Následně jsou detailně probírány vlastnosti jednotlivých představitelů inhibitorů izoenzymu fosfodiesterázy 5 – sildenafilu, vardenafilu, tadalafilu a avanafilu z pohledu účinnosti a bezpečnosti i z pohledu lékových interakcí. Poslední část shrnuje naše znalosti o vlivu řady kardiovaskulárních léků na vlastní erektilní funkci., This review article summarizes current findings regarding the mechanism of induction of erection and contemporary possibilities of pharmacotherapy for erectile dysfunction. The next part summarizes the characteristics of phosphodiesterase enzymes and the significance of their inhibition in general, with special emphasis on phosphodiesterase type 5 (PDE-5) isoenzyme. After this, the characteristics of the individual representatives of PDE-5 inhibitors – sildenafil, vardenafil, tadalafil and avanafil – are discussed from the point of view of effectiveness, safety and drug interactions. V populaci starších osob je běžný úbytek funkčních kapacit, postupný vznik různých nemocí až k širší multimorbiditě a nárůstu problémů v sociální oblasti, které mohou akcentovat stařeckou křehkost a neschopnost žít samostatně. Článek rozebírá komplexně problematiku geriatrické multimorbidity a klade důraz na její úskalí a rizika, potřebu interdisciplinárního přístupu a myšlení lékařů, a to včetně úskalí moderní farmakoterapie. Cílem geriatrické medicíny je optimalizace reziduálních funkcí navzdory poklesu celkových funkčních kapacit organizmu při rostoucí multimorbiditě. Autoři chtějí tímto přehledovým článkem především upozornit i odbornou veřejnost na neustále rostoucí počet seniorů v populaci a na potřebu povědomí umět správně interpretovat a chápat tuto zcela novou realitu., In the elderly population there are common permanent decrease of functional capacity, gradual emergence of various diseases leading to the wider multimorbidity and increased problems in the social sphere, which can develop frailty and social dependency. Ukazuje na nebezpečí, kterými hypoglykemie pacienty ohrožují, ale všímá si také rizikových skupin pacientů z hlediska rizika hypoglykemie a také různé míry rizika, která s sebou nesou různá antidiabetika. Moderní perkutánní nefrolitotomie (percutaneous nephrolithotomy – PCNL) je bezpečná technika spojená s nízkou morbiditou. PCNL umožňuje odstranit většinu konkrementů a představuje preferovanou modalitu při léčbě větších a odlitkových močových kamenů. Pro dosažení bezpečného a úspěšného výsledku musí být operatér nezbytně obeznámen s důležitými aspekty variability anatomie, ostatními komorbiditami pacienta a potenciálními komplikacemi, které hrozí při tomto výkonu. Ačkoli pro vytvoření přístupové cesty během PCNL, odstranění konkrementu a uzavření přístupu existuje celá řada nejrůznějších technik, pro dosažení kýženého výsledku je nutné, aby operatér disponoval znalostmi všech v současné době dostupných technik., Modern-day PCNL is a safe procedure with low morbidity. Kalcifikující uremická arteriolopatie, neboli kalcifylaxe, je raritní onemocnění vyznačující se systémovou kalcifikací médie arteriol vedoucí k ischemii a podkožním nekrózám. Nejčastěji se vyskytuje u pacientů s konečným selháním ledvin v chronickém hemodialyzačním programu (incidence se udává 1–4 %) a po transplantaci ledviny. Kalcifylaxe je zrádná svou rychlou progresí v nekrózu tkáně, obtížným hojením a vysokým rizikem sekundární infekce, která je nejčastější příčinou úmrtí při tomto onemocnění. Úmrtnost se udává v rozmezí 60–80 % v průběhu několika měsíců od stanovení diagnózy. Základem léčebného úspěchu je včasné stanovení diagnózy a správně a trpělivě vedená léčba. Na příkladu 3 pacientů zařazených v našem chronickém hemodialyzačním programu demonstrujeme typické klinické projevy a léčbu kožních defektů při kalcifylaxi, jež vyžaduje těsnou spolupráci pacienta a ošetřujícího personálu. Základem léčby u všech našich pacientů byla korekce kalciofosfátového metabolizmu a sekundární hyperparatyreózy. Na konci dialýz byl všem aplikován tiosulfát sodný. Další důležitou součástí byla péče o ránu, citlivý debridement a intenzivní lokální péče. Po 5–6 měsících došlo k úplnému zhojení kožních defektů u první pacientky, výraznému zlepšení v případě druhého pacienta a progresi u třetí pacientky. Nebyly pozorovány žádné nežádoucí účinky aplikace tiosulfátu sodného., Calcific uremic arteriolopathy or calciphylaxis is a rare disorder characterized by systemic medial calcification of arterioles that leads to ischemia and subcutaneous necrosis. It most commonly occurs in patients with end-stage renal disease who are on haemodialysis or who have received a renal transplant. Calciphylaxis is dangerous by its fast progression into tissue necrosis, difficult healing process and a great risk of secondary infection which is the most common cause of death in this condition. The reported mortality rates are as high as 60–80 % in a couple of months once it is diagnosed. The key to