Cíl: Cílem této studie je poskytnout orientační normy k Addenbrookskému kognitivnímu testu (ACE‑<span class=""></span>R) a jeho jednotlivým kognitivním doménám pro českou populaci. Soubor a metodika: Do studie bylo zařazeno 143 osob (89 žen a 54 mužů) ve věku 55–<span class=""></span>89 let. Subjekty byly zástupci zdravé populace, bez onemocnění mozku, smyslové poruchy a závažného psychiatrického onemocnění, s MMSE nad 27 bodů. Osoby byly podrobeny detailnímu anamnestickému rozhovoru pro zjištění případných onemocnění mozku v minulosti a pro zjištění míry soběstačnosti v běžných denních činnostech. Osoby s poškozením mozku v anamnéze či porušenou soběstačností v běžných denních činnostech nebyly do studie zařazeny, stejně jako osoby s MMSE nižším než 27 bodů. Srovnání hodnot ACE‑<span class=""></span>R a jeho složek u mužů a žen bylo provedeno pomocí Mannova‑<span class=""></span>Whitneyova testu a srovnání hodnot u čtyř skupin daných věkem a vzděláním pomocí Kruskalova‑<span class=""></span>Wallisova testu. Výsledné p‑<span class=""></span>hodnoty byly korigovány na mnohonásobné testování užitím Bonferroniho korekce. Hraniční skóry byly vytvořeny na úrovni 2. a 7. percentilu. Výsledky: Byla prokázána negativní korelace s věkem (p < 0,001; r = –<span class=""></span>0,43) a pozitivní korelace s proměnnou vzdělání na celkový výsledek v testu (p < 0,001; r = 0,41) i v dosažených výsledcích některých jednotlivých kognitivních domén. Vliv pohlaví nebyl statistický prokázán. Hraniční skór pro všechny subjekty ve věku 55–<span class=""></span>89 na úrovni 2. percentilu je 74 bodů pro celkový skór v Addenbrookském kognitivním testu a 79 bodů na úrovni 7. percentilu. Závěr: Studie poskytuje orientační normy pro českou populaci pro Addenbrookský kognitivní test a nabízí bázi pro vytvoření kvalitních norem za předpokladu navýšení počtu subjektů., Aim: The aim of this study was to provide benchmark normal values for Addenbrooke’s cognitive examination (ACE-R) and its domains for the Czech population. Methods: The study included 143 healthy subjects (89 women and 54 men) aged 55–89 years, without brain injury, neurodegenerative disease, severe hearing or visual impairment and without a psychiatric disease, with MMSE above 27 points. Participants were interviewed in detail to ascertain any previous brain injury history and to determine the level of self-sufficiency in daily living activities. Individuals with a history of brain injury or impaired self-sufficiency were excluded from the study. ACE-R values and values for its domains in men and women were compared with Mann-Whitney test and values for the four age and education groups were compared using the Kruskal-Wallis test. P-values were corrected for multiple testing using Bonferroni correction. Results: Cut-off scores were set at 2nd and 7th percentile. Negative correlation with age (p < 0.001, r = –0.43) and positive correlation with education (p < 0.001, r = 0.41) were statistically significant for the overall test performance and also for the performance in individual cognitive domains except for the Attention and orientation domain. The effect of sex was not statistically significant. The cut-off score for the total score in Addenbrooke’s cognitive examination for all subjects aged 55–89 years is 74 points at the 2nd percentile and 79 points at the 7th percentile. Conclusion: The study suggests cut-off scores for the Czech population of ACE-R and provides the basis for the development of Czech norms for this test. V souboru 39 případů adenokarcinomu in situ děložního hrdla byly sledovány: věk pacientek, cytologické, kolposkopické a histologické nálezy, operační výkony a definitivní řešení. Medián věku byl 38 let (24–75). Prebioptická suspekce žlázové léze byla vyslovena v 7 cytologických (18 %) a v 7 kolposkopických (18 %) nálezech. U 28 nemocných (72 %) byla současně přítomna koexistující skvamózní léze. V 16 případech (46 %) z 35 konizací byly popsány histologicky pozitivní okraje léze a u 15 z nich byl proveden opakovaný nebo definitivní operační výkon. V něm