Článek popisuje případ pacienta s náhle vzniklou obstipací, bolestmi břicha a normocytární anémií. Endoskopické vyšetření horní a dolní části trávicího traktu vyloučilo organickou příčinu obtíží. Vyšetření nátěru krve a aspirátu kostní dřeně s nálezem hrubého bazofilního tečkování v erytrocytech a erytroblastech upozornilo na možnost otravy těžkými kovy. Hodnota plumbemie 8,4násobně překročila limit přípustný pro běžnou populaci. Jako zdroj intoxikace byla identifikována olovnatá glazura keramického džbánku, ze kterého pacient pil zhruba 3 měsíce čaj s citronem. Koncentrace olova v čajovém výluhu dosahovala hodnoty 227 mg/kg. Otrava olovem v souvislosti s neprofesionální expozicí tomuto kovu je ve vyspělých zemích výjimečnou situací provázenou nespecifickou symptomatologií, a tak může zůstat nerozpoznána. Mikroskopické vyšetření nátěru krve s nálezem převážně hrubého bazofilního tečkování erytrocytů však může na tuto vzácnou diagnózu upozornit., This article reports on patient that has been presented with sudden onset of constipation, abdominal pain and normocytic anemia. Gastroscopy and colonoscopy ruled out an organic diseases. In peripheral blood and bone marrow aspirates mears, coarse basophilic stippling of erythrocyte (and erythroblasts) point out a possibility of heavy metal poisoning. The level of plumbemia exceeded 8.4 times the maximal permitted value for common (non-professional) population. A source of poisoning was indentified from a glaze on a ceramic jug, from which the patient had drank tea with lemon for three months. A lead concentration in the tea extract was 227 mg/kg. In developed countries, lead poisoning is a rare diagnosis. As the symptoms are nonspecific, missed diagnoses could occur, especially in sporadic, non-occupational exposure. Description:
Purpose: To investigate the protective action of Ceruloplasmin (Cp) on the lysis of erythrocytes in men with prostate tumors. Material and Methods: The blood erythrocytes of the patients with benign hyperplasia of prostate (BHP) and prostate adenocarcinoma (CaP) were studied. Patients at the age 60-75 with early stages of a cancer have been investigated. The control group was consisted of apparently healthy males with the compatible age. n=15 for each group. The clinical stage of the disease was diagnosed by means of rectal, histological and echographic examination of the prostate gland. Photometric methods were applied to register lysis dynamics in order to test out the protective action of Cp. Results: The nonspecific protective function of Cp preparation on BHP and CaP erythrocytes as well as on the control group erythrocytes was revealed. CaP erythrocytes have shown more sensitivity to the lysis provocative factor than BHP and the control group erythrocytes, that was presumably attributed to the structural and functional changes of the erythrocytes developed in the presence of malignant tumor., N. 14. Relationship of connective tissue dysplasia and hypomagnesemia in genesis of juvenile dysmenorrhea therapy

We studied the relationship of connective tissue state with magnesium and hydroxyproline levels in blood and urine of young women with primary dysmenorrhea. Study showed that in girls with dysmenorrhea and phenomena of connective tissue dysplasia hydroxyproline level in urine was increased which was associated with increased degradation of collagen and decreased level of magnesium in blood serum. Autoři prezentují případ 73leté ženy léčené v posledních 15 letech pro arteriální hypertenzi diuretiky (tiazidy, amiloridem a indapamidem) a současně antidepresivem trazodonem. Po 6 letech této léčby začala pacientka trpět manifestní osteoporózou s vícečetnými frakturami i po banálním traumatu a současně byla po celou tuto dobu dokumentována těžká hyponatremie bez adekvátní suplementace. Osteoporóza a hyponatremie u této pacientky spolu s největší pravděpodobností těsně souvisely – autoři diskutují možné patofyziologické mechanizmy na úrovni renálních tubulů, osteoblastů a osteoklastů, hypofyzárně-skeletální osy, syndrom neadekvátní sekrece adiuretinu (SIADH) a možný vliv porušené acidobazické rovnováhy., The authors present a case-report of a 73years old woman treated for arterial hypertension within past 15 years with diuretics (thiazides, amiloride and indapamide) and concomitantly with antidepressive trazodone. The patient developed severe osteoporosis after 6 years of such a treatment with multiple fractures even after minor trauma; during whole this period severe hyponatremia without adequate supplementation was documented.
