Akromegalie patří mezi vzácná a závažná onemocnění, základem její léčby je kombinace terapie chirurgické, radiační a medikamentózní. V práci se popisuje případ akromegalie diagnostikované před 10 lety ve stadiu, v němž byla možná již jen parciální resekce hypofyzárního tumoru. Přesto se podařilo dosáhnout dlouhodobé stabilizace stavu kombinací opakovaného chirurgického zákroku, radioterapie lineárním urychlovačem i Leksellovým gama nožem a medikamentózní léčby somatostatinovým analogem oktreotidem a blokátorem receptorů pro růstový hormon pegvisomantem. Dávka oktreotidu i pegvisomantu byla opakovaně měněna podle výsledků vyšetření hladiny IGF1, v současnosti je podáváno somatostatinové analogon Sandostatin LAR 30 mg jednou za 3 týdny a blokátor receptorů pro růstový hormon Somavert 20 mg denně. Přes přítomnost závažné choroby nemocný již po 10 let vede spokojený život. Náklady na finančně náročnou léčbu jsou z etického hlediska plně odůvodněné., Acromegaly is a rare and serious disease. A successful and rational therapy of acromegaly ought to combine surgery, radiotherapy and pharmacotherapy. The submitted article presents a case of acromegaly that was only diagnosed at the stage when total pituitary adenoma removal was impossible. Even so, the long-term stabilisation of the disease was reached by way of repeated surgery through transfenoidal and transcranial approach, by linear accelerator radiation therapy and Leksell Gamma Knife radiotherapy and by pharmacotherapy with somatostatin analogon octreotide and growth hormone receptor antagonist pegvisomant. The octreotide and pegvisomant dosage has been repeatedly changed according to IGF1 levels. The contemporary somatostatin analogon Sandostatin LAR 30 mg is given once every 3 weeks and the growth hormone receptor antagonist Somavert 20 mg is applied daily. Despite this serious disease, the patient has already been living contentedly for 10 years. Akromegalie je vzácné onemocnění způsobené autonomní nadprodukcí růstového hormonu, ve většině případů hypofyzárním adenomem. Neléčená akromegalie vede ke zvýšené morbiditě a mortalitě a také zhoršené kvalitě života pacientů. Pro prognózu pacientů je proto nezbytné včasné stanovení diagnózy a časná léčba a péče o pacienty s akromegalií by měla být koncentrována do specializovaných center. Článek shrnuje současný pohled na diagnostiku a léčbu akromegalie., Acromegaly is a rare disorder caused by autonomous oversecretion of growth hormone mostly by pituitary adenoma. Untreated acromegaly leads to significantly increased morbidity and mortality and impaired quality of life. Early diagnosis and treatment is therefore essential for improvement of patients’ prognosis and management of acromegaly should be concentrated in specialized centres. Because greater Akt substrate of 160 kDa (AS160) phosphorylation has been reported in insulin-stimulated skeletal muscles without improved Akt activation several hours post-exercise, we hypothesized that prior exercise would result in attenuated AS160 dephosphorylation in insulin-stimulated rat skeletal muscle. Epitrochlearis muscles were isolated from rats that were sedentary (SED) or exercised 3 h earlier (3 h postexercise; 3hPEX). Paired muscles were incubated with [3H]-2-deoxyglucose (2-DG) without insulin or with insulin. Lysates from other insulin-stimulated muscles from SED or 3hPEX rats were evaluated using AS160Thr642 and AS160Ser588 dephosphorylation assays. Prior exercise led to greater 2-DG uptake concomitant with greater AS160Thr642 phosphorylation and a non-significant trend (P=0.087) for greater AS160Ser588. Prior exercise also reduced AS160Thr642 and AS160Ser588 dephosphorylation rates. Considerable evidence demonstrates that phenotypic switching of vascular smooth muscle cells (VSMCs) is influenced by aging and hypertension. During phenotypic switching, VSMCs undergo a switch to a proliferative and migratory phenotype, with this switch being a common pathology in cardiovascular diseases. The aim of this study was to explore the joint influence of age and hypertension on thoracic aortic smooth muscle phenotypic switching and the balance of Akt and mitogen-activated protein kinase (MAPK) signaling during this switch. Different ages of spontaneously hypertensive rats (SHR) and Wistar-Kyoto rats (WKY) were used to establish hypertension and aging models. The phenotypic state was determined by detecting the marker proteins α-SM-actin, calponin, and osteopontin (OPN) via immunohistochemical staining and Western blot. Signaling proteins associated with the Akt and MAPK pathways were detected in rat thoracic aorta using Western blot. Both aging and hypertension caused a decrease in contractile (differentiated) phenotype markers (α-SM-actin and calponin), while the synthetic (proliferative or de-differentiated) phenotype maker was elevated (OPN). When combining hypertension and aging, this effect was enhanced, with Akt signaling decreased, while MAPK signaling was increased. U pacientů s Alzheimerovou nemocí (AN) se postupně zhoršuje soběstačnost v běžném životě, a tak vzrůstá závislost