CD200 and its receptor were recognized as having the multiple immunoregulatory functions. Their immunoregulatory, suppressive, and tolerogenic potentials could be very effectively exploited in the treatment of many diseases, e.g. Alzheimer disease, rheumatoid arthritis, and allergy to name only some. Many research projects are aimed to develop clinically valuable methods being based on the structure and function of these paired molecules. CD200/CD200R are highly conserved type I paired membrane glycoproteins that belong to the Ig superfamily containing a two immunoglobulin‑like domain (V, C). CD200 is broadly distributed in a variety of cell types, whereas CD200R is primarily expressed in myeloid and lymphoid cells. They fulfill multiple functions in regulating inflammation. The interaction between CD200/CD200R results in activation of the intracellular inhibitory pathway with RasGAP recruitment and thus contributes to effector cell inhibition. It was confirmed that the CD200R activation stimulates the differentiation of T cells to the Treg subset, upregulates indoleamine 2,3‑dioxygenase activity, modulates cytokine environment from a Th1 to a Th2 pattern, and facilitates an antiinflammatory IL‑10 and TGF‑β synthesis. CD200/CD200R are required for maintaining self‑tolerance. Nádory měkkých tkání jsou rozsáhlou a značně heterogenní skupinou onemocnění vyžadující multimodalitní diagnostický přístup. Magnetická rezonance se stala díky vysoké senzitivitě a kontrastnímu rozlišení nejefektivnější metodou při jejich detekci a lokálním stagingu. Její možnosti při stanovení histologického typu nádoru a často i při rozhodnutí, zda je leze benigní či maligní jsou značně omezené. Z tohoto důvodu musí být až na výjimky kombinována s histologickým vyšetřením, které je rovněž nezbytné pro kompletní staging. Článek se zabývá technikou vyšetřování nádorů měkkých tkání pomocí magnetické rezonance a využitím metody při stagingu a diferenciální diagnóze. Krátce jsou zmíněny i možnosti dalších zobrazovacích modalit., Soft tissue tumors are large and heterogenous group of diseases requiring multimodality diagnostic approach. Magnetic resonance is, thanks to its high sensitivity and contrast resolution, the most effective method for their detection and local staging. Its possibilities in determining the histological type of tumor and in the decision whether the lesion is benign or malignant are quite limited. For this reason it MRI must be, with few exceptions, combined with histological examination, which is also necessary for complete staging. This article deals with MRI examination technique and the use of this method for staging and differential diagnosis. Článek uvádí přehled současných poznatků o žaludečních neuroendokrinních nádorech. Jde o heterogenní skupinu zahrnující 3 typy, které se odlišují svým biologickým chováním a léčebným přístupem. Typ 1 vzniká na podkladě chronické korporální atrofické gastritidy, je relativně málo maligní, léčba spočívá v endoskopickém odstranění drobných nádorů, u větších lézí či infiltrativních forem je nutná resekce žaludku, výjimečně gastrektomie. Typ 2 vzniká v rámci Zollinger-Ellisonova syndromu. Oba uvedené typy jsou gastrin-dependentní. Naproti tomu typ 3 je solitární nádor, vznikající bez závislosti na gastrinu, jeho maligní potenciál je značný. Nádor nutno léčit chirurgicky, u pokročilých forem je nezbytná paliativní terapie., The paper presents recent opinions on gastric neuroendocrine neoplasms. This is a heterogeneus group including three types that distinguish in their biological behaviour, and require different therapeutical approach. Type 1 arises in setting of chronic corporal atrophic gastritis, malignant potential of this type is lower, this type 1 tumour may be treated with endoscopic removal of multiple small tumour lesions. Gastric resection is indicated in larger lesions with infiltrative pattern of growth. Gastrectomy is rarely indicated. Type 2 arises as consequence of Zollinger-Ellison syndrome. Both above-mentioned type are gastrin-dependent. Type 3 is a solitary, gastrin-independent tumour with higher malignant potential. The colorectal cancer ranks high among the malignant tumours in incidence and mortality and irinotecan is standardly used in palliative treatment of metastatic disease in every therapeutic line. Unfortunately, the treatment with irinotecan is often associated with severe toxicities, especially neutropenia and diarrhea. The majority of the toxic manifestation is caused by the insufficient deactivation (glucuronidation) of irinotecan active metabolite SN-38 by UGT1A enzyme. The elevated SN-38 plasma concentration is responsible for the hematological and gastrointestinal toxicity that can become life-threatening. The patients carrying the mutation of the gene encoding UGT1A enzyme lack the ability of bilirubin glucuronidation, and suffer from the inherited un-conjugated hyperbilirubinemia (Gilbert syndrome, Crigler-Najjar type 1 and 2 syndrome). The mutations in other enzyme systems also play role in the etiopathogenesis of the irinotecan toxicity: CYP3A (cytochrome P-450), ABC family of transmembrane transporters (adenosine-triphosphate binding cassette). Článek pojednává o historii politiky podpory zdraví v ČR v období let 1990 až 2012. Cílem článku je 1) zachytit odlišné přístupy k politice podpory zdraví (a potažmo ke zdraví a determinantám zdraví) formulované vládami ve významných koncepčních materiálech a 2) na základě studia sekundárních dokumentů identifikovat významné události samotné implementace sledované politiky. Vládou vnímaná (resp. v koncepčních dokumentech deklarovaná) hodnota zdraví a konkrétní pojetí determinant zdraví, které, jak článek ukazuje, do značné míry souvisí s politickou orientací vlád, jsou nikoliv jedinými, ale významnými faktory následně realizované politiky podpory zdraví., The paper deals with the history of health promotion policy in the Czech Republic during 1990–2012. The aims of the paper are as follows: 1) to reveal different approaches to health promotion policy (and to health and health determinants), defined by Czech governments in significant conceptual documents and 2) based on the study of secondary documents to identify significant events of the implementation of the policy under consideration.
