V těhotenství je anafylaktická reakce naštěstí sice relativně vzácná, její důsledky ale mohou vést až k závažnému postižení matky i plodu. Ve většině publikovaných případů těžké anafylaxe v těhotenství bylo těžké postižení dokonce spíše na straně plodu než matky. Vznik anafylaxe se většinou nedá předvídat, první symptomy jsou někdy netypické, pouze lokální, ale rychle mohou přecházet v generalizované postižení celého organismu. Ať už jsou zprostředkovány protilátkami typu IgE (pravá anafylaxe), nebo vznikají na jiném podkladě (anafylaktoidní reakce), klinický obraz bývá obdobný. Je třeba myslet na to, že i klinicky nenápadné počáteční symptomy mohou mít průběh závažný a život ohrožující. Každé opomenutí a prodleva v diagnostice a léčbě anafylaxe může vést k fatálnímu konci. O úspěšném výsledku léčby rozhoduje velkou měrou medicínská zkušenost a včasnost zásahu. Protože je toto téma v současné literatuře zmiňováno spíše jen výjimečně, je cílem tohoto článku shrnout problematiku anafylaxe v těhotenství z hlediska patofyziologie, klinického průběhu, diagnostiky i léčby., Anaphylaxis in pregnancy is a relatively rare reaction but it can lead to serious maternal and fetal impairment. In published cases of severe anaphylaxis in pregnancy there were even more severe disabilities in the fetus than the mother. Development of anaphylaxis is usually unpredictable, the first symptoms are sometimes atypical and only local, but can move quickly in a generalized impairment of the whole organism. Whatever they are mediated by IgE antibodies (right anaphylaxis), or are caused on another basis (anaphylactoid reactions), the clinical picture is similar. It should be kept in mind that even clinically inconspicuous initial symptoms may progress into serious and life-threatening state. Any omission and delay in diagnosis and treatment of anaphylaxis can lead to fatal end. A successful outcome is largely dependent on experience and timeliness of medical intervention. CD200/CD200R are highly conserved type I paired membrane glycoproteins that belong to the Ig superfamily containing a two immunoglobulin‑like domain (V, C). CD200 is broadly distributed in a variety of cell types, whereas CD200R is primarily expressed in myeloid and lymphoid cells. They fulfill multiple functions in regulating inflammation. The interaction between CD200/CD200R results in activation of the intracellular inhibitory pathway with RasGAP recruitment and thus contributes to effector cell inhibition. It was confirmed that the CD200R activation stimulates the differentiation of T cells to the Treg subset, upregulates indoleamine 2,3‑dioxygenase activity, modulates cytokine environment from a Th1 to a Th2 pattern, and facilitates an antiinflammatory IL‑10 and TGF‑β synthesis. Úkolem diagnostické terminologie v jakékoli morfologické metodě je poskytnutí jednoznačně interpretovatelné informace klinickému specialistovi tak, aby mohl zvolit adekvátní terapeutický postup. Cytologická klasifikace dysplastických změn na děložním hrdle se od původní Papanicolauovy pětistupňové postupně zjednodušila na dvoustupňovou. V histopatologické rovině hodnocení se rychle prosadila fúze kategorií CIN III a CIS, další vývoj je však v porovnání s cytopatologickým hodnocením pomalejší. Navrhovaná dvoustupňová klasifikace histopatologických lézí děložního hrdla nese v sobě potenciál zpřesněné komunikace klinika a patologa ve vyhodnocování cervikálních lézí a jejich následném ošetřování. V porovnání k předchozím je plně převoditelná. Zachovává si ovšem i předchozí omezení ve vyhodnocení základního histopatologického nálezu, zejména závislost na primárně reprezentativním vzorku. The former Papanicolaou five-tiered cytological classification of dysplastic changes on the uterine cervix has been simplified to a two-tiered approach. In the histopathological evaluation, fusion of the categories CIN III and CIS was rather smooth. Further progress toward simplification of the histopathology terminology is proceeding more slowly. The proposed two-tiered classification of the histopathological lesions of the cervix has the potential to create a more precise communication between the clinician and the pathologist in the evaluation and subsequently in the treatment of the cervical lesions. In comparison to the previous classifications, it is fully convertible. It preserves the previous limits in the evaluation of the histopathological finding, mainly the dependence on the primary representativeness of the specimen. Deeper understanding of the cervical