Významný úspech liečby s inhibítormi tyrozínkinázy (TKI) u chorých s Philadelphia chromozóm (Ph) pozitívnou chronickou myelocytovou leukémiou (CML) dramaticky zmenil prognózu tohto ochorenia. Aj napriek tomu, že prvolíniová liečba CML použitím imatinibu znamená významný benefit pre väčšinu chorých, ukazuje sa, že 25–35 % pacientov potrebuje v priebehu 5 rokov zmenu liečby z dôvodu rezistencie alebo zlyhania liečby. Najčastejším mechanizmom zodpovedným za rezistenciu proti imatinibu u pacientov s CML je rozvoj mutácií v BCR-ABL kinázovej doméne. Tieto mutácie vyvolávajú rôznu mieru rezistencie proti imatinibu a zatiaľ čo niektoré z nich môžu reagovať na zvýšené dávky imatinibu, iné sú rezistentné proti nilotinibu a ďalšie vykazujú zvýšenú mieru rezistencie proti dasatinibu. Práca je prehľad liečebných možností v primárnej liečbe chronickej fázy CML a pri jej zlyhaní. Ukazuje sa, že pokroky v molekulových metódach umožňujú lepšie porozumieť podstatu choroby, zvážiť benefit a riziko terapie, individualizovať terapeutický prístup a prispôsobiť liečbu CML včas tak, aby sa minimalizovalo riziko progresie do akcelerovaných fáz., The phenomenal success of therapy with tyrosine kinase inhibitors (TKI) in Philadelphia chromosome (Ph) – positive chronic myeloid leukemia (CML) has dramatically changed the prognosis of this disease. Despite the fact that first-line therapy of CML using imatinib represents revolutionized the treatment of this disease, it became clear that during 5 years 25–35 % of the patients require change in the therapy due to the development of imatinib resistance or failure. The most frequent mechanism responsible for imatinib resistance is development of mutation in BCR-ABL kinase domain. Mutations cause different level of imatinib resistance and while some of them can be overcome by increased dose of imatinib, others seem to be resistant to nilotinib and others are more resistant to dasatinib. This review is focused on primary treatment and treatment after imatinib failure. Chronické myeloproliferativní neoplázie (MPN) představují různorodou skupinu myeloidních neoplázií, jejichž molekulární podstata je charakterizovaná přítomností mutace JAK2V617F. Po tomto zásadním objevu bylo zjištěno několik dalších mutací, což jen zdůraznilo nečekanou molekulární složitost. Ústředním rysem MPN je deregulace dráhy JAK/STAT a i když vzbudila velký zájem vzhledem k možnosti cílené léčby inhibitory JAK2, dospělo se k závěru, že ve většině případů, ne-li ve všech, jsou mutace JAK2 sekundární mutační událostí. Další opakující se otázkou je postižení genů ovlivňujících epigenetickou kontrolu genové exprese a nově také sestřih (splicing) RNA. Většinu těchto mutací mají rovněž pacienti s myelodysplastickými syndromy. Studie zaměřené na složitou klonální hierarchii MPN svědčí o stavu genetické nestability, který by mohl být buď získaný, nebo dědičný. V tomto ohledu nám objevení specifického zárodečného haplotypu u JAK2 poskytlo vysvětlení jevu „familiárního clusteringu” MPN, přestože se na něm pravděpodobně podílejí i jiné, dosud neznámé haplotypy. Cílem tohoto přehledu je shrnout současné poznatky o molekulárních abnormalitách MPN a probrat jejich úlohu v diagnostice a prognóze., Chronic myeloproliferative neoplasms (MPNs) represent a heterogeneous group of myeloid neoplasia whose molecular asset is marked by the presence of a JAK2V617F mutation. Following this seminal discovery, several other mutations have been identified highlighting an unexpected molecular complexity. Deregulation of the JAK/STAT pathway is a central feature of MPNs and, although it has raised much interest for the possibility of targeted therapy with JAK2 inhibitors, it has been realized that JAK2 mutations are secondary mutational events in most, if not all, cases. Another recurrent theme is the involvement of genes intervening in the epigenetic control of gene expression and, more recently, in RNA splicing. Most of these mutations are shared by patients with myelodysplastic syndromes as well. Studies focusing on the complex clonal hierarchy of MPNs suggest a condition of genetic instability, that could be either acquired or inherited. At this regard, the discovery of a specific germline haplotype in JAK2 provided an explanation for the phenomenon of familial clustering of MPNs, although other still unknown haplotypes are likely involved. Myeloproliferativní neoplázie (MPN) jsou klonální poruchy, které vznikají na úrovni hematopoetických kmenových buněk. Somatické mutace, jako např. JAK2V617F, lze nalézt u purifikovaných hematopoetických kmenových buněk a jejich potomstva. Nicméně je možné objevit výrazné interindividuální rozdíly v mutantní alelické zátěži u zralých krvinek pacienta a v počtu hematopoetických linií, které jsou zasažené. Ne všichni pacienti s MPN vykazují přítomnost klonálních markerů u B lymfocytů a T buňky jsou téměř vždy vyloučeny. U mnoha pacientů s MPN je možné objevit více než jednu somatickou mutaci a pořadí, v němž se tyto mutace objevují, se může u jednotlivých pacientů lišit. Myší modely ukázaly, že MPN může být iniciována JAK2V617F bez přítomnosti dalších mutací. Všechny dosavadní myší modely jsou však založeny na polyklonálním onemocnění. Za účelem navržení lepších léčebných postupů a potažmo i vyléčení MPN bude důležité pochopit časné kroky při iniciaci choroby., Myeloproliferative neoplasms (MPN) are clonal disorders that originate at the level of hematopoietic stem cells. Nevertheless, large inter-individual differences in the mutant allele burden in the patient’s mature blood cells and in the number of hematopoietic lineages that are involved can be found. Not all patients with MPN show presence of clonality markers in B lymphocytes and T cells are almost always excluded. In many MPN patients more than one somatic mutation can be found and the order in which these mutations occur can vary between individual patients. Mouse models have shown that MPN can be initiated by JAK2V617F without the presence of additional mutations. However, all mouse models to date are based on polyclonal disease.
