Few studies concerning the occurrence of cow’s milk allergy with the use of double-blind, placebo controlled food challenge test in adolescents and adult patients suffering from atopic dermatitis exist. Aim: To evaluate the occurrence of cow’s milk allergy in adolescents and adults suffering from atopic dermatitis. Method: Altogether 179 persons suffering from atopic dermatitis were included in the study: 51 men and 128 women entered the study with the average age of 26.2 (s.d. 9.5 years). Complete dermatological and allergological examinations were performed. Results: The positive results in specificIgE and in skin prick tests were recorded in 12% of patients. According to the open exposure tests and double-blind, placebo controlled food challenge tests these patients are only sensitized to cow’s milk without clinical symptoms of allergy. Double-blind, placebo controlled food challenge test confirmed food allergy to cow milk only in one patient (worsening of atopic dermatitis), the oral allergy syndrome was observed in another one patient, occurrence of this allergy was altogether 1.1%. Cíl: Cílem studie bylo zhodnocení vlivu dlouhodobého podávání nápoje se zvýšeným obsahem syrovátkových proteinů a sníženým obsahem laktózy vyrobeného podle nové receptury (Nutrisen) seniorům žijícím v institucionalizované péči. Metodika: Studie proběhla u 47 klientů domova pro seniory v Praze starších 65 let v období od května roku 2013 do července roku 2013. Suplementovaná skupina (23 osob) konzumovala denně 200 ml mléčného nápoje ve 3 různých chuťových variantách po dobu 8 týdnů. Kontrolní skupina konzumovala běžnou stravu. V základních charakteristikách se obě skupiny významně nelišily. Byly hodnoceny antropometrické a biochemické ukazatele nutričního stavu a snášenlivost preparátu při dlouhodobém podávání. Výsledky: Snášenlivost a kompliance nutričně definovaného nápoje byla velmi dobrá (86 %). U intervenované skupiny došlo po 8 týdnech ke zvýšení tělesné hmotnosti v průměru o 700 g. Průměrná hladina albuminu a prealbuminu se významně zvýšila (začátek studie vs 8týdenní podávání): 35,5 ? 4,52 g/l vs 36,19 ? 4,1 g/l a 0,160 ? 0,05 vs 0,174 ? 0,04 g/l (p < 0,05), hladina vitaminu D se významně zvýšila z 31,2 ? 16,4 nmol/l na 36,8 ? 17,7 nmol/l (p < 0,001) a došlo k významnému vzestupu HDL-cholesterolu: 1,29 ? 0,33 mmol/l vs 1,35 ? 0,35 mmol/l (p < 0,001). Závěr: Nutričně definovaný mléčný nápoj byl v 8týdenní klinické studii seniory dobře snášen a měl příznivý vliv na vývoj hmotnosti, hladinu albuminu, prealbuminu, vitaminu D a HDL-cholesterolu., Aim: The aim of this study was to evaluate the effect of long-term administration of liquid nutritional supplement with increased amounts of whey protein and reduced amounts of lactose, produced in accordance with a new recipe “Nutrisen” on the elderly living in institutionalized care. Methods: The study was carried out from May to July, 2013, on 47 retirement home residents, living in Prague, all of which were 65 years or older. Supplemented group (n = 23) consumed (200 ml) milk drinks with three different flavours on a daily basis for eight weeks. The reference group was on a normal diet. There was no significant difference in baseline characteristics between participants in both groups. Anthropometric and biochemical indicators of nutritional status and tolerance of the nutritional supplement during long-term use were evaluated. Results: Both compliance (daily intake program) and tolerance of the nutritionally defined supplement were very good. For the supplemented group, there was an average weight increased of 700 grams after the 8 week nutritional supplement test period. Average levels of albumin and prealbumin increased significantly (from the beginning to the end of the program), 35.5 ? 4.52 g/l vs 36.19 ? 4.1 g/l and 0.160 ? 0.05 vs 0.174 ? 0.04 g/l (p < 0.05), vitamin D levels increased from 31.2 ? 16.4 nmol/l to 36.8 ? 17.7 nmol/l (p < 0.001) and HDL-cholesterol levels increased from 1.29 ? 0.33 mmol/l to 1.35 ? 0.35 mmol/l (p < 0.001). Conclusion: The specific nutritionally defined milk drink (Nutrisen), used in this study, was well tolerated by the elderly study participants, over the eight-week clinical study.
