Léčba kolorektálního karcinomu (KRK) vyžaduje multidisciplinární přístup s účastí gastroenterologa, chirurga, patologa, onkologa a radiologa. Možnosti léčby jsou závislé na stadiu nemoci, celkovém stavu pacienta a také molekulární charakteristice nádoru. V rámci předoperační péče se léčebné postupy u karcinomu konečníku mírně liší od léčby karcinomu tlustého střeva. V případě metastatického KRK jsou možnosti léčby jednotné, použití kombinace chirurgické a cílené systémové léčby. Každá léčebná modalita KRK je zatížená rizikem komplikací. Proto je nezbytné s každým pacientem probrat prospěch a rizika navrhované léčby. Prezentujeme dvě kazuistiky pacientů s KRK, u kterých léčba vedla k dosažení remise i za cenu významných komplikací., Treatment of colorectal cancer (CRC) requires a multidisciplinary approach involving a gastroenterologist, surgeon, pathologist, oncologist and a radiologist. Treatment options depend on the stage of the disease, the overall condition of the patient and the molecular characteristics of the tumor. In the context of preoperative care, treatment procedures for cancer of the rectum is slightly different from the treatment of the colon cancer. In the case of metastatic CRC, there are unified treatment options, using a combination of surgical and targeted systemic therapy. Each treatment modality is at risk of complications. Therefore it is necessary to discuss the benefits and risks of the proposed treatment with each patient. The article analyses a surgical treatment of 506 patients taking inpatient treatment in the Center for colon proctology. All the patients underwent a comprehensive examination including radiological and tool test methods in addition to conventional clinical and laboratory ones. On the basis of the complex study, indications, volume and terms of surgery performance were determined. Performance of preand postoperative management of patients is substantiated in details. Postoperatively, 28 (5.5%) had postoperative wound pyesis in the region of the previous colostomy. In the postoperative period 2 (0.4%) deaths of patients were caused with peritonitis which occurred due to incompetence of anastomosis sutures. Rehabilitation of stomed patients is one of challenges in colorectal surgery. Positive results treatment can be obtained only at carrying out of a comprehensive outpatient and inpatient preoperative preparation including recommendations in diet, colostome care, hydrogymnastics of the disconnected part of the intestine and prevention of infectious complications., Isakulov T. Autori prezentujú kazuistiku zriedkavej náhlej brušnej príhody. Ide o 39-ročnú polymorbídnu pacientku so systémovým lupus erythematosus, chronickou renálnou insufficienciou na podklade lupusovej nefritídy, v hemodialyzačnom programe, po transplantácii ľavej obličky v minulosti, prijatej na Traumatologickú kliniku za účelom totálnej endoprotézy (TEP) ľavej koxy pre bionekrózu hlavice femuru. V pooperačnom priebehu bol stav komplikovaný gangrénou kolonu, CT i peroperačne verifikovanou. Pacientka bola operovaná, bola vykonaná subtotálna kolektómia, terminálna ileostómia a ľavostranná ovarektómia. Pooperačný priebeh bol komplikovaný perforáciou jejuna, bola vykonaná sutura jejuna. Pacientka bola hospitalizovaná na OAIM, následne na Chirurgickej klinike, bola plne realimentovaná. Na Internej klinike bola doriešená pre rozvrat vnútorného prostredia. Po 60 dňoch bola prepustená z nemocnice v klinicky dobrom stave s obnovenou pasážou, plne per os alimentovaná, mobilná., The authors present a case report of a 39-year-old woman with acute abdomen – a comorbid patient with systemic lupus erythematosus, chronic renal insufficiency as a complication of lupus nephritis, included in a haemodialysis programme. The patient had also undergone transplantation of the left kidney in the past. She was initially admitted to the Department of Traumatology for a total endoprosthesis procedure due to bionecrosis of the head of the thigh bone. Postoperatively, the patient’s condition was complicated by gangrene of the colon confirmed by CT scan and during the operation. The patient was operated on – subtotal colectomy, terminal ileostomy and left-sided ovariectomy was performed. The postoperative course was complicated by perforation of the jejunum which was sutured. The patient was admitted to ICU and, after recovery, to our surgical department. Because of the metabolic disturbance she was treated in the internal medicine department. After 60 days she was discharged in a good condition, walking and with full per os realimentation., and S.
