Cíl: Vytvoření kvalitního digitálního obrazu systémem ClarityIQ na angiografickém přístroji AlluraClarity Xper FD 20/20 (Philips Medical System, Best, Holandsko) je spojeno s nižší radiační zátěží pacienta než u předchozí generace přístrojů. Cílem práce bylo srovnání efektivních dávek ionizujícího záření na dvou angiografických přístrojích Allura Xper FD 20 (Philips Medical System, Best, Holandsko) a Philips Allura Clarity Xper FD 20/20 (Philips Medical System, Best, Holandsko). Metodika: V retrospektivní studii jsme analyzovali konsekutivně vybraných 100 angio-intervenčních vyšetření provedených u 100 nemocných v časovém období od 1. 7. 2012 do 1. 7. 2013. Na angiografickém kompletu Philips Allura Clarity Xper FD 20/20 (Philips Medical System, Best, Holandsko) s technologii ClarityIQ byla konsekutivně vybrána vyšetření 50 nemocných (34 mužů, 16 žen průměrného věku 64 let, průměrné hmotnosti 79 kg a průměrné výšce 170 cm). Na přístroji Allura Xper FD 20 (Philips Medical System, Best, Holandsko), který nepracuje se systémem ClarityIQ, bylo konsekutivně vybráno 50 pacientů (30 mužů, 20 žen, průměrného věku 64 let, průměrné hmotnosti 78 kg a průměrné výšce 170 cm). Dané skupiny byly rozděleny podle druhu intervenčního výkonu na podskupiny čítající 10 provedených intervenčních vyšetření na každém z přístrojů: vytvoření transjugulárních intrahepatálních portosystémových spojek (TIPS), zavedení břišních stentgraftů u aneuryzmatu břišní aorty, provedené neurointervenční ošetření u pacientů po prodělaném subarachnoideálním krvácení z aneuryzmatu na mozkové cévě (SAK), provedené neurointervenční ošetření u pacientů s prodělanou cévní mozkovou příhodou (CMP), provedené neurointervenční ošetření zavedením stentu u pacientů s karotickou stenózou (CAS). U vytvořených podskupin jsme výpočtem stanovili hodnoty efektivních dávek. Statistická významnost snížení efektivních dávek pacientů vyšetřených na přístroji vybaveném systémem ClarityIQ byla prokázána testem ve statistickém programu NCSS 2007. Výsledky: Významné snížení radiační zátěže použitím systému ClarityIQ bylo prokázáno výrazným poklesem efektivních dávek. Snížení efektivní dávky se pohybovalo od 50 % do 70 % a bylo statisticky významné. Závěr: Systém ClarityIQ významně snižuje radiační zátěž pacienta spojenou s výkony v oblasti intervenční radiologie, a proto by měla být přístrojům s podobnými systémy dána přednost., Aim: Creation of a high-quality digital image system ClarityIQ on angiography equipment AlluraClarity Xper FD 20/20 (Philips Medical System, Best, The Netherlands) is associated with lower patient radiation burden than the previous generation of equipments. The aim of the study was to compare the effective doses of ionizing radiation on two angiographic intervention equipments Allura Xper FD 20 (Philips Medical System, Best, The Netherlands) and Philips Allura Xper FD Clarity 20/20 (Philips Medical System, Best, The Netherlands). Methods: In this retrospective study we analyzed 100 interventional examinations consecutively performed in 100 patients from 1st July 2012, until 1st July 2013. Fifty patients (34 males, 16 females, mean age 64 years, average weight 79 kg and average height 170 cm) were examined using the angiography set Philips AlluraClarity FD 20/20 employing the ClarityIQ technology. Fifty patients (30 males, 20 females, mean age 64 years, average weight 78 kg and average height 170 cm) were examined using the Allura Xper FD 20 set that does not work with the ClaritylQ system. The groups were divided according to the type of intervention on the performance of subgroups comprising 10 interventional procedures on each of the machine: the creation of transjugular intrahepatic portosystemic shunts (TIPS), the implantation of abdominal stentgrafts for abdominal aortic aneurysm, neurointerventional procedures in patients after subarachnoideal hemorrhage from a cerebral aneurysm (SAH), neurointerventional treatment of the patients with ischemic stroke, patients with carotid artery stenting (CAS). The generated subgroups are determined by calculating the values of effective doses. The statistical significance of the reduction in effective doses of patients examined on the device that is equipped with ClaritylQ was demonstrated by tests of statistical program NCSS 2007. Results: Significant reduction of radiation burden to patients has been shown by significant decrease of effective doses in the ClarityIQ system. The effective dose decrease ranged from 50% to 70% and was proved to be statistically significant. Introduction: Ankylosing spondylitis (AS) is an inflammatory rheumatic disease characterized by the development of osteoproductive changes in the spine which could possibly result in ankylosis. Treatment with tumour necrosis factor alpha (TNFα) inhibitors has proved to be an important step forward in the treatment of this disease, but for the time being it is not clear whether it favourably influences radiographic progression of the disease. Vascular endothelial growth factor most probably plays a role in the development of osteoproductive changes and recently its predictive influence on radiographic progression has been demonstrated. Bone morphogenic protein 2 (BMP-2) participates in the regulation of bone proliferation and its increased serum level has been demonstrated in patients with advanced AS and correlated with the degree of radiographic changes. Aim: The study aims to evaluate the VEGF and BMP-2 levels in patients with ankylosing spondylitis and how these levels relate to the concurrent treatment with TNFα inhibitors. Methods: Sera were evaluated from patients at the Rheumatologic Clinic of the Hradec Králové Faculty Hospital who fulfilled the modified New York Criteria for AS (n = 55). In these patients, the parameters of the activity of the disease (BASDAI = Bath Ankylosing Spondylitis Disease Activity Index, CRP = C-reactive protein) and the concurrent therapy (TNFα inhibitors, n = 21, vs. non-anti TNFα, n = 34) were recorded. The levels of VEGF and BMP-2 were analyzed using the ELISA method. Results: In patients treated with TNFα inhibitors, a significantly lower VEGF level was found when compared to untreated patients (140.3 (109.4; 262.2) vs. 261 (172.4; 396.6) pg/ml; p = 0.02). No difference was found between BMP-2 levels in both groups (treated vs. untreated patients) (254.8 (2301; 267.3) vs. 261.1 (248.6; 273.5) pg/ml; p = 0.24). A correlation analysis did not reveal any relationship between VEG F and BMP-2 (r = 0.057; p = 0.68). Serum levels of VEGF correlated with serum levels of CRP (r = 0.56; p = 0.00001) and the BASDAI value (r = 0.33; p = 0.015). Conclusion: Significantly lower VEGF levels were found in patients treated with TNFα inhibitors versus the untreated patients. These findings are in harmony with some hitherto published analyses and may give evidence of a favourable effect of TNFα inhibitors on radiographic progression.