na pomoci druhé osoby - pečovatele. Pečovatelé 24 pacientů s AN vybrali z nabídky 34 různých aktivit denního života ty položky, které byly u jejich pacientů nejvíce zachované, nejvíce postižené a které nejvíce obtěžovaly pečovatele. Současně se u pacientů s AN měřila kognitivní výkonnost pomocí testu Mini-Mental State Examinati on (MMSE). Nejméně se choroba dotýkala základních aktivit, zatímco nejvíce postižená byla orientace v čase. Dezorientace v čase byla rovněž nejvíce obtěžující pro pečovatele pacientů s lehčí demencí (MMSE 18- 25), zatímco pečovatelům pacientů s těžším kognitivním deficitem (MMSE 0- 17) vadila zejména narušená komunikace. Tyto poznatky moho u být využitelné pro diagnostiku Alzheimerovy nemoci, při sociální péči o pacienty a k většímu porozumění problémům pečovatelů., In patients with Alzheimer's disease (AD) the ability to perform activities of daily living (ADL) deteriorates causing them to become increasingly dependent on their caregivers. 24 caregivers of patients with AD were given a questionnaire on 34 ADL with the task of stating which activities are the most preserved and the most impaired in their patients. They also identified which activity?s impairment was the biggest source of burden for caregivers themselves. The global cognitive performance of AD patients was measured using the Mini-Mental State Examination (MMSE). The least impact of the disease was seen in basic ADL, while orientation in time was most affected. Impaired orientation in time was found the most troublesome by the caregivers of patients with milder dementia (MMSE 18-25), while caregivers of patients with more pronounced dementia suffered predominantly from impaired communication. Rozlišení mezi aktivní a pasivní eutanazií je již po mnoho let předmětem intenzivních diskusí a nepřestává vzbuzovat kontroverze. Tyto kontroverze se týkají dvou důležitých distinkcí: na jedné straně je zde problém deskriptivní adekvátnosti načrtnutého rozlišení mezi aktivní a pasivní eutanazií, na straně druhé potom známý problém morální ekvivalence základnějších kategorií jednání, na jejichž základě je možné toto rozlišení načrtnout. Tento článek se věnuje prvnímu ze zmíněných problémů a klade si následující otázku: existují nějaké základnější kategorie jednání, do nichž aktivní a pasivní eutanazie spadají? Existuje celá řada pokusů analyzovat rozdělení aktivní-pasivní eutanazie, někteří autoři se pokoušejí založit ho na rozlišení mezi jednáním a nejednáním, jiní zase na konkrétnějších kategoriích usmrcení a ponechání zemřít. Během kritického hodnocení těchto návrhů ukazuji, že jsou neadekvátní a neposkytují korektní kritérium demarkace. Navrhuji, že bychom namísto toho měli věnovat pozornost návrhu britské filosofky Philippy Footové a zaměřit se na pojem fatální sekvence vedoucí obecně k újmě, konkrétněji ke smrti. Na základě nejnovějších analýz Fiony Woollardové v její monografii Doing and Allowing Harm a analýz odstranění bariéry Jeffa McMahana docházím ke konečnému závěru, že rozlišení aktivní/pasivní eutanazie je deskriptivně adekvátní a je možné ho založit na pojmech fatální sekvence, podstatných a nepodstatných faktů relevantních k této sekvenci a kauzálních a normativních faktorů spojených s odstraněním bariéry, např. plicního ventilátoru. Etický význam tohoto rozlišení bude předmětem kritické diskuse v druhé části tohoto článku., The distinction between active and passive euthanasia has been the subject of intensive discussions for many years and the issue continues to raise controversies. In dealing with this controversy two important distinctions have to be made: first, there is the problem of descriptive adequacy of drawing a distinction between active and passive euthanasia and second, there is the famous problem of moral equivalence of the more basic categories of actions on the basis of which the distinction in parole can be drawn. This article deals with the first of the aforementioned problems: are there more basic categories of action under which active and passive euthanasia fall? There have been many attempts to analyse the distinction between active and passive eut0hanasia, some authors tried to base it on the distinction between actions and omissions, others on the more concrete categories of killing and letting die. In my critical evaluation of these proposals I show that they are inadequate and fall short of providing a sound criterion of demarcation. I propose instead that we should pay attention to the proposal of the British philosopher Philippa Foot and concentrate on the notion of a fatal sequence leading to harm in general and death in particular. After taking into account the newest analysis by Fiona Woollard in her monograph Doing and Allowing harm and the analysis of removing a barrier by Jeff McMahan I finally reach the conclusion that the distinction active/passive euthanasia is descriptively adequate and can be stated in terms of fatal sequences, substantive and non-substantive facts relevant to these sequences and causal and normative factors regarding removing barriers, for example ventilators.