byl ve 2 případech již invazivní adenokarcinom. Klíčová slova: adenokarcinom in situ – cytologie – děložní hrdlo – kolposkopie – pozitivní okraje, In a series of 39 cases of adenocarcinoma in situ of the uterine cervix, age of patients, cytological, colposcopical and histological findings, operations and definitive solutions have been analyzed. Median age was 38 (24 – 75). Praebioptic suspicion of a glandular laesion was expressed in 7 cytological (18 %) and 7 colposcopical (18 %) findings. In 28 patients (72 %) the coexistent squamous laesion was concurrently present. In 16 cases (46 %) from 35 conisations histologically positive margins were described and in 15 of them repeated or definitive operation was performed. Zlatým standardem léčby karcinomu endometria je chirurgická léčba. Adjuvantní radioterapie ve formě brachyterapie či zevní radioterapie je indikována u středně a vysoce rizikového karcinomu, kde snižuje zejména výskyt lokoregionální recidivy. U vysoce rizikového karcinomu jsou však pacientky ohroženy i generalizací onemocnění, což je důvodem pro zvažování zařazení adjuvantní chemoterapie do léčebného schématu. Chemoterapie může být použita současně s radioterapií jako konkomitantní chemoradioterapie nebo sekvenčně po skončení radioterapie. Článek popisuje výsledky prací, které použily adjuvantní chemoterapii. Výsledky nejsou zatím zcela jednoznačné, na závěry velkých studií stále čekáme. Přesto u vysoce rizikového karcinomu endometria již dnes v klinické praxi zvažujeme použití chemoterapie., Surgical treatment is the gold standard for treating endometrial carcinoma. Adjuvant radiotherapy in the form of brachytherapy or external radiotherapy is indicated in moderate and high-risk carcinoma where it particularly reduces the rate of locoregional recurrence. In high-risk carcinoma, however, patients are also at risk of disease generalization, which is the reason for considering the inclusion of adjuvant chemotherapy in the treatment regimen. Chemotherapy can be used concomitantly with radiotherapy as concomitant chemoradiotherapy or sequentially after radiotherapy termination. The article describes the results of papers that have utilized adjuvant chemotherapy. The results have not been entirely unequivocal so far, and the conclusions of large studies are yet to be published. Cílem imunoterapie je využití imunitních mechanizmů k eliminaci nebo ke kontrole růstu maligně transformovaných buněk. Jednou z možností imunoterapie je aplikace adoptivního transferu lymfocytů T, jehož podstatou je infuze ex vivo expandovaných tumor specifických lymfocytů, které rozeznávají a ničí neoplastické buňky. Úspěšnost adoptivní T buněčné terapie se významně zlepšila pochopením základních molekulárních mechanizmů interakce imunitního systému s nádory. Slibných výsledků bylo dosaženo zejména u maligního melanomu, karcinomu ledviny a lymfatické leukemie. Adoptivní T buněčná terapie se postupně etabluje mezi nadějné imunoterapeutické postupy a rozšiřuje tak portfolio léčebných možností, i když pro svoji metodickou náročnost nenašla dosud velké rozšíření., The aim of current immunotherapeutic approaches is to harness the individual’s immune system towards a tumor tissue to eliminate or, at least, to control proliferation of the neoplastic cells. Among the cancer treatment strategies, the adoptive T cell transfer is progressively developing method that uses an ex vivo modified and expanded tumor-specific T cells as a tool to target and destroy the neoplastic tumor cells. The efficacy of the adoptive T cell therapy has significantly improved in recent years due to a better understanding of the molecular mechanisms that control the interactions of immune system and tumor cells. Promising results have already been achieved within the clinical trials in malignant melanoma, renal cancer and lymphoid leukemia. Hemangiom je nejčastější primární benigní nádor páteře typu hamartomu, který nikdy nemalignizuje. V drtivé většině případů se jedná o asymptomatické solitární léze hrudní páteře jakožto náhodné nálezy radiografických