Úvod: U 60 % nemocných s kolorektálním karcinomem jsou diagnostikovány jaterní metastázy (JMKRK), ať už v době stanovení diagnózy, nebo v průběhu léčby. Chirurgická léčba je jedinou potenciálně kurabilní metodou s pětiletým přežíváním dosahujícím cca 50 %. Jaterní resekci však podstoupí méně než 20 % pacientů s JMKRK. Společnou snahou onkologů a chirurgů je navýšit počet resekovaných pacientů. Jedním z postupů, jak navýšit resekabilitu JMKRK, je „liver first approach“ (LFA). Autoři prezentují vlastní předběžné výsledky této metody. Metody: Do multicentrického grantu IGA MZ NT 13660 – Hodnocení kvality multimodální péče u nemocných s jaterními metastázami kolorektálního karcinomu – bylo od září 2012 do ledna 2015 zařazeno v ÚVN 102 pacientů. LFA jsme použili u 12 pacientů (12 %). Po příznivé léčebné odpovědi neoadjuvantní systémové léčby jsme pacienty indikovali k resekci jater. U 11 pacientů se jednalo o mnohočetné (>4 meta) bilobární postižení. V jednom případě se jednalo o objemnou solitární metastázu pravého jaterního laloku. Primární tumor byl u 9 nemocných v rektu, u 3 v oblasti rektosigmatu, 3 pacienti měli derivační kolostomii, ostatní byli bez poruchy pasáže. Výsledky: U 11 pacientů jsme provedli R0 resekci jaterních metastáz, 3x jsme indikovali dvoufázovou hepatektomii s portální embolizací (PVE), 1x jsme pro progresi po PVE ustoupili od 2. fáze). Velkou resekci jsme provedli 7x, neanatomickou resekci včetně RFA 11x (6x v kombinaci s velkou resekcí). Perioperační mortalita byla nulová, morbidita 33 % (Dindo-Clavien >2). 10 pacientů absolvovalo adjuvantní chemoterapii, 7 z nich včetně radioterapie malé pánve pro lokálně pokročilý tumor rekta. U 7 pacientů (58 %) jsme doplnili resekci primárního tumoru. V současné době zemřeli na progresi onemocnění 2 pacienti (17 %), progresi onemocnění jsme zaznamenali u 6 pacientů (50 %). Závěr: LFA považujeme za vhodný především u pacientů s generalizovaným tumorem rekta, kdy lze po resekci jater adjuvantní systémovou léčbu kombinovat s radioterapií malé pánve. Otázkou zůstává načasování operace primárního tumoru, kdy je nutné vyloučit recidivu jaterních metastáz, která se objevuje u více než 50 % pacientů. Profit LFA musí být prokázán randomizovanými studiemi., Introduction: Liver metastases are diagnosed in 60% of patients with colorectal cancer, both at the time of diagnosis or later in the course of their management. Surgical treatment is the sole potentially curable method with 5–year overall survival of approximately 50%. However, only less than 20% of patients underwent liver resection. A joint effort of medical oncologists and surgeons is to increase the numbers of resected patients. The “liver first approach” (LFA) is one of approaches aimed at increasing resecability. The authors present their preliminary results using this method. Methods: 102 patients were included in the multicentre study supported by the grant IGA NT 13660 − Evaluation of quality of multimodal treatment for patients with colorectal cancer liver metastases – conducted at the Central Military Hospital between September 2012 and January 2015. We used LFA in 12 patients (12%). Patients were indicated for liver resection based on good response to neoadjuvant systemic therapy. Multiple bilobar liver involvement (>4 metastases) was present in 11 cases and one large solitary metastasis in the right liver lobe in one case. The primary tumor was located in the rectum in 9 patients, in the rectosigmoid in 3 patients; 3 patients had a colostomy. Others showed no signs of bowel obstruction. Results: We have performed R0 resections in 11 cases, and two-stage hepatectomy with portal vein embolisation was indicated 3 times (in one case we did not finish the second stage due to quick progression after PVE). We performed major resections 7 times, along with sever extraanatomic resections, incl. 11 RFA (6 times in combination with major resections). Perioperative mortality was 0%, morbidity 33% (Dindo-Clavien >2). Ten patients underwent adjuvant chemotherapy, in 7 cases including radiotherapy of the small pelvis due to a local advanced primary tumor. Resection of the primary tumor