successful treatment of calciphylaxis is fast diagnosing of the disease and appropriate treatment management. On the examples of three patients from our haemodialysis centre we demonstrate typical clinical manifestation of calciphylaxis and its treatment, which requires close patient-medical staff cooperation. The basic principle of treatment of all our patients was normalization of calcium-phosphate metabolism and secondary hyperparathyroidism. Sodium thiosulfate had been administered to all patients at the end of haemodialysis session. The wound care played another major role with gentle debridement and intensive local care. After five to six months the skin defects resolved in the first patient, partially resolved in the second patient and deteriorated in the third patient. Nádory anu (análního kanálu) jsou méně častá nádorová onemocnění zažívacího traktu specifikované především histologickým obrazem spinocelulárního karcinomu. V jejich etiopatogenezi se až v 90 % uplatňuje infekce HPV-16 a imunosuprese. Proto je jejich výskyt vyšší u HIV pozitivní populace. Souvislost s HPV dává prostor pro primární prevenci vakcinací a současně pro cílenou biologickou terapii (kupř. nivolumab). Současné možnosti účinné léčby spočívají v první řadě v chemoradioterapii. Od 80. let poskytlo několik významných studií základ pro současné léčebné standardy: radioterapie primárního nádoru a spádových lymfatických oblastí do celkové dávky ekv. 54–60 Gy, konkomitantní chemoterapie režimem mitomycin + 5-fluorouracil nebo cisplatina + 5-fluorouracil. Dosahuje se tak kompletní regrese nálezu v 90 %, pětileté přežívání do 70 % a pětileté přežívání bez relapsu do 70 %. Indukční chemoterapie ani chemoterapie po radioterapii nepřináší benefit. Chirurgický resekční výkon je indikován jako záchranná léčba při relapsu nebo pokud není primární léčbou dosažena kompletní regrese. Zásadní problém léčby nádorů anu je výrazná akutní i chronická toxicita chemoradioterapeutických režimů, jíž se daří částečně omezovat technicky dokonalejšími metodikami ozařování. Cílená biologická terapie včetně protinádorových vakcín je zatím ve fázi preklinického nebo počátečního klinického vývoje., Cancer of the anus (anal canal) is a less frequent malignant disease of gastrointestinal tract, specified by micromorphology, squamous cell carcinoma. In about 90 % HPV-16 infection and immunosuppression are involved in cancerogenesis. Therefore the incidence is higher in HIV positive population. The relationship to HPV allows primary prevention through vaccination as well as targeted biologic therapy (e.g. nivolumab). Current treatment options consist predominantly in chemoradiotherapy. Standard treatment is based on several fundamental studies conducted after 1980. It consists in frontline chemoradiotherapy of primary tumor and regional lymph nodes up to a dose 54–60 Gy eq, concomitant chemoradiotherapy mitomycin + 5-fluorouracil or cisplatinum + 5-fluorouracil. Complete regression is achieved in 90%, 5 year survival up to 70 % and 5 year relapse free survival up to 70%. There is no benefit of either induction chemotherapy or chemotherapy after radiotherapy. Surgery is indicated as a salvage therapy for local relapse or if a complete regression is not achieved. Acute and chronic toxicity of chemoradiotherapy regimens presents a substantial problem of chemoradiotherapy. It may be partially reduced by more advanced radiotherapy technologies. Úvod: Kardiovaskulární (KV) onemocnění patří celosvětově k hlavním příčinám úmrtí. Časné odhalení rizikových faktorů a zvýšeného celkového KV rizika může být významným pomocníkem při snižování morbidity i mortality. Za optimální věk pro hodnocení KV rizika v populaci se doporučuje věk 40 let u mužů a 50 let u žen. V rámci prezentovaného průřezového šetření byl zhodnocen KV rizikový profil osob v těchto věkových kategoriích při vyšetření všeobecným praktickým lékařem. Metodika: Do průzkumu bylo zařazeno 1 812 osob, z toho mužů v průměrném věku 40 a žen v průměrném věku 50 let. U každého vyšetřeného byla zjištěna a do protokolu studie zaznamenána rodinná, osobní a farmakologická anamnéza, abúzus, fyzická aktivita. Byly změřeny základní antropometrické údaje, tepová frekvence, krevní tlak, zjištěny hodnoty sérového lipidogramu a glykemie. Vyšetřující lékaři zaznamenávali přítomnost nově zjištěných rizik KV onemocnění a stanovili celkové KV riziko dle tabulek SCORE. Bylo hodnoceno procento nemocných s již existujícím sledovaným onemocněním, procento zastoupení těch, kteří měli dobře kontrolované rizikové faktory, a dále procento pacientů, kterým byl nově zjištěn některý ze sledovaných rizikových faktorů. Hypotéza vztahu mezi pozitivní rodinnou anamnézou a výskytem onemocnění byla testována za použití Pearsonova testu. Výsledky: Bylo zařazeno 961 mužů průměrného věku 42,9 ? 4,7 let a 851 žen průměrného věku 51,2 ? 