vyšetření. Pouze v 1 % případů se hemangiom obratle projeví klinicky a agresivita této benigní léze pak spočívá v expanzivním chování nádorové tkáně či v důsledku patologické fraktury obratle. Klinickými příznaky potom jsou především dorzalgie, v pokročilých případech pak známky kořenové nebo míšní komprese. Radiodiagnostika je poměrně snadná, protože hemangiom obratle vykazuje zcela typické grafické příznaky, pro potvrzení diagnózy agresivního hemangiomu obratle jsou stanovena grafická kritéria. Spektrum léčebných modalit je velmi široké od konzervativní léčby přes hodně využívanou vertebroplastiku po radikální chirurgickou léčbu. Komplementárně jsou často využívány embolizační výkony, alkoholová sklerotizace a lokální radioterapie. Agresivní hemangiom obratle je poměrně velmi vzácná diagnóza. Proto zatím neexistují velké soubory či klinické studie, podle kterých by bylo možné stanovit optimální terapeutický postup., Hemangioma is the most common primary benign hamartoma type spine tumor. Malignant degeneration has never been reported. In the majority of cases, this is an asymptomatic solitary lesion of the thoracic spine found incidentally during a radiographic examination. Only 1% of cases manifests clinically and this benign lesion is considered aggressive because of the expansive nature of the tumor and because it may cause pathological fracture of the vertebrae. Clinical symptoms then include dorsalgia and either root or spinal neurological symptoms. Radiological diagnosis is relatively easy because vertebral hemangioma is associated with quite typical graphic signs; graphic criteria to confirm the diagnosis of aggressive vertebral hemangioma have been clearly defined. The range of treatment options is very wide, from conservative treatment through frequently used vertebroplasty to radical surgical treatment. Embolization procedures, alcohol sclerotherapy and local radiotherapy are often used as complementary procedures. Aggressive vertebral hemangioma is a relatively rare diagnosis, so far there are no large cohorts or clinical studies from which it would be possible to determine an optimal therapeutic approach. Akromegalie patří mezi vzácná a závažná onemocnění, základem její léčby je kombinace terapie chirurgické, radiační a medikamentózní. V práci se popisuje případ akromegalie diagnostikované před 10 lety ve stadiu, v němž byla možná již jen parciální resekce hypofyzárního tumoru. Přesto se podařilo dosáhnout dlouhodobé stabilizace stavu kombinací opakovaného chirurgického zákroku, radioterapie lineárním urychlovačem i Leksellovým gama nožem a medikamentózní léčby somatostatinovým analogem oktreotidem a blokátorem receptorů pro růstový hormon pegvisomantem. Dávka oktreotidu i pegvisomantu byla opakovaně měněna podle výsledků vyšetření hladiny IGF1, v současnosti je podáváno somatostatinové analogon Sandostatin LAR 30 mg jednou za 3 týdny a blokátor receptorů pro růstový hormon Somavert 20 mg denně. Přes přítomnost závažné choroby nemocný již po 10 let vede spokojený život. Náklady na finančně náročnou léčbu jsou z etického hlediska plně odůvodněné., Acromegaly is a rare and serious disease. A successful and rational therapy of acromegaly ought to combine surgery, radiotherapy and pharmacotherapy. The submitted article presents a case of acromegaly that was only diagnosed at the stage when total pituitary adenoma removal was impossible. Even so, the long-term stabilisation of the disease was reached by way of repeated surgery through transfenoidal and transcranial approach, by linear accelerator radiation therapy and Leksell Gamma Knife radiotherapy and by pharmacotherapy with somatostatin analogon octreotide and growth hormone receptor antagonist pegvisomant. The octreotide and pegvisomant dosage has been repeatedly changed according to IGF1 levels. The contemporary somatostatin analogon Sandostatin LAR 30 mg is given once every 3 weeks and the growth hormone receptor antagonist Somavert 20 mg is applied daily. Despite this serious disease, the patient has already been living contentedly for 10 years.
