Gastrointestinální stromální tumory (GIST) jsou nejčastějším mezenchymálním nádorem trávicího traktu s incidencí kolem 7–19 případů na 1 milion. Vyskytují se v celém zažívacím traktu, s predilekční lokalizací v žaludku (65 %), tenkém střevě (25 %), tlustém střevě a konečníku (5–10 %) a výjimečně v jícnu a jiných lokalizacích (5 %). Incidence v České republice není přesně známa, ale odhaduje se na 300–350 nových případů za rok. Jejich léčba je primárně chirurgická. Na rozdíl od zhoubných epiteliálních nádorů GIT, které mají vysokou tendenci k metastazování a vyžadují kromě radikálního odstranění i velmi radikální spádovou lymfadenektomii, chirurgická léčba GIST je ve většině případů omezena na radikální odstranění nádoru do zdravé tkáně. Rozsáhlá ložiska někdy vyžadují multiorgánové resekce, ale základní podmínkou úspěchu je jejich odstranění bez porušení celistvosti. Způsoby chirurgické léčby jsou dány zkušenostmi pracoviště a zahrnují jak klasický otevřený přístup, tak stále častěji používané miniinvazivní laparoskopické metody. Prognóza onemocnění se odvíjí od radikality operace a maligního potenciálu, který je dán velikostí nádoru a mitotickým indexem. Adjuvantní léčba GIST je v kompetenci specializovaného onkologického oddělení a ve většině případů spočívá v podávání molekulárně cílené biologické terapie. Zavedení cílené biologické léčby v podobě imatinib mesylátu představuje obrovský pokrok s dramatickým zlepšením výsledků přežívání i pokročilých a metastatických forem onemocnění. V současné době jsou k dispozici i další tyrozinkinázové inhibitory jako sunitinib malát, sorafenib, nilotinib, masitinib aj. Ačkoli jsou silné indicie proto, že by neoadjuvantní léčba mohla mít přínos v léčbě pokročilých stadií GIST, k dispozici zatím nejsou „evidence based“ údaje, které by opravňovaly její použití. V současné době probíhají v Německu a Kanadě studie fáze 2., které na tyto otázky přinesou odpověď., Gastrointestinal stromal tumours (GISTs) are the most common gastrointestinal mesenchymal tumours with an incidence of about 7–19 cases per one million population. They occur in the whole gastrointestinal tract; the predilection sites include the stomach (65 %), small intestine (25 %), large intestine and rectum (5–10 %) and rarely the oesophagus and other sites (5 %). The incidence in the Czech Republic is not exactly known but it is estimated to be 300–350 new cases per year. The treatment is primarily surgical. Unlike malignant epithelial GIT tumours that are very likely to metastasize and require, in addition to being removed radically, a very radical sentinel lymphadenectomy, the surgical treatment for GIST is mostly restricted to radical resection of the tumour from healthy tissue. Extensive foci sometimes require multiorgan resection but success is mainly determined by their removal without disrupting integrity. The methods of surgical treatment depend on the experience of the workplace and include both the classic open approach technique as well as the more increasingly used mini-invasive laparoscopic procedures. The disease prognosis depends on the radicality of surgery and the malignant potential which is determined by the size of the tumour and mitotic index. Adjuvant treatment for GIST is provided by a specialized oncological centre and, in most cases, consists in administering molecular-targeted biological therapy. The introduction of targeted biological therapy using imatinib mesylate represents a tremendous progress with a dramatic improvement in survival outcomes for both advanced and metastatic forms of the disease. Currently, other tyrosine kinase inhibitors, such as sunitinib malate, sorafenib, nilotinib or masitinib, are available. Although there are strong suggestions that neoadjuvant therapy might be beneficial in treating advanced stages of GIST, no evidence-based data are available to date that would warrant its use. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. Článek se zabývá možnostmi terapie gastrointestinálního stromálního nádoru – jednoho z prvních solidních nádorů, u kterého byla chemická a radiační léčba zcela vytlačena cílenou biologickou léčbou. Primární „target“ léčbou u pokročilého inoperabilního gastrointestinálního stromálního nádoru představuje imatinib mesylát. Kromě přehledu klinických studií a léčebných výsledků odpovídá i na kontroverzní otázky – iniciální dávka imatinibu, prediktivní faktory, které možná povedou ke standardní primární eskalaci dávky, dále otázka pokračování v léčbě i po dosažení kompletní remise. Článek také poukazuje na další možnosti pokračování v biologické cílené terapii i po selhání imatinibu – užití sunitinibu, nilotinibu, mTOR., The article deals with possibilities of gastrointestinal stromal tumour therapy – one of the first solid tumour, whose chemical and radiation treatment was completely displaced by biologic treatment. Imatinib mesylát represents primary target treatment of progressed inoperable gastrointestinal stromal tumour. Except overview of clinical studies and treatment results the article responses to controversial questions – initial imatinib dosage, predictive factors, which cal lead to standards primary dosage escalation, question of treatment continuation after complete remission. Stanovení FreeLite Chains (FLC) nachází v klinické praxi stále širší uplatnění. Stává se vedle elektroforézy bílkovin séra a imunofixační elektroforézy součástí diagnostiky, prognostiky a monitorování terapie u monoklonálních gamapatií. Vlastní stanovení FLC má však zatím četná metodická úskalí a je snaha o jejich postupné odstranění. Touto snahou byly také vedeny Česká myelomová skupina a Česká společnost klinické biochemie, když organizovaly studii, při které šest laboratoří center léčby mnohočetného myelomu v České republice současně vyšetřilo 12 vzorků sér od nemocných monoklonálními gamapatiemi na stanovení FLC. Z této studie vyplývají tyto závěry: jednotlivé laboratoře sjednotí ředění sér podle pokynů výrobce, index kappa/lambda je stanovitelný s menší přesností, proto je doporučeno používat ve větší míře hodnot koncentrací FLC. But there are numerous uncertainties regarding the detection, interpretation and FLC quantification. Our interlaboratory study showed that kappa/lambda ratio is strongly influenced by measurement errors and therefore we recommended the preferential use of FLC concentration values. Věkem podmíněná makulární degenerace (VPMD) je v rozvinutých zemích nejčastější příčinou ztráty zraku lidí starších 55 let. Vlhká forma této choroby je příčinou těžké ztráty zraku u 90 % postižených. VPMD vzniká spolupůsobením genetických faktorů a faktorů životního stylu. Klíčovým mechanizmem vzniku vlhké formy VPMD je novotvorba cév, stimulovaná růstovým faktorem VEGF. Léčbou volby VPMD jsou proto preparáty anti-VEGF, podávané formou injekcí do sklivce. V mezinárodních studiích jsou zkoumány nové léky na VPMD. Léčbu je třeba zahájit včas, dokud není vážně postižena zraková ostrost., Age-related macular degeneration (AMD) is the most common cause of vision loss in developed countries in people older than 55 years. The wet form of this disease is the cause of severe vision impairment in 90 percent of patients. AMD develops as a result of an interaction between genetic and lifestyle factors. The key mechanism in the development of wet AMD is the growth of new vessels, stimulated by the vascular endothelial growth factor (VEGF). The treatment of choice is, therefore, the use of anti-VEGF agents administered as intravitreal injections. International clinical studies are evaluating new AMD treatments.
