Po téměř deseti letech od zahájení screeningu kolorektálního karcinomu (KRCA) v České republice došlo ke změně jeho programu. Hlavní epidemiologické ukazatele KRCA, incidence a mortalita, pro nás zůstávají nepříznivé. Sekundární prevence KRCA zatím nenaplňuje očekávání. Úprava programu spočívá ve volbě vlastní screeningové metody a modifikaci jejího intervalu. Zvýšení adherence populace ke screeningu se očekává od nové alternativy, primární screeningové kolonoskopie. Kvalitativní změnu přináší imunochemický test na okultní krvácení, který se jak u praktických lékařů a gynekologů, tak u pacientů těší stále větší oblibě. Prohlubuje se spolupráce mezi jednotlivými odbornostmi participujícími na screeningu. Prostřednictvím médií i jiných marketingových akcí se zvyšuje informovanost české populace. Data o výkonech poskytovatelů za rok 2009 naznačí, zda screening v nové podobě je krokem správným směrem. Efekt screeningu ovšem budeme moci zhodnotit až s odstupem několika let., Almost ten years after the introduction of colorectal cancer (CRC) screening in the Czech Republic, there has been a change in the programme. The major epidemiological indicators of CRC, i. e. incidence and mortality, still remain unfavourable. Secondary prevention of CRC has not fulfilled the expectations so far. The change of the programme consists in choosing an individual screening method and modifying its interval. Primary screening colonoscopy, which is a new alternative, is expected to increase the adherence of the population to the screening. A qualitative change has been brought by the use of an immunochemical faecal occult blood test that is becoming increasingly popular with general practitioners and gynaecologists as well as patients. The cooperation among the individual disciplines involved in the screening programme has become more in-depth. Through the media and other marketing strategies, the awareness of the Czech population has been rising. The 2009 data on the procedures done by providers will suggest whether this new form of screening is a step in the right direction. Semantic changes observed in aphasic patients are examined in two aspects: as transfers of names and transfers of meaning. The most frequent in the material under scrutiny are semantic changes resulting form the contiguity of meanings (metonymies) and shifts motivated by the similarity of names (paronymies). Changes based on the similarity of meanings (metaphors) and the contiguity of names (ellipses) are less frequent. The relative frequencies of the mechanisms underlying semantic change in aphasia lead one to reflect on the nature of cognitive functions in aphasic patients. That the configurations of form and content, obtained by analyzing aphasic texts, are stored in people´s cognitive structures and as such constitute templates or patterns necessary for the comprehension and description of reality, as well as for the segmentation of interpreted sequences of information. The patterns recognized in the present study pertain primarily to the visible, physical world, judgments of non-material mental images are less frequent.
