Vysoká incidence v České republice a vysoký impakt chirurgické léčby u nemocných s kolorektálním karcinomem představují velkou zátěž pro naše chirurgická pracoviště. Zatímco léčbu nemocných s karcinomem kolon, především u akutních komplikací (zejména ileus), je nutné zabezpečit i na menších chirurgických pracovištích, karcinom rekta by měl být diagnózou léčenou na specializovaných pracovištích v rámci komplexních onkologických center. K výraznému zlepšení a zefektivnění léčby těchto nemocných může pomoci založení onkochirurgie jako specializační vzdělávací nástavby chirurgie. Jednoznačnou podmínkou úspěšné terapie nemocných s KRK je existence funkčních multioborových komisí., High incidence of colorectal carcinoma in Czech Republic and high impact of surgery leads to heavy burden for our surgery department. Whereas surgical treatment of patients with colon carcinoma should be covered by even smaller departments, rectal carcinoma is the diagnosis for the treatment in the complex oncologic centres. Přehledový článek je věnován problematice endoskopické léčby časných kolorektálních neoplazií. Autor popisuje indikace a technické provedení metod endoskopická polypektomie, endoskopická slizniční resekce a endoskopická submukózní disekce. Cílem je ukázat optimální využití počítačové tomografie (CT), magnetické rezonance (MR) a PET/CT (fúze počítačové emisní tomografie a CT) v předoperačním (klinickém) stagingu, plánování léčby a dispenzarizaci onkogynekologických pacientek se zhoubnými nádory děložního hrdla, těla a ovarií. Podáváme přehled o možnostech a limitech jednotlivých zobrazovacích modalit komplementárních k ultrazvukovému vyšetření, které zůstává metodou první volby v zobrazení v gynekologii. Práce by měla být vodítkem pro kliniky, jak využívat dané techniky efektivně., The aim of this review is to demonstrate the optimal use of modern imaging methods (CT, MRI and PET/CT) in the preoperative staging, therapeutic management and follow-up of gynecological cancer patients (neoplasm of cervix, uterus and ovary). We present an overview of the advantages and limitations of modern imaging modalities that are complementary to ultrasound, which remains the first choice in gynecology.
