Cíl práce: Flavonoidy, obsažené v preparátu Comfort-G, působí nejen jako antioxidanty, ale mají také protizánětlivý účinek. Comfort-G inhibuje nejen cyklooxygenázu-2 (COX-2), ale také 5-lipoxygenázu (5-LO), a to tím, že snižuje tvorbu prostaglandinů, tromboxanů a leukotrienů, pocházejících z metabolismu kyseliny arachidonové (AA). Naším úkolem bylo potvrdit toto tvrzení analýzou molárního zastoupení AA v synoviální tekutině a v séru pacientů, léčených pro synovialitidu kolenního kloubu. Typ studie: Intervenční studie. Název a sídlo pracoviště: IV. interní klinika, Univerzita Karlova v Praze, 1. lékařská fakulta a Všeobecná fakultní nemocnice v Praze Materiál a metody: Comfort-G byl podáván skupině deseti pacientů (4 muži/6 žen) ve věku 27–79 let v předepsané dávce (odpovídající 250 mg extraktu flavonoidů užívaných dvakrát denně) po dobu jednoho měsíce. Odběr synoviální tekutiny a séra se uskutečnil jednak v den 0, tj. před podáním první dávky preparátu a v den 31., jeden den po poslední dávce. Volné radikály byly stanoveny přímou spektrofotometrickou metodou založenou na chlorofylinové reakci. Molární poměr mastných kyselin včetně arachidonové kyseliny byl stanoven plynovou chromatografií. Účastníci studie podstoupili klinické vyšetření a poskytli subjektivní hodnocení pomocí dotazníku, které se týkalo stavu kolenního kloubu. Statistická analýza byla uskutečněna softwarem STATISTICA pro Windows, verze 4.0. Výsledky: Výsledky, ukazující statisticky významný pokles koncentrace volných radikálů, a to jak v synoviální tekutině, tak v séru, jsme prezentovali a diskutovali v předchozí studii, stejně jako zlepšení stavu pacientů. V této práci přinášíme výsledky měření obsahu kyseliny arachidonové v synoviální tekutině a séru ve stejné skupině pacientů. Po ukončení studie jsme zjistili zvýšené zastoupení AA jak v synoviální tekutině (4,54 ± 0,39 vs 6,73 ± 0,83 mol%, p = 0,005), tak v séru (4,78 ± 0,90 vs 6,57 ± 0,98 mol%, p = 0,01) ve sledované skupině pacientů. Závěr: Naše výsledky potvrdily, že flavonoidy, obsažené v přípravku Comfort-G, jsou schopny snižovat množství volných radikálů a zvyšovat zastoupení kyseliny arachidonové, a to zejména v synoviální tekutině, ale i v séru pacientů. Toto zjištění souhlasí s tvrzením, že následující enzymatické reakce AA, které produkují prozánětlivé sloučeniny, jsou zřejmě zablokovány. Další souvislosti, týkající se jiných částí organismu, a význam našich výsledků pro jiná další onemocnění jsou předmětem diskuse, Objective: Flavonoids from preparation Comfort-G have, beside of antioxidant effects, the anti-inflammatory effect, too. Comfort-G inhibits both COX-1 (cyclooxygenase-1)/COX-2 (cyclooxygenase-2) and 5-LO (5-lipoxygenase) enzymatic activity thereby decreasing the transformation of arachidonic acid (AA) to prostaglandins, thromboxanes, and leukotrienes. Our goal was to confirm this finding by the analysis of the content of AA in synovial fluid and serum of patients suffering by synovialitis of knee joint. Design: Interventional study. Settings: IV Department of Internal Medicine, Charles University in Prague, First Faculty of Medicine and General Teaching Hospital in Prague. Material and Methods: Comfort-G was given to the group of ten patients (4M/6F) aged 27–79 years in prescribed dose (equal to 250 mg of flavonoid extract twice a day) for one month. Synovial fluid and serum were drawn before first application of the preparation (day 0) and one day after the last application (day 31). Free radicals (FR) were determined by direct spectrophotometric method based on chlorophyllin reaction with FR; composition of fatty acids, including arachidonic acid, was measured by gas chromatography. The probands were also asked to respond to a questionnaire (subjective evaluation) and they were also examined by the physician. Following changes of knee joint were evaluated: swelling and degree of motion-angle. Statistical analysis was performed with the help of software STATISTICA for Windows, version 4.0. Results: The results showing the statistically significant decrease of free radical concentration