Úvod: Incidence kolorektálního karcinomu narůstá. Nejlepší léčebné výsledky má multimodální léčba, jejíž základní součástí je snaha o R0 resekci všech nádorových ložisek. Materiál a metodika: Byla provedena literární rešerše v souvislosti z vlastními zkušenostmi. Výsledky: Spektrum chirurgických zákroků zahrnuje endoskopickou submukózní resekci, transanální endoskopickou mikrochirurgii, laparoskopické resekce, otevřené resekce a cytoredukční chirurgii s peritonektomií a hypertermickou intraperitoneální chemoterapií. Nově se kromě konceptu totální mezorektální excize začíná uplatňovat také koncept totální mezokolické excize, který vede k nižšímu počtu lokálních recidiv a zvýšenému přežívání. Závěr: Stejně jako protinádorová farmakoterapie se významně stále vyvíjí i chirurgická léčba. K určení správné pozice jednotlivých chirurgických postupů v multimodálním algoritmu léčby je nutné sledování pacientů ve vhodných databázích, případně je léčit v rámci studií., Background: The incidence of colorectal carcinoma has been increasing. Only a multimodal treatment provides the best results. Surgical R0 resection of all tumour sites is the goal of all treatment regimes. Material and methods: A survey on the literature and our experiences has been conducted. Results: Surgical treatment comprises endoscopic submucosal dissection, transanal endoscopic microsurgery, laparoscopy, open surgery and cytoreduction surgery with the hyperthermic intraperitoneal chemotherapy. Gastrointestinální stromální tumory (GIST) jsou nejčastějším mezenchymálním nádorem trávicího traktu s incidencí kolem 7–19 případů na 1 milion. Vyskytují se v celém zažívacím traktu, s predilekční lokalizací v žaludku (65 %), tenkém střevě (25 %), tlustém střevě a konečníku (5–10 %) a výjimečně v jícnu a jiných lokalizacích (5 %). Incidence v České republice není přesně známa, ale odhaduje se na 300–350 nových případů za rok. Jejich léčba je primárně chirurgická. Na rozdíl od zhoubných epiteliálních nádorů GIT, které mají vysokou tendenci k metastazování a vyžadují kromě radikálního odstranění i velmi radikální spádovou lymfadenektomii, chirurgická léčba GIST je ve většině případů omezena na radikální odstranění nádoru do zdravé tkáně. Rozsáhlá ložiska někdy vyžadují multiorgánové resekce, ale základní podmínkou úspěchu je jejich odstranění bez porušení celistvosti. Způsoby chirurgické léčby jsou dány zkušenostmi pracoviště a zahrnují jak klasický otevřený přístup, tak stále častěji používané miniinvazivní laparoskopické metody. Prognóza onemocnění se odvíjí od radikality operace a maligního potenciálu, který je dán velikostí nádoru a mitotickým indexem. Adjuvantní léčba GIST je v kompetenci specializovaného onkologického oddělení a ve většině případů spočívá v podávání molekulárně cílené biologické terapie. Zavedení cílené biologické léčby v podobě imatinib mesylátu představuje obrovský pokrok s dramatickým zlepšením výsledků přežívání i pokročilých a metastatických forem onemocnění. V současné době jsou k dispozici i další tyrozinkinázové inhibitory jako sunitinib malát, sorafenib, nilotinib, masitinib aj. Ačkoli jsou silné indicie proto, že by neoadjuvantní léčba mohla mít přínos v léčbě pokročilých stadií GIST, k dispozici zatím nejsou „evidence based“ údaje, které by opravňovaly její použití. V současné době probíhají v Německu a Kanadě studie fáze 2., které na tyto otázky přinesou odpověď., Gastrointestinal stromal tumours (GISTs) are the most common gastrointestinal mesenchymal tumours with an incidence of about 7–19 cases per one million population. They occur in the whole gastrointestinal tract; the predilection sites include the stomach (65 %), small intestine (25 %), large intestine and rectum (5–10 %) and rarely the oesophagus and other sites (5 %). The incidence in the Czech Republic is not exactly known but it is estimated to be 300–350 new cases per year. The treatment is primarily surgical. Unlike malignant epithelial GIT tumours that are very likely to metastasize and require, in addition to being removed radically, a very radical sentinel lymphadenectomy, the surgical treatment for GIST is mostly restricted to radical resection of the tumour from healthy tissue. Extensive foci