BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. Cíl: Posoudit možnosti diagnostiky cho-lesteatomu středouší pomocí magnetické rezonance (MR) za použití non-echo-planární difúzně-vážené (non-EPI DWI) sekvence. Metodika: Prospektivní studie zahrnovala všechny pacienty, kteří byli indikování k primární nebo revizní operaci cholesteatomu středouší v období mezi 10/2012 až 1/2015. Všichni tito pacienti před operací podstoupili vyšetření MR za použití non-EPI DWI techniky. Šlo o 25 pacientů, u dvou s oboustranným postižením. Deset bylo před primární operací, 17 před revizním výkonem. MR vyšetření bylo zhotoveno v rozmezí 0-106 dnů (medián 2 dny) před operací. Výsledky MR vyšetření byly srovnány s operačním a definitivním histologickým nálezem. Výsledky: MR vyšetření s non-EPI DWI sekvencí mělo pro diagnostiku cholesteatomu senzitivitu 93,75 %, specificitu 93,75 %, pozitivní prediktivní hodnotu 93,75 % a negativní prediktivní hodnotu 93,75 %. Nejmenší správně diagnostikovaný cholesteatom měl velikost 4 mm, největší 20 mm, průměr 11 mm. Závěr: Vyšetření MR s použitím non-EPI DWI sekvence je spolehlivou metodou k zobrazení cholesteatomu středouší. Lze ji využít jak u pacientů před primárním operačním výkonem, tak u pacientů před second-look či revizní operací., Aim: To evaluate the possibility of diagnosis of middle ear cholesteatoma on MRI using non-echo-planar diffusion-weighted imaging (non-EPI DWI). Method: The prospective study included all patients indicated to primary or second-look surgery of middle ear cholesteatoma between 10/2012 and 1/2015. All these patients underwent MRI using non-EPI DWI technique before the surgery. There were 25 patients, two patients had bilateral disease. 10 patients were before primary surgery, 15 patients before second-look surgery. The MRI was performed from 0 to 106 days (median 2 days) before surgery. The results of the MR investigation were compared with those obtained by surgery and confirmed by definitive histopatological evaluations. Results: The sensitivity of 93,75%, specificity of 93,75 %, positive predictive value 93,75 %, negative predictive value 93,75% was reached in detection of the middle ear cholesteatoma using MR with non-EPI DWI. The smallest correctly diagnosed cholesteatoma had the diameter of 4mm, the largest 20 mm, mean 11 mm. Conclusion: MR investigation using non-EPI DWI sequence is reliable method for the middle ear cholesteatoma diagnosis. Objective: The study aimed to evaluate plasma adiponectin (ADP), adipocyte fatty acid binding protein (A-FABP) and fibroblast growth factor 21 (FGF 21) levels as potential predictors of severity of acute pancreatitis (AP) in day 4 after admission. Simultaneously, the classical proinflammatory makers were analysed as well. Settings: Department of Clinical Biochemistry and 2nd Department of Internal Medicine, Faculty Hospital Olomouc, I. P. Study design: The study was conducted in subjects with acute pancreatitis (n = 84, 37 females, 47 males). The analyses were performed in the groups according to the mild/severe classification of AP, and partly in the computed-tomography severity index (CTSI) score subgroups. Results: From adipokines, only FGF 21 tended to be higher in the severe AP subgroup in day 4. The multiple regression analysis revealed a positive association of A-FABP with procalcitonin (PCT), whilst FGF 21 was associated positively with ADP and negatively with C-reactive protein (CRP) in patients with the severe AP. The receiver-operator characteristics (ROC) analysis confirmed that not adipokines, but only CRP and interleukin 6 (IL-6) were suitable as potential predictors of the disease severity. The cut-off values were established for both parameters: 100 mg/L for CRP and 37 ng/L for IL-6, with negative predictive values (NPV) 96 % and 92 %, and positive predictive values (PPV) 39 % and 29 %, respectively. Conclusion: The role of ADP, A-FABP and FGF 21 has been limited in a prediction of the disease severity in day 4 after admission, while CRP and IL-6 might be useful to exclude a severe AP., Cíle studie: Studie byla zaměřena na vyhodnocení sérových hladin adiponektinu (ADP), adipocytárního proteinu vázajícího mastné kyseliny (A-FABP) a fibroblastového růstového faktoru 21 jako potenciálních prediktorů závažnosti akutní pankreatitidy (AP) 4. den po přijetí. Laboratorní markery byly měřeny v podskupinách podle klasifikace lehká/ těžká AP, a částečně také v podskupinách vytvořených na základě CTSI (computed-tomography severity index) skóre. Výsledky: Ze sledovaných adipokinů měl u pacientů s těžkou AP pouze