on both sides. Knowledge of such variations in the innervations of muscles and the arterial supply of the limbs are important to remember before performing any reconstructive procedures or interventions on the limb. and V. Cíl: Výměnný obrázkový komunikační systém (VOKS) je na podmínky České republiky (ČR) modifi kovaná metoda vycházející z komunikačního systému PECS (The picture exchange communication system), který byl vytvořen v Chicagu v USA. Výzkum měl za cíl zmapovat spokojenost uživatelů s touto metodou, zhodnotit obtížnost osvojení metody VOKS a zmapovat dodržování doporučených postupů. Uživatelem autoři rozumí, pracovníka, který využívá metodu VOKS při komunikaci s klienty s PAS. Ve výzkumné části jsou prezentovány výsledky šetření, které se uskutečnilo v pěti vybraných speciálních zařízení a čtyřech školách na území celé ČR. Metody: Výzkumné šetření proběhlo v období od září 2010 do prosince 2010. Byl vytvořen dotazník vlastní konstrukce, který obsahoval 24 otázek. Kritériem pro zařazení respondentů do výzkumu, byla práce s metodou VOKS v zaměstnání, které respondenti aktuálně vykonávali. Výzkumný soubor tvořil 54 respondentů. Výsledky: Analýzou výsledků bylo zjištěno, že většina respondentů je spokojena s metodou VOKS. Rozdíly jsou patrné v době osvojení metody VOKS v praxi. Na tomto konkrétním vzorku respondentů se nepodařilo určit obecně platný čas potřebný k osvojení metody VOKS. Jako problematické se jeví i přesné dodržování postupů. Hlavními důvody nedostatečného dodržování postupů v praxi je potřeba individuálního přizpůsobení klientovi, nedostatek personálu a nedostatek času na nácvik. Velkým problémem je aplikace metody v době dovolených proškoleného personálu. Závěr: Výzkum naznačil, že metoda VOKS se prozatím, z pohledu individuálního hodnocení respondentů, jeví jako vhodná pro komunikaci s klienty s různorodými poruchami řeči. Metoda VOKS může přispět pracovníkům ve speciálních zařízeních ke zkvalitňování ošetřovatelské péče a k lepšímu uspokojování potřeb klienta., Aims: Czech picture exchange communication system called VOKS is a modifi ed method of communication system PECS (Picture Exchange Communication System) which was created in Chicago in the USA. The research aim was to assess user satisfaction with this method, to evaluate diffi culties with acquiring the VOKS methods and to map compliance with recommended practice. A “user” is understood as somebody who uses VOKS method to communicate with clients suff ering from ASD. The research section presents results of the survey which took place in selected fi ve specialised facilities and four schools all over the Czech Republic. Methods: The survey covered the period from September 2010 to December 2010. We designed our own questionnaire which contained 24 questions. The criterion for inclusion of respondents in the research was current professional working experience with the VOKS method. The research sample consisted of 54 respondents. Results: The results of the analysis revealed that most respondents are satisfi ed with the VOKS methods. Differences are evident in the time when they adopted VOKS methods in practice. With this particular sample of respondents the time necessary to master the VOKS method could not be established. Accurate compliance with recommended procedures proved to be problematic. The main reason for poor compliance with the procedures in practice is the necessity to adapt it for an individual client, staff shortages and insuffi cient time for training. A major problem is the application of the method when the trained staff is on holiday. Conclusion: The research indicates that for the time being the respondents consider the VOKS method to be a suitable method for communicating with clients with a variety of speech disorders. Unilateral otosclerosis combined with avascular necrosis of stapes crura is a rare entity. It should be considered in a case of high grade otosclerosis. Symptoms are the same as in patients who suffer from common otosclerosis. Patients complain on progressive hearing loss and tinnitus. The diagnosis is made clinically by conventional audiologic evaluation and radiologically by x-ray mastoid Schuller's view and CT scan. HRCT scan makes visible all parts of ossicular chain and gives surgeon some information about ossicular chain damage. Surgery with stapedotomy and stapes prosthesis implantation in a case of otosclerosis with avascular necrosis of stapes crura can be success therapy to improve patient's hearing and M. Erdoglija, J.