3,6 let. V pásmu nadváhy se nacházelo 49 % mužů a 31 % žen, obézních bylo 32 % mužů a 31 % žen. Aktivně kouřilo 36 % mužů a 22 % žen. Prevalence diabetu 2. typu byla shodně 11 % u obou pohlaví. Arteriální hypertenze byla zjištěna u 43 % mužů a 45 % žen a dyslipidemie u 39 % mužů a 41 % žen. Farmakologicky léčeno pro kteroukoli z uvedených chorob bylo 48 % osob, z nich pouze 8 % dosahovalo dobré kontroly všech identifikovaných rizik. Nově byl zjištěn diabetes u 3 % mužů i žen, arteriální hypertenze u 7 % mužů a 5 % žen a dyslipidemie u 20 % mužů i žen. 62 % mužů a 65 % žen doporučili participující lékaři nefarmakologickou léčbu. U 53 % mužů a 51 % žen byla zahájena nová farmakoterapie. U mužů i u žen šlo nejčastěji o antihypertenzní medikaci ze skupiny ACE-inhibitorů (29 % u mužů a 24 % u žen) a hypolipidemikum ze skupiny statinů (29 % u mužů a 27 % u žen). Z analýzy vztahu mezi pozitivní rodinnou anamnézou a výskytem onemocnění u dané osoby bylo zjištěno, že tato souvislost existuje pro arteriální hypertenzi, diabetes mellitus 2. typu i dyslipidemii. Závěr: Provedené šetření dokumentuje vysokou prevalenci rizikových faktorů kardiovaskulárních onemocnění v populaci 40letých mužů a 50letých žen. Vysoké zastoupení nově zjištěných rizikových faktorů i nadpoloviční podíl osob, u nichž byla v rámci provedeného šetření zahájena farmakoterapie, jasně dokazují nezbytnost časného screeningu rizikových faktorů. Genetická determinace jednotlivých kardiovaskulárních rizikových faktorů odrážející se v rodinné anamnéze je významnou komponentou celkového rizika oběhových onemocnění, a musí být aktivně zjišťována a zohledňována při úvaze o volbě intenzity preventivních strategií., Background: Cardiovascular (CV) disease belongs to the most important mortality causes worldwide. Early identification of risk factors and increased CV risk may help decrease of morbidity and mortality. An optimal age for CV risk factors screening ages of 40 in males and of 50 in females have been identified. CV risk profile of persons in these age categories has been examined by general practitioners and recorded for the purposes of this cross-sectional survey. Methods: 1812 persons, males at average age of 40 years and females at average age of 50 years, were included into the survey. In each of the examined family and personal history including pharmacological were recorded into the study protocol as well as abusus and physical activity patterns. Basic anthropometrical parameters, e.g. heart rate, blood pressure, body mass index and laboratory measures including blood lipids and glycaemia. The participating physicians recorded newly identified risk factors CV disease and determined the global CVD risk according to the SCORE charts. Percentage of those already treated for any of the main CVD risk factors and, among these, also attaining the treatment goals. The percentage of patients with a newly identified CV risk factor was calculated. We also tested the hypothesis of a relationship between positive family history of any of the followed risk factors and its risk in the examined probands using Pearson´s test. Results: 961 males, average age of 42.9 ? 4.7 years, and 851 females , average age of 51.2 ? 3.6 years, were enrolled into the study. 49% of males and 31% of females were overweight and 32% men and 31% of women were obese. There were 36% of smokers among men and 22% among women. The prevalence of type 2 diabetes was 11% in males and also in females. Arterial hypertension was diagnosed in 43% of males and 45% of females while dyslipidemia was present in 39% of males and 41% of females. Pharmacological treatment of any of the above mentioned diseases was used in 48% of the probands, however, only 7% of them attained treatment goals of blood pressure, LDL-cholesterol and glycaemia. Type 2 diabetes was newly identified in 3% of both males and females, arterial hypertension in 8% of males and 5% of females and dyslipidemia was newly detected in 20% of probands of both genders. Non-pharmacological treatment was recommended to 62% of male and to 65% of female participants, respectively. Pharmacological treatment was initiated in 53% of males and 51% of females. In both genders this was mostly antihypertensive treatment with ACE inhibitors (29% of males and 24% of females) and lipid lowering therapy with a statin (29% of males, 27% females). The analysis of relationship between the positive family history of any of the followed diseases and their presence in the probands examined revealed significant increases of the risk for arterial hypertension, type 2 diabetes mellitus and dyslipidemia. Conclusion: The survey following CV risk profile in a cohort of 40 years old men and 50 years old women showed high prevalence of CV risk factors in these age categories in the Czech probands. The high frequency of modifiable risk factors and the need to initiate pharmacological treatment in more than one half of the examined population documents the need of early detection of risk.