was done in 7 patients (58%). Two patients died recently because of disease progression (17%); progression was observed in 6 patients (50%). Conclusion: We deem the LFA suitable especially for patients with metastatic rectal tumors where adjuvant systemic therapy can be combined with radiotherapy. The timing of the resection of the primary tumor still remains a question: it is necessary to rule out potential recurrence of liver metastases, which affected more than 50% of the patients. Takotsubo kardiomyopatie je náhle vzniklé onemocnění charakterizované reverzibilní poruchou kinetiky levé srdeční komory. V typickém případě se rozvíjí následkem silného stresového podnětu a prezentuje se symptomy, EKG a laboratorním nálezem, které připomínají akutní koronární syndrom. Od toho se liší nepřítomností onemocnění koronárních tepen a ve většině případů nekomplikovaným průběhem s následnou velmi dobrou dlouhodobou prognózou. V kazuistice popisujeme případ 63leté ženy, u které se počátek onemocnění projevil fibrilací komor. Díky okamžité kardiopulmonální resuscitaci a následné odborné péči ve specializovaném kardiocentru, která mimo jiné zahrnovala implantaci implantabilního kardioverteru-defibrilátoru v rámci sekundární prevence náhlé srdeční smrti, se pacientka zotavila bez jakýchkoliv následků. Takotsubo kardiomyopatie je důležitou jednotkou v diferenciální diagnóze akutního koronárního syndromu. I přes dobrou prognózu, kterou se vyznačuje ve většině případů, ji nelze podcenit a zaslouží si náležitou pozornost a léčbu, která umožní předcházet případným komplikacím., Takotsubo cardiomyopathy (TTC) is an acute syndrome characterized by rapid onset of transient systolic dysfunction of the left ventricle. Symptoms, ECG and laboratory findings resemble acute coronary syndrome, from which TTC differs by the absence of coronary artery disease. In typical cases, TTC is triggered by exposure to unexpected stress and the clinical course and prognosis are very good. We present a case of a 63-year-old woman, in whom the onset of the disease was complicated by ventricular fibrillation. The patient recovered without sequelae thanks to immediate cardiopulmonary resuscitation and following care in a specialized cardiocenter, which involved also implantation of implantable cardioverter defibrillator to prevent sudden cardiac death. Takotsubo cardiomyopathy is an important entity in the differential diagnosis of acute coronary syndrome. Záštitu nad projektom prevzala Slovenská diabetologická spoločnosť a slovenská kancelária WHO ako jeden z edukačných programov v rámci Národného diabetologického programu. Cieľom projektu bolo zlepšiť informovanosť mladých ľudí o ochorení diabetes mellitus, zmapovať ich zdravotný stav a podporiť zdravý životný štýl (zdravé správanie) ako významný komponent v prevencii proti uvedenému ochoreniu. Small bowel obstruction is a common clinical problem presenting with abdominal distention, colicky pain, absolute constipation and bilious vomiting. There are numerous causes, most commonly attributed to an incarcerated hernia, adhesions or obstructing mass secondary to malignancy. The aim of this study was to compare the isolation systems OptraDam® Plus and OptiDam™ with the conventional rubber dam in terms of objective and subjective parameters. The isolation systems were applied during the dental treatment of the patients. The time of preparation, placement, presence and removal were measured and the quality of isolation was evaluated. The median time of rubber dam placement was 76 s (Q1=62 s; Q3=111.25 s). The application time of OptraDam® Plus was significantly longer compared to the other systems (P ® plus. BACKGROUND: Radio frequency (RF) and chemical peels have been used for nonablative skin rejuvenation. Both of these cause collagen remodeling in the dermis and neo-collagen formation resulting in facial rejuvenation. There is limited literature on the evaluation of collagen remodeling by objective methods. OBJECTIVE: To compare the benefits of monopolar radiofrequency and glycolic acid peels in facial rejuvenation with regards to histopathology and Ultrabiomicroscopic sonography (UBM). METHODOLOGY: In this study, forty patients with mild to moderate photoaging received four treatments with 3 weeks interval of monopolar radiofrequency on one side of face and glycolic acid peels in increasing concentrations (NeostrataR) on the other side. Pre and post treatment, 2 mm biopsies were taken from both preauricular areas and Ultrasonography using a 35 MHz probe was done from outer canthus of eye and nasolabial folds from both sides of face. A blinded assessment was done to measure the increase in the grenz zone and dermal thickness. RESULTS: In 35/40 patients there was a significant increase in the grenz zone on histopathology and decrease in subepidermal low-echogenic band (SLEB) on UBM of the nasolabial folds on both sides of the face (p < 0.05). Cíl: Publikace případu falešně zvýšené koncentrace bilirubinu u pacientky s mnohočetným myelomem. Metoda: U pacientky s diagostikovaným a léčeným mnohočetným myelomem byla stanovena atypicky vysoká hodnota sérového bilirubinu diagnostickým kitem Roche na přístroji Modular a normální, o dva řády nižší hodnota téhož analytu diagnostickým kitem Olympus na přístroji Olympus 400. Výsledky: Pacientka měla v krvi koncentraci paraproteinu IgG kappa 61,6 g/l. Sérový bilirubin, stanovený na přístroji Modular měl hodnotu 546 μmol/l, zatímco při opakovaném měření na přístroji Olympus 400 byla hodnota u stejné pacientky 6 μmol/l. Nižší hodnota korespondovala s klinickým stavem pacientky. Diskuse a závěr: Oba výrobci upozorňují ve svých pracovních návodech na potenciální možnost pozitivních interferencí v přítomnosti IgM lambda. Zjistili jsme extrémní pozitivní chybu o velikosti dvou řádů v přítomnosti paraproteinu IgG kappa u jednoho z nich (Roche). Chyba je snadno detekovatelná inspekcí záznamu kinetiky reakčního průběhu a neměla by ujít pozornosti při vydávání výsledků., Objective: Falsely elevated serum bilirubin in patient with multiple myeloma. Method: We determined serum bilirubin in a patient with multiple myeloma by using Modular and we repeated analysis by using Olympus 400. Results: Concentration of IgG kappa was 61.6 g/l. Serum bilirubin value measured on the Modular Roche has been 546μmol/l, while repeated measurement on Olympus 400 provided value only 6 μmol/l. Lower Olympus value corresponded to clinical status of the patient. Discussion and conclusion: Both producers introduce in their working manual the possibility of IgM lambda interference for bilirubin measurement. We found extremely high systematic error in presence IgG kappa only in Modular Roche, not in Olympus 400. Hereditární hemoragická teleangiektázie neboli morbus Rendu‑Osler‑Weber je fibrovaskulární dysplazie s autozomálně dominantní dědičností charakteristická přítomností teleangiektázií a arteriovenózních malformací kůže, sliznic a vnitřních orgánů. Plicní arteriovenózní malformace jako zdroj septických embolů mohou zapříčinit vznik mozkového abscesu, který tak bývá prvním a často jediným projevem heriditární hemoragické teleangiektázie. V práci jsou prezentovány tři kazuistiky pacientů trpících touto chorobou, kde prvním a závažným příznakem byl vzniklý mozkový absces. Protože se jedná o relativně vzácné onemocnění, nebývá vždy zvažováno v rámci diferenciální diagnostiky a tím pádem ani diagnostikováno. U každého pacienta s mozkovým abscesem je nezbytné na toto onemocnění pomýšlet a v případě podezření je nutné jej řádně vyšetřit. Při verifikované plicní arteriovenózní malformaci je třeba ji ošetřit a zabránit tak vzniku recidivujících mozkových abscesů. Péče o tyto pacienty je komplexní a multioborová., Hereditary hemorrhagic telangiectasia or Rendu‑Osler‑Weber disease is an autosomal‑dominant disorder that results in fibrovascular dysplasia and is characterized by telangiectases and arteriovenous malformation of the skin, mucosa and viscera. Pulmonary arteriovenous malformations as a source of septic emboli may give rise to brain abscess that so often is the first and only clinical manifestation of hereditary hemorrhagic telangiectasia. Three cases of patients suffering from this disease with brain abscess as the first and serious symptom are presented. Since