The article is focused on the key problems of long-term care provided to disabled seniors above 65 years of their age and disabled children in the Czech Republic from informal carers' point of view. The research is based on 25 semi-structured interviews with informal carers. The results show a serious shortage of (social and health care) services generally as well as for informal carers alone, shortage of useful information and medical devices. In some cases, insufficient coordinative and informative role of general practitioners and paediatricians is also a problem. Background: Few data are available on the specific care giving-related problems of stroke patient’s caregivers and factors that influence the burden of these caregivers. Aim: To study the influences of the active rehabilitation process on anxiety, depression, care burden and perceived social support level of stroke patients caregivers. Design: A prospective clinical trial. Setting: Patients and caregivers entering a rehabilitation program at a university hospital in Turkey. Populations: Ninety patients with a first episode of stroke and 90 caregivers responsible for their care were recruited for our study. Methods: Patients and caregivers were assessed before and after the active rehabilitation process. The functional disability level of the patients was assessed by Functional Independence Measure (FIM). The Beck Anxiety Scale (BAS) and the Beck Depression Scale (BDS) were used for anxiety and depression assessment, the Zarit Care Burden Scale (ZCBS) for care burden assessment and the Multi-Dimensional Scale of Perceived Social Support (MDSPSS) for perceived social support assessment. Results: A statistically significant rise is observed in the special person sub-assessment of MDSPSS in both female and male caregivers. Also, a significant decrease in care burden, anxiety and depression levels of caregivers was noted after the rehabilitation program (p < 0.05). Conclusion: Caregivers accept the rehabilitation period as important social support in addition to the support provided by family and friends. Also, our positive results were associated with an improvement in the patients’ functional level and an increase in the acquisition of knowledge and skill required of caregivers in order to provide care during rehabilitation. Clinical Rehabilitation Impact: The rehabilitation team should be aware of the fact that the perceived care burden may be greater due to the lack of knowledge concerning available resources and due to the inability to cope with stress effectively. and A. Cieľ: Príspevok má design prierezovej štúdie, s parciálnym cieľom testovať psychometrické vlastnosti nástroja kvality ţivota (Caregiver Quality of Life Index Cancer) opatrovateľov v starostlivosti o zomierajúcich z hľadiska formy poskytovanej starostlivosti. Metodika: Výskumný súbor tvorilo 263 neprofesionálnych opatrovateľov, poskytujúcich starostlivosť zomierajúcemu v podmienkach domácej, kombinovanej a paliatívnej/hospicovej starostlivosti v troch krajoch Slovenskej republiky (Prešovský, Ţilinský, Banskobystrický). Psychometrické vlastnosti nástroja pouţitého v našej štúdii boli testované na základe analýzy spoľahlivosti, korelačnej a faktorovej analýzy. Výsledky: Faktorovou analýzou sa v našom výskume podarilo potvrdiť 4-faktorovú štruktúru nástroja CQOLC (záťaţ/narušenie ţivotného štýlu; sociálna opora; pozitívna adaptácia; finančná situácia). Extrahované faktory na základe odpovedí respondentov v našom výskumnom súbore nie sú úplne identické s originálnou verziou škály CQOLC. Najlepšiu konštruktovú validitu a vnútornú konzistenciu škály CQOLC sme potvrdili u respondentov, ktorí poskytujú domácu starostlivosť zomierajúcemu príbuznému. Záver: Psychometrickou analýzou dotazníkov sme zistili, ţe reliabilita a konštruktová validita nami pouţitého nástroja je porovnateľná s originálnou verziou, napriek odchýlkam v lokalizácii niektorých poloţiek. Nástroj CQOLC je pouţiteľný na posúdenie kvality ţivota neprofesionálnych opatrovateľov v kontexte starostlivosti o zomierajúcich v domácom prostredí., Aim: The paper has the design of the cross-sectional study. The aim of the study was to test the psychometric properties of the quality of life (Caregiver Quality of Life Index of Cancer) the caregivers who provide care of the dying patients in terms of forms of care. Methods: The sample of our research consisted of 263 non-professional caregivers, providing home care of the dying, combined care and palliative care in three districts of the Slovak Republic (Prešovský, Ţilinský, Banskobystrický). Psychometric properties of the instrument used in our study were tested on the basis of the analysis of the reliability, of the correlation analysis and factor analysis. Results: Factor analysis of questionnaire CQOLC in our research confirmed the fourfactor structure of the instrument CQOLC (burden/lifestyle disruption; social support; a positive adaptation; the financial concerns). Extracted the factors on the basis of the answers of the respondents in our research are not completely identical with the original version CQOLC. The best construct validity and reliability of the CQOLC we have confirmed with the caregivers, who provide a home care of the dying patients. Conclusion: Psychometric analysis of the questionnaires we found that, the reliability and construct validity is comparable to the original version of CQOLC, despite variations in the location of some items. Stárnutí populace má ekonomické, politické i sociální důsledky. Na demografické změny reaguje plánování potřeb dlouhodobé péče včetně úvah o počtu profesionálních pečujících. Ačkoli v řadě zemí Organizace pro hospodář skou spolupráci a rozvoj (OECD) včetně ČR převládá péče rodinných pečujících, množství profesionálů se má v těchto zemích do roku 2050 zdvojnásobit (v ČR dokonce ztrojnásobit). Potřeba existence kvalitních institucí zajišťujících dlouhodobou péči je provázána s nastavením příznivých pracovních podmínek a podporou dobré zdravotní kondice jejich zaměstnanců. Prevence a podpora zdraví jak stárnoucí populace, tak formálních i neformálních pečujících, představují významná témata v politice dlouhodobé péče. V rámci reflexe současného stavu podpory zdraví a pracovních podmínek formálních pečujících v českém prostředí jsme provedli kvalitativní sondu (focus groups), jejímž cílem bylo zjistit subjektivní názory této cílové skupiny na snahy zaměstnavatelů o podporu jejich zdraví formou zkvalitňování pracovních podmínek. Klíčovým tématem byla úroveň podpory zdraví na pracovišti a s ní související pozitivní a negativní faktory ovlivňující pracovní výkon., Population aging has economic, political and social consequences. Planning needs of long term care (LTC) and reflecting the number of professional caregivers responds to demographic changes. Although family caregivers dominate in many OECD countries including the Czech Republic, a number of professionals in these countries will double until 2050 (even triple in the Czech Republic). The need for the existence of quality institutions providing LTC setting is linked with good working conditions and health promotion of their employees. Prevention and health promotion of the aging population and of formal and informal caregivers are important topics in LTC policy. Within reflection of the current state of health promotion and working conditions of formal caregivers in the Czech environment, we conducted a qualitative probe (focus groups). The objective was to determine subjective opinions of this target group of employers’ efforts to promote their health by improving working conditions.