in both synovial fluid and serum were presented and discussed in previously presented paper as well as the observed amelioration of patient stage. After the trial period, we found increased content of AA in both synovial fluid (4.54 ± 0.39 vs. 6.73 ± 0.82 mol%, p = 0.005) and serum (4.78 ± 0.90 vs. 6.57 ± 0.98 mol%, p = 0.01) in the same group of patients. Conclusion: Our results confirm that flavonoids from Comfort-G are able to decrease the amount of FR and increase the concentration of AA especially in synovial fluid, but also in serum of patients. This finding corresponds with the suggestion that the enzymatic transformations of AA resulting in production of pro-inflammatory compounds are blocked. Fatty acid (FA) profiles as potentially relevant components of Crohn’s disease (CD) have been insufficiently analysed. We sought to explore the plasma profiles of n-3 and n-6 polyunsaturated fatty acids (PUFAs) in newly diagnosed untreated active CD. We included 26 consecutive CD pediatric patients (<19 years) and 14 healthy controls (HCs). Disease characteristics, including inflammatory markers, dietary histories, and the Pediatric Crohn’s Disease Activity Index (PCDAI), were obtained. The profiles of plasma FAs in plasma lipid classes were analysed by gas chromatography with FID detection of methyl esters. The erythrocyte sedimentation rate, C-reactive protein level and fecal calprotectin level (all p<0.001) were significantly higher in CD patients than in HCs. Most changes were observed in plasma phospholipids (PLs), such as a higher content of n-3 and changes in n-6 long-chain PUFAs in the CD group. The CD group had a lower ratio of n-6/n-3 PUFAs in PLs (p<0.001) and triacylglycerols (TAGs) (p<0.01). Correlations of the FA content in plasma PLs with disease activity scores of CD were also observed, which were positive for the sum of monounsaturated fatty acids (MUFAs) as well as oleic acid (18:1n-9) (both p<0.05). The fatty acid composition is associated with obesity. Omega 3 polyunsaturated fatty acid (PUFA) could have a beneficial role in the prevention and treatment of many disorders, including cardiometabolic diseases. A cohort of 84 men and 131 women were examined in adolescence and after 8 years. Body weight (BW) and fat mass (FM) were measured. The composition of fatty acids (FAs) of serum phospholipids was assessed using gas chromatography. Statistics: PLS method. Aim: to determine the relationships between FAs in adolescence and FM (explanatory variable 1, EV1) and BW (explanatory variable 2, EV2) in adulthood. In the predictive models, a cluster of FAs in boys explained 47.2 % of EV1 and a cluster of 6 FAs in girls explained 32.3 % of EV1 measured in adulthood. FAs measured in adolescents explained 23.7 % of EV2 in early adults regardless of gender. A significant negative association was found between 18:1n-9c and EV1 in males and EV2 in both genders. We found a significant negative association between 18:2n-6 and 20:0 and both EV1 and EV2. In all analyses, we found a significant negative association of 20:1n-9 and 18:3n-3 with EV1-2 in both genders. A significant positive association was found in 20:3n-6 with EV1 and EV2 in males. 20:4n-6 was positively associated with EV1 in females and EV2 in both genders. A positive association between FM and very long chain n- 6 PUFAs was also observed. It is concluded that serum MUFAs and essential PUFAs in adolescence are associated with lower BW and FM in adulthood.