sometimes require multiorgan resection but success is mainly determined by their removal without disrupting integrity. The methods of surgical treatment depend on the experience of the workplace and include both the classic open approach technique as well as the more increasingly used mini-invasive laparoscopic procedures. The disease prognosis depends on the radicality of surgery and the malignant potential which is determined by the size of the tumour and mitotic index. Adjuvant treatment for GIST is provided by a specialized oncological centre and, in most cases, consists in administering molecular-targeted biological therapy. The introduction of targeted biological therapy using imatinib mesylate represents a tremendous progress with a dramatic improvement in survival outcomes for both advanced and metastatic forms of the disease. Currently, other tyrosine kinase inhibitors, such as sunitinib malate, sorafenib, nilotinib or masitinib, are available. Although there are strong suggestions that neoadjuvant therapy might be beneficial in treating advanced stages of GIST, no evidence-based data are available to date that would warrant its use. Péče o nemocné se sarkomy měkkých tkání či sarkomy dutiny břišní a retroperitonea, kterých je celkem ročně v České republice asi 350, patří pouze omezenému počtu pracovišť, která jsou schopna a ochotna zajistit komplexní a kvalifikovanou zobrazovací a histopatologickou diagnostiku, širokou víceoborovou škálu operačních výkonů v různých anatomických lokalitách, včetně rekonstrukčních přístupů, zevní radioterapii i intersticiální brachyradioterapii, systémovou i intraarteriální chemoterapii, bedlivou dispenzarizaci nemocných a hodnocení dlouhodobých výsledků vlastní léčebné péče. Výsledky chaotické a náhodné péče o nemocné se sarkomy nelze účinně vylepšit ani novými diagnostickými a léčebnými metodami., Management of patients with soft tissue sarcomas elsewhere in the body (annual incidence in the Czech Republic about 350 cases) should be concentrated only to limited number of hospitals and teams which are able to provide comprehensive diagnostic services, including specialized histopathology, multidisciplinary spectrum of surgical operations, external radiotherapy and interstitial brachyradiotherapy, systemic and regional chemotherapy, careful long-term follow-up of patients and self-evaluation of therapeutic outcomes. Autoři prezentují sestavu 76 nemocných radikálně operovaných pro karcinom hlavy pankreatu a analyzují pooperační komplikace a přežívání. Materiál a metodika: V období 1/2006–9/2010 podstoupilo 76 nemocných potenciálně kurativní hemipankreatoduodenektomii pro adenokarcinom hlavy pankreatu. Pacienti jsou prospektivně sledováni, diskutovány jsou komplikace a přežití. Výsledky: 30denní pooperační mortalita je 2,6 %. Na základě TNM klasifikace 11 % mělo stadium I, 78 % stadium II a 3 % stadium III. Nejzávažnější komplikací byla insuficience pankreatojejunální anastomózy (6,5 %) a opožděná evakuace žaludku (11,8 %). Ve stadiu I přežívá 1., 2., 3. a 4. rok 82 %, 52 %, 35 % a 35 % operovaných. Závěr: Resekce hlavy pankreatu je bezpečná operace s nízkou mortalitou, ale neuspokojivým dlouhodobým přežíváním. Toto je zřejmě způsobeno agresivní biologickou aktivitou nádoru a pozdní diagnostikou., The authors refer seventy six patients radically operated for pancreatic head carcinoma and analyzed postoperative complications and survival. Material and Methods: During 1/2006–9/2010 76 patients underwent potentially curative hemipancreato-duodenectomy for adenocarcinoma of the head of pancreas. The patients are prospectively followed, complications and survival are discussed. Results: The 30-days postoperative mortality was 2,6 %. According to TNM classification 11 % of patients had stage I, 78 % stage II and 3 % stage III. Most severe complications were leak from pancreato-jejunal anastomosis (6,5 %) and delayed gastric emptying (11.8 %). The survival analyses showed 1,2,3 and 4 years survival as follows: 82 %, 52 %, 35 %, 35 % respectively. Conclusion: Resection of the pancreatic head is a safe procedure with low mortality but disappointing long-term survival. Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary.
