Diabetes mellitus je závažným zdravotnickým a ekonomickým problémem současnosti. Terapie je vnímána primárně jako prevence pozdních komplikací diabetu. Většina farmakoekonomických analýz uvádí, že náklady spojené s terapií diabetes mellitus jsou z větší části spotřebovány na terapii komplikací. Projekt Národního diabetologického informačního systému vychází ze základního principu automatizovaného sběru dat, která jsou k dispozici v České republice v elektronické podobě a která současně popisují kvalitu a kvantitu péče poskytovaného pacientů s diabetes mellitus i s jejími výsledky. Výhodou postulovaného Národního diabetologického informačního systému je minimalizace selektivní ztráty dat, komplexní pohled na epidemiologii, terapii a její výsledky, možnost sledování trendů, zahrnutí nákladů spojených s hospitalizací a sociálními dávkami. Rizikem zůstává správná interpretace a možnost systémové chyby. Jestliže by se podařilo v České republice podobný systém uskutečnit, bude to první takto komplexní plně automatizovaný informační systém na světě. Závěr: Data popisující incidenci a prevalenci diabetes mellitus, strukturu terapie, náklady přímé na zdravotní péči i indukované na sociálních platbách jsou v naprosté většině k dispozici v elektronické podobě. Vzniká tak možnost založení Národního diabetologického informačního systému, který umožní kontinuální vyhodnocování kvality péče o pacienty s diabetes mellitus včetně farmakoekonomických a sociálních důsledků. Therapy is primarily perceived as prevention of the late complications of diabetes. Most pharmacoeconomic analyses state that the cost associated with the therapy of diabetes mellitus is largely spent on the therapy of its complications. The project of the National Diabetes Information System is based on the essential principle of automated collection of data which is available in digital form in the Czech Republic and which describes the quality and quantity of the care provided for patients with diabetes mellitus and its results. Benefits of the posited National Diabetes Information System embrace minimizing of selective loss of data, a comprehensive view of epidemiology, therapy and its results, possibility of following trends, inclusion of costs related to hospitalization and social benefits. Risks involve correct interpretation and a possible system error. If we succeeded in implementing such system in the Czech Republic, it would be the first fully automatic information system on such a comprehensive scale worldwide. Conclusion: The data describing the incidence and prevalence of diabetes mellitus, therapy structure, direct healthcare costs and induced costs of social payments is in the largest part available in digital form. This creates a basis for the foundation of the National Diabetes Information System which will allow for continuous quality assessment of care for patients with diabetes mellitus, including the related pharmacoeconomic and social impacts. Whole cell patch-clamp recordings from GABAergic cells of thalamic reticular nucleus (RTN) in thalamocortical slices made from postnatal day 6 (P6) to 10 (P10) were used to investigate the pattern of rebound bursts (RBs) triggered by an injection of hyperpolarizing current into RTN cells. The number of RBs in the RTN and the overlying Na+/K+ spikes changed in an agedependent manner. The generation of RBs depended largely on the amplitude of the after-hyperpolarizations (AHPs). RB patterns in response to hyperpolarizing current injection into relay cells were markedly different from RB patterns in RTN cells with an after-depolarization. GABAA receptor antagonist bicuculline methiodide (BMI) changed burst firing patterns, increasing the duration of RB and decreasing the amplitude of AHP in RTN cells. Furthermore, local puffs of NMDA in the presence of BMI induced RBs. K+ channel blocker 4-aminopyridine partially mimicked the effect of BMI on AHPs. Micro-displacement monitoring in Slopy a nd Zbojnícka caves is described. It st arted at the end of 2005 with the use o f verified, stable and sensitive 3D crack gauges TM71 produced by GESTRA Sedloňov. Two gauges were installed in the caves across significant tectonic structures. Registered displacements are pertinent to the last significant earthquake events. The f irst one appeared in March 13, 2006 at Vrbové, M=3.2; and later in August 5-8, 2006, a swarm of 10 quakes M max = 2.2 at Trstín was registered. The paper is focused to the analysis of permanent and epoch-wise GPS networks extending over the region of Slovakia. We considered almost all available GPS data measured in Slovakia and its close surroundings over the period of last 17 years. Our analysis is based on combination of homogenized permanent and epoch-wise network solutions leading to the ITRF2005 related coordinates, velocities and their global covariance matrix. Estimated velocities were further reduced for APKIM2005d plate motion model and submitted to the velocity filtering and smoothing using the least square collocation approach. The obtained homogenized intra-plate velocity field is then used for the surface deformation analysis. The paper presents results of epoch satellite GPS and gravimetric measurements performed on the geodynamic network in central part of the Stołowe Mts. between 1993 and 2009. The research results show significant changes of gravity on most of the points and significant horizontal movement of one point in the central part of the area. The results confirm present day activity of the zone where faults Polický, Bělský and Czerwona Woda Fault Zone exists. The spontaneously hypertensive rat (SHR) is the most widely used animal model of essential hypertension an d accompanying metabolic disturbances. Recent advances in sequencing of genomes of BN- Lx and SHR progenitors of the BXH/HXB recombinant inbred (RI) strains as well as ac cumulation of multiple data sets of intermediary phenotypes in the RI strains, including mRNA and microRNA abundance, quantitative metabolomics, proteomics, methylomics or histone modifications, will make it possible to systematically search for genetic variants involved in regulation of gene ex pression and in the etiology of complex pathophysiological traits. Distributed control and measurement system for evaluation of hydrologic, geomechanical and other parameters has been built up in the medieval ore mine Jeroným. The instrumentation is generally based on commercial measurement and control kit and sensors. Technique of implementation of special sensors, e.g. laser distance meter and/or CCBM probe for measurement of rock massif stress changes, is also described. The distributed system is integrated into the existing seis mic recording station equipped by data transmission via GSM network. Three year monitoring of micro-displacements on four tectonic fracture planes in a tunnel driven by a milling cutter into granitoids of Bohemian Massif revealed micro-movements that develop in certain trends and impulses. Two investigated fractures are of the Krušné Hory Mts. orientation (NE - SW), other two of Sudeten orientation (NW - SE). These have been found the two prevailing fracture orientations in the massif. Results from all the four fractures indicate trends in overfaulting of southern blocks over the northern ones and a stress state model characteristic of approximate N-S compression with overthrust vergence to N. Besides, some impulses were indicated with abrupt occurrence that dominates the development of movements. The first most significant impulse occurred by the end of the year 2004, the second one at the break of 2005/6.