cancerogenesis will be achieved with the increasing spectrum of prognostication markers. Od objavenia mimobunkovej fetálnej DNA v krvi matky uplynulo už 16 rokov. Avšak až v priebehu posledných 5 rokoch prebieha čoraz viac výskumov za účelom stanovenia jej presnej charakteristiky, koncentrácie a molekulárnej veľkosti, ktoré sa neskôr premietli do jednotlivých testov. Tie sú ale dostupné len v posledných 2 rokoch. Pri aktuálnom trende preferovania neinvazívnych metód prenatálnej diagnostiky a ich posúvania do obdobia prvého trimestra sa stanovenie mimobunkovej fetálnej DNA v krvi matky ukazuje ako veľmi perspektívnou metódou diagnostiky chromozómových aberácií u plodov. Jej širšie uplatnenie je limitované malým počtom štúdií na nerizikových skupinách pacientok a v nemalej miere aj ekonomickou náročnosťou vyšetrenia. Aj napriek tomu sa však táto metóda čoraz viac rozširuje zo Spojených Štátov Amerických do celého sveta, čo znižuje náklady na ňu, a stáva sa dostupnejšou pre čoraz väčší počet pacientok. Ukazuje sa, že táto metóda by v rámci podrobnejšieho vyšetrovania u rizikových pacientok mohla perspektívne minimalizovať riziko fetálnych strát, ktoré sú závažnou komplikáciou invazívnych metód prenatálnej diagnostiky., It has been 16 years since there has been a discovery of cell-free fetal DNA (cffDNA) in mother‘s blood. However in the last 5 years there has been more researching done in the purpose of its specific diagnosis, concentration and molecular size, which have later on reflected into particular tests. These have been available only in the past 2 years. Within the actual trend in preference of non-invasive methods of prenatal diagnosis and its shifting into first trimester the examination of mother’s blood has been showing as a very perspective method for early cell-free fetal DNA discovering of chromosome aberration diagnosis of fetus. Its wider enforcement is unfortunatelly limited by a small amount of studies within non-risk pregnancy groups of patients as well as its higher financial cost. Background: Despite the fact that the genetic basis of schizophrenia has been intensively studied for more than two decades, our contemporary knowledge in this field is rather fractional, and a substantial part of it is still missing. The aim of this review article is to sum up the data coming from genome‑wide association genetic studies in schizophrenia, and indicate prospective directions of further scientific endeavour. Methods: We searched the National Human Genome Research Institute’s Catalog of genome‑wide association studies for schizophrenia to identify all papers related to this topic. In consequence, we looked up the possible relevancy of these findings for etiology and pathogenesis of schizophrenia using the computer gene and PubMed databases. Results: Eighteen genome‑wide association studies in schizophrenia have been published till now, referring to fifty‑seven genes supposedly involved into schizophrenia’s etiopathogenesis. Most of these genes are related to neurodevelopment, neuroendocrinology, and immunology. Infantilní hemangiomy jsou nejčastějšími benigními vaskulárními nádory novorozeneckého a kojeneckého věku. Jedná se o nádory s velmi dobrou prognózou. Většina infantilních hemangiomů spontánně zcela regreduje v prvních deseti letech života. Jen deset procent infantilních hemangiomů vyžaduje nějakou terapii. V současné době je preferováno použití b-blokátoru propranololu. Ostatní typy benigních cévních nádorů je zapotřebí diferenciálně diagnosticky odlišit pro zahájení vhodné terapie a určení prognózy. Předložený článek shrnuje základní poznatky o hemangiomech u novorozenců. V závěru článku je zmíněna krátká kazuistika, kdy byla léčba rozsáhlého hemangiomu zahajována po konzultacích na našem pracovišti za kontroly kardiology., Infantile hemangiomas are the most common benign vascular tumors of neonatal and infancy. These are tumors with a good prognosis. Most infantile hemangiomas spontaneously regress completely in the first decade of life. Only ten percent of infantile hemangiomas require some therapy. Currently it is preferred to use b-blocker propranolol. Other types of benign vascular tumors are needed to distinguish the differential diagnosis for initiation of appropriate therapy and prognosis. The present article summarizes the main findings of hemangiomas in infants. Pro klasifikaci prekanceróz vulvy se používá termín vulvární intraepiteliální neoplázie (VIN). Histologicky lze rozlišit dva základní typy VIN: VIN obvyklého typu (u-VIN) a VIN diferencovaného