FGF 21 tendenci k vyšším hladinám v den 4 po přijetí. Za pomoci mnohočetné regresní analýzy byla zjištěna pozitivní asociace A-FABP s prokalcitoninem (PCT), zatímco FGF 21 asocioval u těžké AP pozitivně s ADP a negativně s C-reaktivním proteinem (CRP). ROC (receiver-operator characteristics) analýza potvrdila, že pouze CRP a interleukin 6 (IL-6) jsou využitelné jako potenciální prediktory závažnosti onemocnění. Pro oba parametry byly stanoveny cut-off hodnoty: 100 mg/l pro CRP a 37 ng/l pro IL-6, s negativními prediktivními hodnotami (NPV) 96 % a 92 %, a s pozitivními prediktivními hodnotami (PPV) 39 % a 29 %. Závěr: Ve studii jsme zjistili, že hladiny ADP, A-FABP a FGF 21 nejsou vhodnými parametry pro predikci tíže AP 4. den po přijetí. Úvod: Komplexní protokol kvantitativního testování senzitivity (QST), zahrnující hodnocení široké škály modalit termické, algické, taktilní a vibrační cítivosti, je často využívaným nástrojem pro kvantifikaci senzitivity a hodnocení tzv. senzitivního profilu, zejména u pacientů s neuropatickou bolestí. Protokol obsahuje složité slovní instrukce, které je pro dosažení standardizovaného výstupu nutné přesně následovat, což implikuje nutnost validace jeho jazykových mutací. Soubor a metodika: Pomocí české verze standardizovaných slovních instrukcí ke komplexnímu QST protokolu bylo vyšetřeno 75 zdravých dobrovolníků, 68 pacientů s periferní neuropatickou bolestí při diabetické neuropatii a 59 jedinců s centrální neuropatickou bolestí při roztroušené skleróze, a to vždy na HKK i DKK. Pro hodnocení spolehlivosti bylo u 26 jedinců testování provedeno opakovaně (4×) s přibližně 30denními rozestupy mezi jednotlivými vyšetřeními. Výsledky: Vyšetření pomocí české verze komplexního QST protokolu bylo pro pacienty nenáročné či málo náročné. V souboru zdravých dobrovolníků spadalo 94 % zjištěných hodnot jednotlivých senzitivních parametrů na HKK a 95 % na DKK do rozsahu fyziologických nálezů podle publikovaných normativních dat. Alespoň jedna abnormita z hodnocených senzitivních modalit byla zachycena u 90 % pacientů s neuropatickou bolestí. Shluková analýza umožnila identifikaci dvou symptomových profilů, z nichž jeden většinově odpovídal bolestivé diabetické neuropatii a druhý centrální bolesti při roztroušené skleróze. Reliabilita jednotlivých modalit hodnocená pomocí Cronbachova alfa se pohybovala v rozmezí 0,593–0,926. Závěr: Česká jazyková verze komplexního QST protokolu prokázala velmi dobrou praktickou použitelnost pro hodnocení širokého spektra senzitivních abnormit a nízkou subjektivní náročnost vyšetření. Jedná se o spolehlivý test s vysokou vnitřní konzistencí a dobrou opakovatelností všech hodnocených parametrů., Background: Quantitative sensory testing (QST) following the protocol of the German Research Network on Neuropathic Pain (DFNS) is frequently used for the evaluation of sensory profi les in neuropathic pain patients. It covers a broad spectrum of sensory modalities using both nociceptive and non-nociceptive thermal and mechanical stimuli. The protocol requires the use of standardized instructions and thus necessitates validation of each language version. Material and methods: The standardized instructions for DFNS-QST protocol were translated using the forward-backward translation method and validated in a group of patients suffering from neuropathic pain of peripheral (n = 68) or central origin (n = 59) due to painful diabetic polyneuropathy or multiple sclerosis, and in a group of healthy volunteers (n = 75). The testing was repeated four times (with one-month interval between examinations) in 26 individuals to determine test-retest reliability. Results: In a group of healthy volunteers, 95% of values across all the parameters were within the physiological range based on published normal data. At least one sensory abnormity was found in 90% of neuropathic pain patients. Cluster analysis identified two distinctive pain profiles, the first mostly consisted of patients with painful diabetic neuropathy and the second of patients with pain in multiple sclerosis. Repeated administration confirmed an acceptable test-retest reliability (Cronbach´s alpha of 0.59–0.93 for the different sensory modalities). Conclusion: The Czech version of DFNS-QST protocol instructions displayed a good practical applicability in evaluating sensory abnormities in neuropathic pain patients, acceptable reliability and high internal consistency of the findings. The protocol provides results comparable to the original language version and can be recommended as an easy-to-apply and suitable testing method.
