Hlavními sledovanými ukazateli kvality po resekci rekta pro karcinom jsou operační radikalita, pooperační komplikace, počet recidiv a přežití. Poruchy funkce rekta, které postihují 25–50 % nemocných, zůstávají v pozadí. Tyto poruchy zahrnují vysokou frekvenci stolice, urgenci, evakuační poruchy, inkontinenci a sexuální a urologické dysfunkce. Funkční poruchy po resekčních výkonech na rektu jsou v anglofonní literatuře označovány akronymem „LARS“ (low anterior resection syndrome). V článku jsou v přehledu rozebrány změny anorektální fyziologie po nízké resekci rekta, etiopatogeneze a rizikové faktory LARS, způsoby konstrukce neorektálního rezervoáru, možnosti prevence a terapie LARS. Samostatně jsou v přehledu rozebrány sexuální a močové dysfunkce., The most important quality parameters of rectal cancer surgery are oncological radicality, postoperative complications, recurrence rate and survival. Rectal dysfunction following low anterior resection occurs in up to 25–50% of patients. Despite its high frequency, however, it does not receive enough attention. Rectal dysfunction after surgery includes frequent bowel movements, urgency, incomplete evacuation, incontinence or sexual and urinary dysfunctions. The complex of symptoms is collectively referred to as the low anterior resection syndrome – LARS. In this review, we discuss the alterations in anorectal physiology after low anterior resection, the etiology and risk factors of LARS, different types of neorectal reservoir construction, and various options for prevention and treatment of LARS. Úvod: Cílem článku je ukázat experimentální a klinické výsledky odpovědi ledvin na teplou a studenou ischemii. Porovnává různé operační postupy a možnosti ovlivnění ledvinné ischemie při resekcích ledvin. Článek je souhrnem aktuálních literárních údajů. Výsledky: Existují tři hlavní mechanizmy ischemického poškození ledvin – cévní mechanizmus s přetrvávající vazokonstrikcí a abnormální reakcí endoteliálních buněk, tubulární obstrukce se zpětným tokem moči a reperfuzní poškození. Spor trvá o maximální toleranci teplé ischemie (WI), která může být ovlivněna hlavně chirurgickou technikou. Závěr: Pokud je předpokládáno při operaci využití ischemie, nádor by měl být odstraněn v co nejkratší možné době. Obecně je doporučován čas kratší než 25 minut teplé ischemie bez ohledu na typ chirurgického přístupu. Pokud je možnost provedení resekce v tomto čase ohrožena, je třeba začít včas s chlazením ledviny. Studená ischemie (cold ischemia – CI) může být dle závislosti na způsobu chlazení tolerována až do doby dvou hodin (autotransplantace). K možným druhům chlazení při resekčních výkonech patří metoda in situ studené arteriální perfuze, povrchové chlazení ledovou tříští, retrográdní perfuze dutého systému nebo ex situ studená arteriální perfuze s autotransplantací. Technika metod dosažení CI závisí na předoperačním hodnocení nálezu, chirurgické technice (otevřená, laparoskopická nebo robotická operace) a zvyklostech pracoviště., Introduction: The objective of this literature review was to summarize clinical and experimental evidence of the renal responses to warm and cold ischemia. The study compared different surgical procedures and options for dealing with renal ischemia during partial nephrectomy. The article is a summary of the current literature data. Results: There are three main mechanisms of ischemic renal injury – vascular, persistent vasoconstriction with an abnormal endothelial cell compensatory response, and tubular obstruction, with backflow of urine, and injury due to reperfusion. Controversy regarding the maximal kidney tolerability to warm ischemia continues. This communication summarizes literary data regarding available surgical techniques used to diminish the effects of warm ischemia. Conclusion: If ischemia is required, the tumour should be removed within 25 minutes of warm ischemia, regardless of the surgical approach. If this longer time of resection is expected, we have to start immediately with cold ischemia. Cold ischemia, depending on the cooling method, can be tolerated for up to 2 hours (autotransplantation). The cold ischemia technique includes in situ cold arterial perfusion, ice slush placed around the kidney, retrograde caliceal perfusion or ex situ cold arterial perfusion with autotransplantation. Úvod, cíle: Poruchy ejakulace jsou po míšním poranění velmi časté. U nemocných s přáním vlastního potomka je většinou nutný asistovaný odběr spermií. Penilní vibrostimulace (PVS) je neinvazivní metoda k získání ejakulátu u pacientů s lézí nad úrovní T10. Cílem práce je zhodnocení efektivity a bezpečnosti PVS, vyhodnocení kvality a dalšího využití ejakulátu. Metodika: Od 2010 do 2014 jsme provedli PVS u 20 pacientů. Poranění krční (C) míchy bylo u 13 pacientů, hrudní (T) míchy u sedmi pacientů. Průměrný věk byl 30,8 let. Průměrná doba od úrazu do PVS byla 64 měsíců. První PVS mělo sedm nemocných déle než 3,5 roku a 13 do 3,5 roku od úrazu. K PVS jsme užili přístroj FERTI CARE Multicept. Výsledky: Ejakulace jsme dosáhli u 11 pacientů (55 %), z toho devět mělo poranění C míchy a dva T míchy. U nemocných se začátkem PVS do 3,5 roku od poranění byla úspěšnost 77 % a nad 3,5 roku 14 %, p = 0,027. Při srovnání ejakulátu z druhé PVS došlo ke zvýšení celkového počtu spermií (p = 0,03), koncentrace spermií (p = 0,025) a počtu progresivně pohyblivých spermií (p = 0,027) oproti první PVS. Získaný ejakulát je v 72 % pouze kryoprezervován k pozdějšímu využití. U dvou pacientů byl ejakulát použit k fertilizaci. Jeden nemocný měl azoospermii. Autonomní dysreflexie se vyskytla v průběhu PVS u sedmi pacientů, příznaky odezněly do 3 min. Závěr: PVS je bezpečná metoda asistovaného odběru ejakulátu. Při opakování PVS jsme zjistili zlepšení kvality ejakulátu. U dvou pacientů byl ejakulát úspěšně použit k fertilizaci., Objectives: Following spinal cord injury (SCI), ejaculatory dysfunctions are frequent. In men who wish to father children, assisted sperm retrieval is often necessary. Penile vibratory stimulation (PVS) is a non-invasive method for sperm retrieval in patients with a lesion above T10. The aim of our research was to evaluate the effectiveness and safety of PVS, semen quality and further utilization of the ejaculate. Methods: Between 2010 and 2014, we performed PVS in 20 patients. An injury of the cervical (C) and thoracic (T) spinal cord was present in 13 and seven patients, respectively. The mean age of patients was 30.8 years. The mean time from the injury to PVS was 64 months. Seven patients had the first PVS more than 3.5 years after the injury and 13 patients less than 3.5 years. FERTI CARE Multicept was used for PVS. Results: Ejaculation was achieved in 11 patients (55%) – in nine with C and in two with T SCI. Success rate of PVS in patients less than 3.5 years after the injury was 77%, compared to 14% in patients more than 3.5 years after the injury, p = 0.027. Total sperm count (p = 0.03), sperm concentration (p = 0.025) and the number of sperm with progressive motility (p = 0.027) increased in the semen from the second PVS. Obtained ejaculate is mostly (72%) cryopreserved only for further use. The ejaculate was used for fertilisation in two patients. One patient had azoospermia. Autonomic dysreflexia during PVS occurred in seven patients, symptoms resolved within 3 mins. Conclusion: PVS is a safe method for sperm retrieval. Semen quality improved with repeated PVS. The ejaculate was successfully used for fertilisation in two patients.