typu (d-VIN). Rozsah a intenzita dysplastických změn se v případě u-VIN vyjadřuje třístupňovým gradingem (u-VIN I, II a III). Prekancerózní potenciál d-VIN odpovídá vždy karcinomu in situ, proto se u této diagnostické jednotky grading neprovádí. Etiologicky je u-VIN spojena s infekcí vysoce rizikovými typy lidského papillomaviru (HPV), zatímco d-VIN je HPV negativní a rozvíjí se v terénu chronických vulvárních dermatóz typu lichen sclerosus a lichen simplex chronicus. u-VIN má tendenci k multifokalitě a k dlouhodobému recidivujícímu průběhu před transformací v invazivní dlaždicobuněčný karcinom. d-VIN se naopak vyskytuje spíše solitárně a má sklon k rychlé stromální invazi. u-VIN typicky postihuje ženy premenopauzálního věku s vyšší incidencí ostatních prekancerózních lézí dolního ženského pohlavního systému včetně perinea a anu. Pacientky s d-VIN bývají postmenopauzální ženy a není u nich pozorována asociace s výše zmíněnými dyspláziemi ostatních anatomických lokalizací. Nízká diagnostická reproducibilita kategorie u-VIN I, pochyby o prekancerózním potenciálu u-VIN I a problematické rozlišení u-VIN II a u-VIN III vedly k revizi současného diagnostického schématu. Podstatou změn je zrušení gradingu u-VIN se sloučením kategorií u-VIN II a u-VIN III a dále zrušení u-VIN I jako diagnostické jednotky. V revidované klasifikaci prekanceróz vulvy proto termín u-VIN reprezentuje high grade dysplastické léze asociované s HPV (dříve u-VIN II a u-VIN III) a d-VIN i nadále zůstává označením pro HPV negativní high grade prekancerózu., Classification of vulvar precancerous lesions is based on the concept of vulvar intraepithelial neoplasia (VIN). Two basic types of VIN are recognized histologically: VIN of the usual type (u-VIN) and VIN of the differentiated type (d-VIN). The three grade evaluation system is used to assess the intesity of dysplastic changes in u-VIN (u-VIN I, II and III). The precancerous potential of d-VIN corresponds to carcinoma in situ and therefore grading is not applied. u-VIN is etiologically linked to HPV infection, whereas d-VIN is HPV negative and emerges in the environment of chronic vulvar dermatoses (lichen sclerosus and lichen simplex chronicus). u-VIN has a tendency to multifocality and prolonged recurrent progression to the invasive squamous cell carcinoma. On the other hand, d-VIN represents rather a solitary lesion with a propensity to the rapid stromal invasion. u-VIN usually affects premenopausal women with a higher incidence of other precancerous lesions of the lower female genital tract including the perineum and anal area. Patients diagnosed with d-VIN are of postmenopausal age without any association with aforementioned dysplastic lesions of other anatomic locations. The low diagnostic reproducibility of the u-VIN I category, doubts about the precancerous potential of u-VIN I and the problematic distinction between u-VIN II and u-VIN III resulted in the modification of the current terminology. The grading of u-VIN was abandoned, u-VIN II and u-VIN III categories were merged and u-VIN I was removed from the classification scheme. Onemocnění lidským papilomavirem (HPV) patří mezi jednu z nejběžnějších sexuálně přenosných infekcí a HPV působí spektrum nemocí, od genitálních bradavic ke karcinomům. Důležitým cílem očkovacích programů v mnoha zemích je snížení incidence karcinomu děložního hrdla použitím kvadrivalentní (Silgard/Gardasil) nebo bivalentní (Cervarix) HPV (human papillomavirus) vakcíny. Randomizované kontrolované studie prokazují robustní protilátkovou odpověď a vysokou účinnost HPV vakcinace také u mužů. V roce 2009 Úřad pro kontrolu potravin a léčiv (FDA) v USA schválil kvadrivalentní HPV vakcínu u mužů k prevenci anální prekancerózy a karcinomu. Podrobné finanční modely, které předcházely tomuto rozhodnutí, ukázaly, že pokud se do modelů zahrne výskyt onemocnění u mužů, záleží na ceně vakcíny, pokrytí očkované populace a dalších faktorech, zda HPV vakcinace mužů může být finančně efektivní. The important goal of immunization programs in many countries is the reduction of the incidence of cervical cancer using either the quadrivalent (Silgard/Gardasil) or the bivalent (Cervarix) HPV (human papillomavirus) vaccine. Randomized controlled trials demonstrate robust antibody responses and high efficacy also in men. In 2009 the Food and Drug Administration in the USA approved the quadrivalent HPV vaccine for young men to prevent anal precancerosis and cancer. Detailed cost effective modeling has preceded this decision showing that when the