Arrestiny jsou významné intracelulární proteiny regulující G-protein-spřaženou receptorovou (GPCR) signalizaci. Tvoří komplexy s většinou GPCRs (po jejich aktivaci navázáním agonisty a fosforylaci) a hrají klíčovou roli v procesech receptorové homologní desenzitizace, sekvestrace a downregulace, které vedou k terminaci aktivace G-proteinů. V lidském organismu je zastoupeno deset typů arrestinů náležících do dvou skupin: mezi zrakové/beta arrestiny a alfa arrestiny. Nedávno bylo zjištěno, že skupina zrakových/beta arrestinů (která je tvořena čtyřmi členy: rod arrestinem, ß-arrestinem 1, ß-arrestinem 2 a cone arrestinem) je odvozena od nově objevených alfa arrestinů. Označení „alfa“ je velmi výstižné, tato skupina arrestinů je fylogeneticky starší a název je komplementární k názvu skupiny beta. Rod a cone arrestiny se nacházejí v buňkách sítnice, kde regulují funkci fotoreceptorů. ß-arrestiny jsou ubikvitně vyjádřeny, jejich nejvyšší koncentrace byly zjištěny v mozku a ve slezině. Kromě tradiční role v desenzitizaci (a následujících procesech) podporují ß-arrestiny též tvorbu signalizačních komplexů s tyrozinkinázou Src a s mitogenem-aktivovanými proteinkinázami, které umožňují G-protein-spřaženým receptorům signalizovat nezávisle na G-proteinech. V těchto kaskádách slouží jako „scaffolding“ a adaptorové proteiny a regulují buněčné procesy jako např. chemotaxi, apoptózu a metastázování. ß-arrestiny se tak stávají lákavým terapeutickým cílem pro léčbu některých nádorových onemocnění (např. karcinomu prsu, plic, kolorekta), alergického astmatu, hypertenze a dalších nemocí., Arrestins are important intracellular proteins, multifunctional regulators of G-protein-coupled receptor (GPCR) signaling. The rod and the cone arrestins are expressed in the retina, where they regulate photoreceptor function. The ß-arrestins are ubiquitously expressed proteins whose highest levels of expression are in the brain and spleen. Besides their role in desensitization (and following processes), ß-arrestins promote the formation of signaling complexes with tyrosine kinase Src and mitogen-activated protein kinase cascades allowing G-protein-coupled receptors to signal independently from G-protein. They serve as scaffold and adaptor proteins in these cascades and regulate cellular processes such as chemotaxis, apoptosis, and metastasis. Serum or plasma creatinine level determination has been serving for decades as the most commonly used marker of renal function. However, creatinine levels are affected by a number of other factors not related to renal function, particularly age, gender, muscle mass, starvation, wasting diseases, post surgical states, and activity or exertion. Alternatives for the assessment of renal function or glomerular fi ltration rate (GFR), through measurement of the clearance of exogenous substances like, inulin, 51Cr-ethylendiaminotetraacetate (51Cr-EDTA), 99mTc-diethylenetriaminepentaacetic acid or iohexol are more accurate, but too complex, laborious, and not suitable for routine clinical use. Therefore, serum creatinine measurement yet remains the most common method to estimate renal function in routine clinical practice. In the presented case study, nearly 50% decline in serum creatinine level as measured by enzymatic colorimetry method according to the manufacture’s recommendation is described. Relatively rapid serum creatinine concentration decline from 67 µmol/L baseline value to 30 µmol/L within 4 days in a CF patient post bilateral lung transplantation, who, was on courses of essential immunosuppressive, antimycotic, and antibacterial drugs including an aminoglycoside (amikacin) was observed. The change in serum creatinine level within four consecutive days after bilateral lung transplantation (mean difference, -49.25 % µmol/L; with 95% confi dence interval, -29 to -37 µmol/L) was signifi cant. Herein, the event is not essentially refl ecting better renal function, since amikacin trough level was unacceptably high (20 