Autoři v předkládaném sdělení informují o zkušenostech z vlastního pracoviště získaných pri využíváni metody vlhkého hojení chronických ran. Přestože tato problematika byla již mnohokrát zpracována a v odborném tisku prezentována, domníváme se, že náš článek bude skromným příspěvkem k úspěšnému zvládání mnohdy tíživých situaci v této léčbě zejména u seniorů. Jsou zmíněny indikace pro metodu vlhkého hojení ran, jejich charakteristika i zastoupení v prezentovaném souboru. A konečně jsou také prezentovány konkrétní kazuistiky, důvodu zestručnění vybíráme z 62 případů 6 dle našeho názoru nejzajímavějších., In this notification the authors are trying to give some information about thein own experiences gained by by using mist festering wound healing. We suppose that our article will contribute to successful mastering of many difficult cases connected with the treatment of patiens (especialy senior), although these problems have often been raised and published in the medical press. V úvodu se autor zabývá možnými aspekty revmatických onemocnění ve stán. Popisuje skupinu onemocnění, která má ve stáří častější výskyt než u mladších osob (příklady osteoartróza, osteoporóza), dále se věnuje nemocem, které se prakticky vyskytují pouze ve stáří (polymyalgia rheumatica a temporální arteriitida). Revmatoidní artritída a další zánětlivé revmatizmy začínají sice většinou ve středním věku, ale část až u seniorů a má pak poněkud modifikovaný obraz. Pokud RA začne v mládí či ve středním věku, vidíme pak u seniorů velmi pokročilé stavy s řadou deformit, komplikací i jatrogenních změn, které vedou ke značné disabilitě. V další části publíkače se autor věnuje třem základním a do určité míry i modelovým onemocněním pro seniory, a to osteoartró ze, revmatoidní artritidě a revmatické polymyalgii s temporální arteriítidou. U osteoartrózy autor zdůrazňuje nebezpečí redukování léčby osteoartrózy na podávání nesteroidních antirevmatik, což může u seniorů vyvolávat závažné nežádoucí účinky. Naopak podporuje nefarmakologické způsoby léčby a upozorňuje na možnosti chirurgické léčby i u vyšších věkových ročníků. U revmatoidní artritídy je zdůrazněna relativní bezpečnost tzv. chorobu modifikujících léků RA u seniorů, včetně biologické léčby. U revmatické polymyalgie jsou podrobně anályzovány různé algoritmy podávání kortikosteroidů., In the introduction the author deals with possible aspects of rheumatic diseases in the elderly. He describes 1 group of diseases with ler incidence in the elderly than in young people (e.g. osteoarthritis, osteoporosis) and also engages in diseases which practically occur only in the elderly (polymyalgia rheumatica and arteriitis temporalis). Rheumatoid arthritis (RA) and other inflammatory rheumatisms start usually in the middle age, but a part of them begins not until old age and in that case they have rather altered manifestation. If RA starts in the young or middle age, we can see very advanced stages with many deformities, complications and iatrogenic changes leading to a significant disability in the elderly. The author devotes the subsequent part of the publication to three basic and to a certain extent model diseases in the elderly - osteoarthritis, rheun jniatoid arthritis and polymyalgia rheumatica. He emphasizes a risk of reducing the treatment of osteoarthritis to administration of nonsteroid antirheumatics, which can lead to severe adverse effects. On the contrary he supports non-pharmacological treatment methods and points out possibilities of surgical treatment in older age-groups. A relative safety of so called disease-modifying rheumathoid arthritis drugs in the elderly including a biological treatment is highUghted in rheumathoid arthritis.