burden of disease in men is included in the models then, depending upon vaccine price, coverage of a vaccinated population, and other factors male vaccination can become cost effective. Hysteroskopia postupne nahrádza diagnostickú kyretáž, ktorá v porovnaní s hysteroskopiou neposkytuje adekvátnu diagnostiku organického nálezu v dutine maternice, prípadne riešenie intrauterinnej patológie diagnostickou kyretážou je skôr náhodné., Hysteroscopy is an effective diagnostic and therapeutic methods of intrauterine pathology that has all of the available diagnostic methodologies highest sensitivity and specificity. Při zjištění tumoru v pánvi se nalézáme před problémem stanovení jak jeho origa, tak i biologického chování. Možnost predikovat biologické chování pánevních tumorů již před léčbou je klíčem ke zvolení individuálního přístupu, který zatíží pacientku úměrně k maximálnímu možnému léčebnému efektu. V článku se dále zabýváme pouze gynekologickými nádory. Hlavní prognostické a prediktivní pooperační faktory dobře známé a rutinně používané jsou histologický typ nádoru, stádium onemocnění, stupeň diferenciace, přítomnost lymfangioinvaze, postižení lymfatických uzlin a pooperační nádorové reziduum. Při předoperační predikci biologického chování gynekologických pánevních tumorů vycházíme z anamnézy, klinických ukazatelů - věku a přidružených chorob nemocné (performance status), dále ze stanovení markerů z krve, grafických vyšetření (zejména sonografie) a charakteristik nádoru stanovených na základě histologického, imunohistochemického či molekulárně-genetického vyšetření tkáně získané z excize, laparoskopie či tru-cut biopsie. Mezi prediktivně-prognostické biomarkery, které mohou pomoci předpovídat úspěšnost naší léčby a je možné je využít v triage pacientek před léčbou, je možné zařadit například markery z periferní krve CA125, HE4, ROMA index, přítomnost BRCA 1/2 mutace, předoperační trombocytózu, neutrophil-to-lymphocyte ratio, a dále tkáňové markery, jako je mutace p53, Her2/Neu pozitivitu, přítomnost ER/PR, pozitivitu L1CAM, dále panel 9 proteinových markerů PROVAR, intraepitelální tumor-infiltrující lymfocyty, stanovení proliferační aktivity pomocí proteinu Ki67 (MIB1), genové expresní profily, proteiny rezistence atd. Tyto biomarkery budou v následujícím textu probrány. Pacientka, daný tumor a zvolený léčebný přístup tvoří vždy individualitu, jakési jedinečné spojení, jehož chování a z toho plynoucí QoL (Quality of life - kvalita života), DFI (Disease free interval - období bez nemoci) a OS (Overall survival - celkové přežití) je obtížné predikovat. Proto žádné z dosud používaných vyšetření ani žádný v současnosti známý marker nedokáže lékaři ani pacientce přesně odpovědět na tyto základní existenciální otázky, které si pacientka a její nejbližší pokládají nejčastěji., When we detect a tumor in the pelvis we find ourselves faced with the problem of determining both the origin, and biological behavior. The possibility to predict biological behavior of pelvic tumors already before treatment is the key to individual patient management with least patient burden proportional to potentially attainable goal. In the article below we deal only with the gynecological tumors. The main postoperative prognostic and predictive factors such as staging, grading, histological type of tumor, lymph node involvement (as included in staging) and postoperative tumor residue are well known and routinely used. However, if we focus on the possibility of preoperative prediction of tumor biological behavior, it can be based on several factors. These include clinical indicators such as patient age at time of diagnosis, patient performance status, blood “markers” assessment, imaging studies results (particularly ultrasound) and processing of biopsy samples obtained by excision from the tumor during laparoscopy or tru-cut biopsy, etc. Other predictive biomarkers that can generally predict success of our therapy include, e.g. CA125, HE4, ROMA index, presence of the BRCA 1/2 or p53 mutation, Her2/neu, ER/PR or L1CAM positivity, PROVAR (a panel of 9 protein markers), intraepithelial tumor-infiltrating lym - phocytes, preoperative thrombocytosis, neutrophil-to-lymphocyte ratio, Ki67 positivity rate (MIB1), gene expression profiles, protein of resistance, etc. These biomarkers will be discussed below. Since the patient, her tumor and the therapeutic approach always form a unique combination, the resulting QoL (Quality of life), DFI (Disease free interval) and OS (Overall survival) are difficult to predict.