this is a relatively rare disease, it is not always considered in the differential diagnosis and thus not diagnosed. This disease has to be considered in patients with brain abscess and, in case of a suspicion, it must be properly examined. When pulmonary arteriovenous malformation is verified, it has to be treated in order to prevent recurrent brain abscesses. Care of these patients should be comprehensive and multidisciplinary. In the last decade the health of young people in Ukraine as well as in many other countries of the world significantly deteriorated according to the results of screening tests. Because of this the main task of prophylactic medicine is development and introduction of new methods of early diagnostics of diseases. The aim of the work to assess the peculiarities of adaptation of students with different types of vegetative regulation. Material and methods: 415 second year students of Lugansk State Medical University were tested. The mechanisms of physiological functions of the body regulation were assessed by registration of cardiac rhythm variability (CRV). Analysis of CRV was performed by three methods: statistical, geometrical, and spectral analysis. The express method proposed by Shlyk, N.I. (2009) was later used to determine the prevailing type of vegetative regulation. To assess adaptability the method of R.M. Baevskij (1979) was used. Results: In the result of investigation 4 types of functional states of regulatory systems were determined: I type 295 (71 %) of tested, II in 14 (3 %), III in 96 (23 %) and IV only in 10 (2%). I type of functional state of regulatory systems in the wakeful state at rest was characterized by moderate prevalence of central regulation of heart rhythm (MPCR), decreased activity of autonomic regulation. It was noticed that some of the parameters of CRV (like RR, SDNN, RMSSD and pNN50) which characterize autonomic regulation, were statistically lowere (р≤0.01) compared to type III, and some (like AMo and SI) higher, in both female and male students. Summarized spectrum capacity and components of its wave structure (HF, LF, VLF) which characterize central regulation were statistically significantly lower (р≤0.01) for the I type of regulation compared to the III one which is characterized by moderate prevalence of autonomic regulation of the heart rhythm. In female students with type I autonomic regulation compared to male students such CRV parameters as RR and AМo were statistically significantly lower (р≤0.05), while the parameters of total potency (TP) and high frequency waves (HF) were higher (р≤0.05), which could point to more centralized heart rhythm regulation in male compared to female. The evaluation of regulatory systems overstress allowed to determine the state of adaptation and risk of overstress or breakdown in students with different types of autonomic regulation. V souboru 39 případů adenokarcinomu in situ děložního hrdla byly sledovány: věk pacientek, cytologické, kolposkopické a histologické nálezy, operační výkony a definitivní řešení. Medián věku byl 38 let (24–75). Prebioptická suspekce žlázové léze byla vyslovena v 7 cytologických (18 %) a v 7 kolposkopických (18 %) nálezech. U 28 nemocných (72 %) byla současně přítomna koexistující skvamózní léze. V 16 případech (46 %) z 35 konizací byly popsány histologicky pozitivní okraje léze a u 15 z nich byl proveden opakovaný nebo definitivní operační výkon. V něm byl ve 2 případech již invazivní adenokarcinom. Klíčová slova: adenokarcinom in situ – cytologie – děložní hrdlo – kolposkopie – pozitivní okraje, In a series of 39 cases of adenocarcinoma in situ of the uterine cervix, age of patients, cytological, colposcopical and histological findings, operations and definitive solutions have been analyzed. Median age was 38 (24 – 75). Praebioptic suspicion of a glandular laesion was expressed in 7 cytological (18 %) and 7 colposcopical (18 %) findings. In 28 patients (72 %) the coexistent squamous laesion was concurrently present. In 16 cases (46 %) from 35 conisations histologically positive margins were described and in 15 of them repeated or definitive operation was performed.