Cíl práce: Lipoperoxidace je dávána do příčinné souvislosti s rozvojem nejdůležitějších rizikových faktorů kardiovaskulárních onemocnění. Lipoperoxidační procesy jsou důsledkem reakcí lipidů s volnými radikály bez ohledu na jejich původ. Vztah mezi triacylglyceroly (TAG) a volnými radikály (VR) není zcela jasný. Může se dokonce stát, že hypertriacylglycerolémie je doprovázena nízkými koncentracemi VR a naopak. Jedno z možných vysvětlení spočívá ve vztahu lipoperoxidace a složení mastných kyselin v molekulách TAG. Cílem této studie bylo analyzovat vztah složení mastných kyselin v TAG a produkci VR u pacientů trpících diabetes mellitus 2. typu. Typ studie: observační s kontrolní skupinou Materiál a metody: Studie zahrnovala 18 normotriacylglycerolemických (NTG) jedinců (M/F 12/6) a 24 hypertriacylglycerolemických (HTG) jedinců (M/F 16/8). Pacienti v obou skupinách byli diabetici 2. typu. Analýzu mastných kyselin v triacylglycerolech plazmy jsme provedli kapilární plynovou chromatografií a koncentrace volných radikálů byly změřeny přímou spektrofotometrií. Statistické analýzy (parametrické i neparametrické) byly provedeny pomocí statistického software STATISTICA pro Windows. Výsledky: Ve skupině HTG jsme pozorovali nižší zastoupení kyseliny myristové (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [medián (1.-3. kvartil]) a vyšší poměr alfa-linolenové kyseliny (18:3n-3) (1.51 [1.21-1.86] vs 1.16 [1.02-1.42], p = 0.021). Poměr kyselin arachidonové/eikosapentaenové byl ve skupině HTG nižší (5.54 [3.62-7.98] vs 7.25 [5.98-15.12], p = 0.027). Když jsme HTG skupinu dále rozdělili podle koncentrací VR na tercily, prokázali jsme nižší hodnoty poměru kyseliny arachidonová/eikosapentaenová ve druhém tercilu (4.05 [2.66-4.67] vs 7.64 [5.83-11.00], 1. tercil, a 6.21 [3.20-8.31], 3. tercil p = 0.019). V spojených HTG+NTG skupinách jsme zaznamenali negativní korelaci mezi MA a TAG (r = -0.3153, p = 0.045). V triaclylglycerolech jsme prokázali pozitivní korelace VR a kyseliny stearové (p < 0.001) spolu s negativní korelací některých n-6 vícenenasycených mastných kyselin (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Závěr: Zastoupení mastných kyselin v molekulách TAG je spojeno s tvorbou VR. Nicméně do tohoto vztahu vstupují i jiné faktory., Objective: The lipoperoxidation plays a causal role in the development of most important risk factors of cardiovascular disease. The lipoperoxidation results from the reaction of lipid molecules with free radicals, regardless of their origin. The relationship between triacylglycerols (TAG) and the free radicals (FR) amount is not clear. Thus, it can turn up that the hypertriacylglyceridaemia is accompanied with the low values of FR, or vice versa. One possible explanation of this phenomenon is that the degree of lipoperoxidation of TAG is dependent on the composition of fatty acids (FA) in their molecule. Therefore, the aim of this study was to analyze FA composition in TAG and its relationship to FR production in type 2 diabetes mellitus. Design: observational study with internal control group Material and Methods: 18 normotriacylglycerolemic (NTG) individuals (12M/6F) and 24 hypertriacylglycerolemic (HTG) patients (16M/8F). Patients in both groups were persons, suffering from type 2 diabetes mellitus. Fatty acid analysis of plasma triacylglycerols was performed by gas chromatography and free radicals were determined by direct spectrophotometric method. Statistical analyses (both parametric and non-parametric) were performed with the statistical software STATISTICA for Windows. Results: We found lower content of myristic acid (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [median (1st-3rd quartile)]) and higher content of alpha-linolenic acid (18:3n-3) (1.51 [1.21-1.86] vs. 1.16 [1.02-1.42], p = 0.021) in HTG group. The ratio of arachidonic/eicosapentaenoic acid was in HTG group lower (5.54 [3.62-7.98] vs. 7.25 [5.98-15.12], p = 0.027). When the HTG group was stratified into the tertiles by the FR concentrations, we observed lower values in the second tertile for ratios based on arachidonic acid (20:4n-6) and eicosapentaenoic acid (20:5n-3) (4.05 [2.66-4.67] vs. 7.64 [5.83-11.00], 1st tertile, and 6.21 [3.20-8.31], 3rd tertile, p = 0.019). In pooled groups, we proved negative correlation between MA and TAG (r = -0.3153, p = 0.045) and the link between FR and FA composition of TAG expressed as positive relationship of FR with content of stearic acid (p < 0.001) and negative correlation with some n-6 polyunsaturated fatty acids (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Conclusion: The composition of FA in TAG molecules has undoubtedly some relationship to the FR generation.