mg/L) at the same time, rather explaining probably low production of creatinine as a result of underlying pathology in cystic fi brosis. Patient related explanations and existing methodological challenges have also been discussed., Hladina sérového nebo plazmatického kreatininu slouží již desítky let jako nejčastější používaný ukazatel renální funkce. Přesto je koncentrace sérového kreatininu ovlivněna jinými, s funkcí ledvin nesouvisejícími faktory (věk, pohlaví, svalová hmota, stav výživy, nemoci poškozující organismus, pooperační stavy, tělesná aktivita nebo vyčerpávající fyzický výkon). Alternativy pro zjištění renální funkce/gromerulární fi ltrace (GFR) pomocí podání exogenních látek (inulin, 51Cr-ethylendiaminotetraacetát (51Cr-EDTA), 99mTc-diethylenetriaminepentaacetát nebo iohexol) jsou přesnější, ale zároveň velmi složité, pracné a nehodí se pro rutinní praxi. Měření sérového kreatininu proto stále zůstává běžnou metodou k odhadu renální funkce v rutinní klinické praxi. V tomto sdělení popisujeme cca 50% pokles sérového kreatininu měřeného enzymatickou metodou s kolorimetrickou detekcí podle návodu výrobce. Relativně rychlý pokles sérového kreatinu z 67 µmol/l na 30 µmol/l v průběhu 4 dnů byl pozorován u pacientky s cystickou fi brózou po oboustranné transplantaci plic, s náležitou imunosupresivní, antimykotickou i antibakteriální léčbou včetně aminoglykosidů (amikacin). Změny v hladině sérového kreatininu během 4 dnů (průměrný rozdíl 49, 25 %, při 95% intervalu spolehlivosti, -29 až -37 µmol/l) jsou významné. Tento jev přesto nemusel být odrazem lepší clearance vzhledem k nepřijatelně vysoké „trough“ hladině amikacinu a nejspíše poukazuje na možné snížení produkce kreatininu v souvislosti s cystickou fi brózou. V roce 1988 byla připravena komplexní sloučenina kyseliny ranelové a stroncia – ranelát strontnatý, který v dnešní době představuje jeden z významných léků při terapii postmenopauzální osteoporózy. Byl prokázán jeho významný vliv na posílení kostní denzity, který snižuje riziko osteoporotických zlomenin vertebrálního i periferního skeletu. Bezpečnost dlouhodobého užívání ranelátu strontnatého byla ověřena v mnoha studiích. V současné době existuje několik metod, kterými je možné hladinu stroncia v klinických laboratořích stanovit v různých biologických materiálech (sérum, plazma, moč). Nejčastěji užívané metody jsou atomová absorpční spektrometrie s plamenovou či elektrotermickou atomizací a hmotnostní spektrometrie s indukčně vázaným plazmatem. Sérové koncentrace stroncia u zdravých osob jsou < 50 µg/l. Pacienti léčení ranelátem strontnatým mají koncentrace stroncia 10463 ± 1376 µg/l. Stroncium je vylučováno hlavně ledvinami. Močové koncentrace dosahují až hodnot 24535 ± 20570 µg/l. Renální clearance je přibližně 5,33 ml/s. Vzhledem k variabilní individuální bioaviabilitě a compliance pacientů může být u některých vybraných pacientů léčených ranelátem strontnatým stanovení hladin stroncia důležité a přínosné., Since 1988 when a mixture of ranelic acid and strontium was prepared strontium ranelate, which represents one of the most signifi cant drugs for treatment of postmenopausal osteoporosis, the signifi cant effect of strontium on enhancement of bone density was confi rmed. This results in a decrease of osteoporotic fractures of vertebral and peripheral skeleton. The safety of a long-term intake of strontium ranelate was proved in many studies. At the present, number of methods exist which are capable of analysing the levels of strontium from different biological materials (serum, plasma, urine). Nevertheless, the most common methods as well as those used most often are atomic absorption spectrometry with fl ame atomization, atomic absorption spectrometry with electrometric atomization in the graphite furnace and inductively coupled plasma atomic emission spectroscopy. Serum strontium levels in healthy volunteers are < 50 µg/l. Average levels in patients on strontium ranelate therapy are 10463 ± 1376 µg/l.
